Abstract

Stroke merupakan penyebab kematian paling tinggi di dunia selain kanker, selain itu terapi stroke juga memerlukan biaya yang besar, sehingga pengatasan tepat sangat diperlukan untuk mengatasinya. Tujuan penelitian untuk mengetahui jenis obat, komponen biaya, dan rata-rata biaya obat terapi stroke perhari rawat inap di RS PKU Muhammadiyah Bantul Yogyakarta. Metode penelitian yang dilakukan adalah deskriptif dengan data retrospektif Januari-Juni 2015 yang memenuhi kriteria inklusi. Analisa biaya dilakukan melalui payers perspective yang difokuskan pada biaya medik langsung. Hasil penelitian menunjukkan dari 130 pasien yang memenuhi kriteria inklusi, 10,4% stroke hemorrhage dan 89,6% stroke non hemorrhage. Jenis obat terapi stroke yang banyak digunakan adalah neuroprotektan citicholine dan piracetam. Komponen biaya terbesar adalah biaya obat, stroke stroke hemorrhage 34.6% dan non hemorrhage 34.7%. Rata-rata biaya obat perhari rawat inap stroke hemorrhage Rp.65.323 – Rp.79.979 dan stroke non hemorrhage Rp.115.384 - Rp.119.723. Rata-rata biaya obat stroke per hari rawat inap pasien stroke hemorrhage dan stroke non hemorrhage menunjukkan tidak ada perbedaan yang signifikan (p>0.05). Kesimpulan penelitian ini adalah jenis obat yang banyak digunakan neuroprotektan citicholine dan piracetam, dengan komponen biaya terbesar adalah biaya obat 34.6% - 34.7% dari biaya total perawatan, serta tidak ada perbedaan yang signifikan (p>0.05) rata-rata biaya obat stroke per hari rawat inap pasien stroke  hemorrhage dan stroke non hemorrhage.

Abstract

Kepatuhan pasien dalam mentaati rekomendasi pengobatan stroke menjadi salah satu faktor yang mempengaruhi kejadian keterulangan stroke. Kepatuhan terhadap obat adalah kunci untuk pencegahan sekunder pada pasien dengan stroke. Kepatuhan pengobatan yang buruk dapat menyebabkan keterulangan, kecacatan, atau kematian pada penderita stroke. Penelitian ini bertujuan untuk mengevaluasi tingkat kepatuhan obat pasien paska stroke dengan kejadian stroke berulang. Penelitian cross-sectional ini melibatkan 60 pasien berusia antara 30 dan 85 tahun, yang semuanya sedang menjalani perawatan stroke rawat jalan di sebuah rumah sakit di Indonesia. Kejadian stroke berulang didefinisikan sebagai mengalami stroke sekunder dalam waktu 30 hari setelah kejadian stroke awal. Dari total partisipan, 12 individu telah mengalami stroke berulang setelah episode awal mereka, sementara 48 pasien tidak mengalami stroke berulang hingga saat pengumpulan data. Data kepatuhan obat dievaluasi dengan menggunakan kuesioner MMAS-8(Morisky Medication Adherence Scale). Hasil penelitian menunjukkan bahwa pada kelompok pasien paska stroke tingkat kepatuhan pasien tinggi 41,7% dan pada kelompok stroke berulang tingkat kepatuhan pasien sedang 41,7%

Abstract

The problem of tolerance in schools arises due to a lack of understanding of diversity, social, communication difficulties, and injustice in treatment so that the role of education is needed in shaping students who have an attitude of tolerance. The purpose of this study is to explore the level of understanding and practice of tolerance among students. This research is a quantitative study, with data acquisition through the survey method. The data collected will be analyzed descriptively to identify the level of understanding of students' tolerance attitudes in everyday life in the school environment. The results of this study indicate that although students at SMP N 1 Bontang are divided into several classes, and have religious differences, the students have a good level of understanding and practice of tolerance as evidenced by the results of the analysis assessment. This illustrates that the educational efforts and school environment at SMP N 1 Bontang was successful in promoting tolerance among students who have diverse backgrounds.

Abstract

Purpose-This study examines the effectiveness of the Jigsaw technique group guidance guide in improving students' career exploration at SMP Negeri Bontang. Career exploration is an important process in student development, especially at the junior high school level, where students begin to recognize their potential, interests, talents, and personal values while exploring relevant career opportunities. An effective career exploration process should be systematically designed through supportive services such as career guidance and interactive approaches, such as the Jigsaw technique, that encourage the development of communication, cooperation, and decision-making skills.Methodology-The Jigsaw technique can be a practical approach in group guidance services to support career exploration. This study used a quasi-experimental method with a control and experimental group design. The research subjects involved 30 students of SMPN 1 Bontang as the control group and 30 SMPN 3 Bontang as the experimental group. Data collection techniques used a career exploration scale questionnaire, while data analysis used the Wilcoxon Signed Rank Test and N-Gain test.Findings-The results showed that the effectiveness of the Jigsaw technique group guidance guide positively influenced students' career exploration. Based on calculations using the Wilcoxon Signed Rank Test, a significance value of 0.00 was obtained, below 0.05, indicating a significant difference between the pre-test and post-test results of the Jigsaw technique group guidance service. In addition, the calculation of the gain score shows a value of 0.67, which puts the guide's effectiveness in the medium category. Thus, this Jigsaw technique group guidance guide is declared effective enough to increase the career exploration of Bontang City Public Junior High School students.Significance- The implementation of group guidance services that support career exploration is often not optimal, mainly due to the unavailability of well-structured guidelines.Keywords: Group guidance guide, Jigsaw technique, Career exploration

Abstract

  Background: Stroke is a cerebrovascular disease that can cause physical and cognitive impairments, including a high risk of depression. Medication adherence plays a crucial role in preventing complications and improving patients' quality of life. Objective: This study aims to describe the therapy and evaluate medication adherence among post-stroke outpatients in the recurrent and non-recurrent stroke groups at RSUD Taman Husada Kota Bontang. Methods: This study employs a cross-sectional design, involving a total of 152 patients, consisting of 119 non-recurrent stroke patients and 33 recurrent stroke patients. Data were collected through interviews, secondary data retrieval from medical records, and primary data obtained from the MMAS-8 (Morisky Medication Adherence Scale-8) questionnaire to assess medication adherence levels. Data were analyzed descriptively, and a chi-square test was performed to examine the association between medication adherence and stroke recurrence. Results: The findings indicate that both recurrent and non-recurrent stroke groups were prescribed antiplatelet, anticoagulant, neuroprotective, and antihypertensive drugs. Additionally, lower medication adherence was more frequently observed in recurrent stroke patients compared to non-recurrent stroke patients (24% vs. 26%). Analysis of the MMAS-8 questionnaire items revealed that forgetting to take medication occurred in both groups, but was more common among non-recurrent stroke patients (23% vs. 9%). Conclusion: This study highlights the need for continuous monitoring of therapy profiles and efforts to improve medication adherence to prevent further complications in post-stroke patients. The findings emphasize the importance of ongoing evaluation and innovative medication education for post-stroke patients.

Abstract

Atrial fibrillation (AF) significantly increases the risk of stroke, necessitating anticoagulation therapy. Warfarin, a commonly prescribed anticoagulant regimen, requires careful monitoring to ensure patient safety. This study aimed to assess the impact of dose switching, dose variation, and potential interactions with warfarin on the incidence of stroke recurrence in stroke patients with AF. The study retrospectively analyzed the treatment records of stroke patients with AF in outpatient settings over one year. The subjects comprised 314 patients who received warfarin prescriptions at two Indonesian Hospitals from January 1, 2015, to December 31, 2019. Out of these patients, 50 had recorded data regarding dose adjustments, variations, and interactions. They were divided into two groups: a case group (n=11) with stroke recurrence and a control group (n=39) without recurrence. Statistical analysis, including chi-square tests and odds ratio calculations, revealed that both warfarin dose switching (OR=7.6) and dose variation (OR=6.6) significantly influenced the incidence of stroke recurrence. It implies that inconsistencies or alterations in warfarin dosing substantially elevate the likelihood of experiencing another stroke, potentially due to inadequate anticoagulation leading to clot formation. Interestingly, the analysis of drug interactions did not significantly impact stroke recurrence. In summary, the recurrence of stroke in patients with AF is notably influenced by warfarin dose adjustments and variations rather than drug interactions. This study highlights the critical importance of precise dosing strategies and vigilant monitoring to enhance the efficacy of anticoagulant therapy in this high-risk population.
