Abstract

Nyeri tenggorok pascaoperasi atau post operative sore throat (POST) terjadi karena iritasi dan inflamasi lokal di daerah faring, laring, dan trakea akibat trauma pemasangan pipa endotrakeal. Penelitian ini bertujuan mengetahui efek tablet hisap Strepsils® untuk mengurangi POST pada pasien yang dilakukan anestesi umum dengan intubasi pipa endotrakeal. Penelitian dilakukan di Rumah Sakit Dr. Hasan Sadikin Bandung selama bulan Oktober–November 2013, terhadap 66 pasien status fisik American Society of Anesthesiologist (ASA) I–II secara prospektif single blind randomized controlled trial. Pasien dibagi menjadi dua kelompok, yaitu kelompok I, sebanyak 33 orang mendapatkan Strepsils® dan kelompok II, sebanyak 33 orang mendapatkan plasebo. Pasien diminta untuk mengulum permen sebelum induksi. Data dianalisis dengan uji-t, uji chi-kuadrat, dan Uji Mann-Whitney. Keluhan POST lebih banyak terjadi pada kelompok II dibandingkan dengan kelompok I. Secara statistik pada T0 didapatkan hasil perbedaan yang bermakna (p<0,05). Pada kelompok II, POST dengan skor 1 sebesar 14 (42%) kasus, dibandingkan dengan kelompok I sebesar 3 (9%) kasus dan tidak ada skor 2. Pada T2 didapatkan hasil perbedaan bermakna (p<0,05), POST sebesar 11 (33%) pada kelompok II, sedangkan kelompok I didapatkan 2(6%) kasus. Pada T4 didapatkan perbedaan tidak bermakna (p>0,05), POST pada kelompok II sebesar 3 (9%) kasus, sedangkan kelompok I skor 1 sebesar 1 (3%) kasus. Sixty six patients, ASA I–II, who underwent elective surgery under general anesthesia and installation of endotracheal tube were enrolled in this single blind randomized controlled trial. Patients were randomly allocated into two groups of 33 subjects: receiving Lozenge and receiving placebo. Designated as Group I (Strepsils®  group) and Group II (Placebo group). Patients were asked to suck the candy slowly in the mouth before the induction of anesthesia.  Data were analyzed with t-test, Chi-square test and Mann-Whitney test using SPSS ver. 13 program for Windows. The results were POST 9.1% and 42.4% at T0, 6.1% and 33.3% at T2 for Strepsils®  and Placebo groups, respectively, which were statistically significant (p<0.05). In T4 and T24, non-significant differences were found (p>0.05). Abstract

Menggigil adalah peningkatan metabolism panas tubuh yang bersifat involunter melalui aktifitas otot dan terjadi ketika suhu permukaan tubuh lebih tinggi disbanding suhu preoptik hipotalamus. Menggigil dapat dipengaruhi oleh jenis dan lamanya tindakan pembedahan. Menggigil menjadi salah satu permasalahan serius karena dapat meningkatkan oksigen, metabolism tubuh, dan tekanan darah. Tujuan penelitian ini untuk mengidentifikasi suhu tubuh dan kejadian menggigil pada pasien mastektomi dengan anestesi umum intubasi di Instalasi Kamar Bedah RS Tk. II dr. Soepraoen Malang. Penelitian ini menerapkan metode deskriptif observasional dengan melakukan observasi langsung ke lapangan. Pengambilan sampel dilakukan dengan teknik total sampling. Analisis penelitian ini menggunakan teknik analisis univariat dan bivariat dengan menggunakan teknik uji hipotesa Chi-Square. Hasil penelitian ini menunjukkan bahwa sebagian besar pasien masuk kategori dewasa (20-60 tahun) sebanyak 28 pasien dan berdasarkan IMT sebagian besar pasien dalam kategori 18-25 sebanyak 19 responden. Observasi 30 menit pertama diperoleh hasil bahwa sebagian besar pasien mengalami hipotermi yakni sebanyak 19 pasien (52,8%) dan 30 menit kedua juga didominasi pasien yang mengalami hipotermi sedang yakni sebanyak 26 pasien (72,2%). Mayoritas pasien tidak menggigil sebanyak 25 pasien (69,4%). Hasil analisis menggunakan uji Chi-Square dan IMT pasien dibuktikan dengan nilai pearson Chi-Square usia 0,26. Hasil uji Chi-Square IMT = 0,01 membuktikan bahwa terdapat pengaruh IMT terhadap kejadian menggigil.

Abstract

ABSTRACT Introduction: Preoperative anxiety is a normal response to life-threatening situations and conditions for patients. Preoperative anxiety is often associated with fear of surgical procedures, anesthetic procedures, and information needs fulfillment factors. Assessment of the right pattern of anxiety can determine the appropriate management in dealing with anxiety. The purpose of this study was to determine the anxiety patterns of preoperative patients at Bhakti Dharma Husada Hospital, Surabaya. Methods: This study used a quantitative descriptive method with a cross sectional approach. The research subjects included preoperative patients at Bhakti Dharma Husada Hospital, Surabaya. The sample is 68 people. Measurement of anxiety using the APAIS scale (Amsterdam preoperative anxiety and information scale). Data analysis using Descriptive Statistical Test. Results: The majority of respondents were female (58.8%), aged 26 -35 years (44.1%), had high school education (63.2%), had a general anesthesia plan (63.2%), and had no surgical experience ( 73.5 %). The majority of respondents experienced moderate anxiety (47.1%), then severe anxiety (32.4%), mild anxiety (14.7%), panic (5.9%), and not anxious (0%). The majority of respondents have an element of anxiety related to the need for information. Discussion: The pattern of respondents' anxiety based on the APAIS instrument shows that the majority of respondents in this study experienced moderate anxiety, with elements of anxiety related to the need for information. Researchers suggest that health workers always carry out an anxiety assessment before surgery, so that they can provide appropriate management in reducing patient anxiety. Abstract

Introduction: Surgery involves invasive procedures using anesthesia, this can cause anxiety and fear in patients and their families. Anesthesia aims to relieve pain, fear and discomfort so that patients feel more comfortable during medical procedures. Pre-operative preparation is very important to ensure the patient's physical and psychological condition is ready to undergo anesthesia and surgical procedures. More than 50% of pre-operative patients experience anxiety worldwide, with 5-25% of them over the age of 20 years. In Indonesia, around 11.6% of the population experiences anxiety. Anxiety can cause delays in surgery, lengthen recovery time, and increase the risk of complications. Health education through videos about spinal anesthesia is considered an effective method because it can be done anytime and anywhere, and can increase patient knowledge and skills. The aim of this study was to analyze the effect of patient education using spinal anesthesia training videos on reducing anxiety levels. Methods: Prae experimental research design, one group pre-post test design, in the population of patients admitted via outpatient who will undergo surgery with spinal anesthesia totaling 30 respondents, the research was carried out in the waiting room of the operating room within a two week research period. Independent variable: provision of health education using video media about spinal anesthesia. Dependent variable: anxiety level. Data were collected using the observation method, carried out pre and post providing health education using video media about spinal anesthesia. The instrument uses The Amsterdam Preoperative Anxiety and Information Scale (APAIS) and is recorded on an observation sheet. Bivariate data analysis, namely by using the Independent T-Test of data, was carried out to determine the difference in the average value of anxiety levels between pre and post treatment, and the data obtained was processed using the SPSS 22 computer application. Results: Hypothesis testing, the significance value is 0.000, which is 0.000. which is less than 0.05. Conclusion: therefore the decision made for this test is that providing health education using video media about spinal anesthesia has an effect on reducing the anxiety level of patients who will undergo surgery with spinal anesthesia and the effect is very strong.  Keywords: Video Media, Health Education, Spinal Anesthesia, Anxiety Level.

Abstract

Menggigil adalah mekanisme pertahanan tubuh untuk melawan hipotermi. Menggigil berpotensi memberikan dampak buruk pada pasien termasuk peningkatan konsumsi oksigen, hipoksemia, dan memperparah nyeri operasi. Penggunaan cairan infus yang hangat merupakan salah satu strategi yang digunakan di instalasi bedah sentral (IBS) untuk membantu menjaga kenyamanan dan stabilitas suhu tubuh pasien. Penelitian ini bertujuan untuk mengetahui Gambaran pemberian cairan infus hangat terhadap pencegahan menggigil pascaoperasi pada pasien pembedahan Sectio Caesarea di RSU Mitra Delima Malang. Desain dalam penelitian ini adalah deskriptif kuantitatif, jumlah sampel sebanyak 50 responden yang diambil secara consecutive sampling. Instrumen yang digunakan adalah cairan infus hangat, jam tangan, termometer digital, dan lembar prosedur pemberian infus hangat. Alat pengumpulan data menggunakan lembar observasi kemudian data dianalisis secara deskriptif untuk menentukan frekuensi dan persentase dari kejadian menggigil pascaoperasi. Hasil penelitian didapatkan Derajat menggigil paling banyak adalah derajat 0 tidak menggigil sebanyak 42 responden (84%), derajat 2 sebanyak 2 responden (4%), derajat 3 sebanyak 5 responden (10%), derajat 4 sebanyak 1 responden (2%). Kejadian menggigil dari 50 responden yang diteliti, hanya terjadi pada 8 responden atau 16% yang menggigil, sedangkan pada 42 responden atau 84% responden tidak menggigil. Simpulan penelitian menunjukkan penggunaan cairan infus hangat efektif dalam pencegahan terjadinya menggigil pada pasien pascaoperasi sectio cesarea.

Abstract

Penggunaan general anastesi pada pasien laparatomi dapat menimbulkan kecemasan karena pasien akan kehilangan kesadaran selama operasi dan menghadapi ketidakpastian terkait proses anestesi dan efek sampingnya. Selain itu, ketakutan akan risiko dan komplikasi general anestesi juga dapat menyebabkan kecemasan yang tinggi pada pasien menjelang prosedur laparatomi. Tujuan : Menganalisis faktor kecemasan pada pasien praoperasi laparatomi dengan general anestesi. Penelitian ini adalah analitik korelasi dengan pendekatan cross sectional. Penelitian ini dilaksanakan di RST dr.Soepraoen, pada bulan Mei - Juni 2023. Populasi penelitian sebanyak 55 orang dan seluruhnya dijadikan sampel. Pemilihan sampel dengan teknik purposive sampling. Analisis data dilakukan secara univariat dalam tabel distribusi frekuensi dan analisis bivariat menggunakan uji korelasi Gamma pada taraf kepercayaan 95% (a = 0,05). Pola kecemasan pasien praoperasi laparatomi dengan general anestesi di RST dr.Soepraoen berhubungan signifikan dengan umur (p = 0,003, r = 0,714), jenis kelamin (p = 0,000, r = 0,762), pendidikan terakhir (p = 0,000, r = -0,753), dan pengalaman operasi sebelumnya (p = 0,000, r = -0,826). Umur, jenis kelamin, pendidikan terakhir, dan pengalaman operasi sebelumnya berhubungan dengan pola kecemasan pada pasien praoperasi laparatomi. Disarankan pada perawat anestesi untuk memberikan teknik relaksasi sederhana seperti pernapasan dalam, meditasi, atau visualisasi yang dapat membantu pasien mengelola kecemasan sebelum operasi.

Abstract

Fast and predictable recovery is essential in breast cancer surgery, yet comparative evidence on inhalation anesthetics within Indonesian clinical settings remains limited. This study compared the effectiveness of desflurane and sevoflurane on early recovery time after modified radical mastectomy. A cross-sectional design was applied to 76 ASA I-II female patients who received standardized induction, maintenance, and Aldrete-based recovery assessment. Recovery was categorized as fast (≤10 minutes) or delayed (>10 minutes). Statistical analysis used Chi-square testing, supported by effect size and risk ratio estimation. Desflurane demonstrated significantly faster recovery than sevoflurane (97.4% vs 78.9%, p = 0.014). The association showed a moderate effect size (Cramér’s V = 0.30), and patients receiving sevoflurane were substantially more likely to experience delayed recovery (RR = 8.3). These results align with recent studies highlighting desflurane’s lower blood-gas solubility and faster elimination profile. The homogeneous sample and standardized anesthetic protocol strengthen internal validity. However, the non-randomized design, purposive sampling, and absence of documented adverse events limit generalizability. Desflurane may offer practical benefits in improving post-anesthesia care unit (PACU) efficiency and surgical throughput in high-volume settings. Further multicenter randomized studies are recommended to confirm these findings and explore broader clinical implications.

Abstract

This quasi-experimental research aimed to compare the effects observed following the administration of ephedrine 10 mg and phenylephrine 50 mg administered as therapeutic bolus doses for the management of intraoperative hypotension due to spinal anesthesia in cesarean section. The study involved 60 parturients who developed intraoperative hypotension during spinal anesthesia, who were divided into two groups treated with either phenylephrine or ephedrine. Maternal blood pressure, including systolic and diastolic values, was assessed pre- and post- vasopressor administration and analyzed using descriptive statistics, normality testing, and independent-samples t-tests. Both vasopressors were associated with increased maternal blood pressure following intervention, with no significant differences observed at baseline. However, between-group comparisons revealed statistically significant differences in post-intervention systolic and diastolic blood pressure (p < 0.05), indicating distinct hemodynamic response profiles. These findings demonstrate the effectiveness of both ephedrine and phenylephrine in treating hypotension associated with spinal anesthesia; vasopressor selection influences maternal blood pressure outcomes. From a practical perspective, this study provides clinically relevant evidence to inform anesthetic decision-making in maternal healthcare services, particularly in settings where therapeutic bolus administration is routinely utilized for the management of hypotension associated with spinal anesthesia.

Abstract

Intraoperative hypothermia and post-anesthetic shivering (PAS) remain frequent complications in oncologic surgery under general anesthesia, yet evidence integrating intraoperative temperature changes with PAS severity in mastectomy patients is limited. This study aimed to evaluate intraoperative temperature profiles, determine the incidence and severity of PAS, and analyze the association between age, body mass index (BMI), and PAS among radical mastectomy patients. A cross-sectional observational study was conducted involving 36 women undergoing radical mastectomy with endotracheal general anesthesia. Core temperature was measured at 30 and 60 minutes intraoperatively, and PAS was assessed using the bedside shivering assessment scale (BSAS) upon admission to the recovery room. Moderate intraoperative hypothermia was observed in 72.2% of patients at 60 minutes, while PAS occurred in 30.6% of cases. Significant associations were identified between PAS and BMI (p = 0.001), as well as age (p = 0.026). Moderate-to-severe shivering (BSAS scores 2-3) was more frequently observed among underweight and elderly patients. This study provides novel evidence by linking intraoperative hypothermia patterns with PAS severity using BSAS in mastectomy patients. Clinically, the findings support structured perioperative temperature monitoring and the implementation of risk-based warming protocols, particularly for patients with low BMI and advanced age.
