Abstract

Human Immunodeficiency Virus (HIV) infection patients are often reported to be associated with changes of lipid metabolism.A previous study suspected that there was a correlation between low CD4+ T-lymphocyte counts with the lipid profile in HIV-infectedpatients. The objective of this study was to know the assessment of the lipid profile (total cholesterol, HDL, LDL and triglyceride) inpatients with HIV infection and their correlation with CD4+ T-Lymphocyte count. This study was conducted at the Hasan Sadikin Hospital,Bandung using retrospective data, and analysis method as the study design. The study was performed on 402 HIV-infected patients.A significant difference was found in all lipid parameters between patients with and without ART (p<0.05). Weak correlations werefound between CD4+ T-Lymphocyte with total cholesterol and HDL levels (r<0.04), and a very weak correlation as well with the LDLlevel (r<0.2). There was no correlation with the triglyceride levels (p=0.751). The lipid profile showed a weak correlation with CD4+T-Lymphocyte, therefore, it can not be used as a parameter to know the severity of disease relating to the HIV infection. However, it canbe useful for monitoring the metabolic effect of the given therapy, because there was a significant difference between those patients withand without ART.

Abstract

Background: The purpose of this study was to determine whether the cARV therapy correlates with the serum level of IL-6, ferritin, and hepcidin in HIV infected patients with ACD.Methods: A cross-sectional study conducted in HIV patients with ACD, aged 15-65 years with exclusion criteria such as chronic renal disease, chronic liver disease, treated for anemia in the last 3 months, taking iron supplements in the last 3 months, history of blood transfusion in the last 1 year, suffering from acute infection, tuberculosis infection, malignancy, hepatitis C virus infection, acute hypersensitivity reaction and pregnancy. Blood specimen examined by high sensitivity human IL-6 In vitro ELISA, DRG Hepcidin- 25 ELISA, serum ferritin by agglutination method and CD4 cell count by flow cytometry. Results: A total of 86 HIV patients with ACD consist of 42 subjects with experienced cARV and 44 subjects with naive cARV. There are several differences between cARV naïve and experienced cARV including: Hb (g/dl) [ 10.8 ( 9.88 – 11.75) Vs. 11.60 (11.00 – 12.34) p: 0.058 ], serum iron (mg/dl) [ 50.00 (31 – 66.8) Vs. 71.40 (56.00 – 108.00) p: 0.121 ), serum ferritin (ug/dl) [ 630 ( 194 – 1101) Vs. 195.40 (128.25 – 589.47) p: 0.008 ], IL-6 (pg/mL) [7.20 (3.88 – 10.76) Vs. 4.03 (1.69 – 8.49) p: 0.009), serum hepcidin (ng/mL) [45.54 (16.74 – 95.31) Vs. 23.35 (6.58 – 49.99) p: 0.007] , CD4 cell count (cell/uL) [31.5 (14.25 – 90.75) Vs. 307 (168.5 – 458.75) p : < 0.001)] There is a significant inverse correlation between the cARV therapy with level of serum IL-6 ( r: - 0. 285, p: 0.008), ferritin (r: - 0.321, p: 0.007) and hepcidin (r: - 0.293, p: 0.006). While there was a significant positive correlation between the cARV therapy with CD4 cell count (r: 0.676, p: 0.001). There was no correlation between the cARV therapy with serum iron levels ( r: 0.176, p: 0.121).Conclusion: There is an inverse correlation between the cARV therapy with serum level of IL-6, ferritin, and hepcidin in HIV patients with ACD.  Keywords: cARV, ACD, IL-6, Ferritin, Hepcidin.

Abstract

HIV/AIDS merupakan penyakit kronis yang memerlukan pengobatan dan perawatan seumur hidupnya. Dalam menunjang keberhasilan pengobatan dan perawatan tersebut perlu perawatan mandiri (self-care), terutama oleh Lelaki Seks Lelaki (LSL) karena LSL menggambarkan perilaku yang menempatkan mereka dalam resiko terinfeksi dan merupakan faktor risiko tertinggi penularan HIV/AIDS. Kemampuan individu dalam melakukan perawatan mandiri ini dapat dikondisikan oleh faktor internal yaitu faktor demografi. Penelitian ini ditujukan untuk menganalisis hubungan faktor demografi dengan self-care pada LSL dengan HIV/AIDS di Klinik Teratai RSUD Kabupaten Sumedang. Penelitian ini merupakan penelitian analitik korelasional dengan rancangan cross sectional. Pengambilan sampel menggunakan purposive sampling dengan jumlah 78 responden. Pengumpulan data primer dengan kuesioner melalui online survey menggunakan google formulir. Data di analisis dalam bentuk univariat dan bivariat menggunakan tendensi sentral dan uji korelasi pearson product moment. Hasil penelitian didapatkan rerata self-care responden tinggi (119.85). Hasil bivariat menunjukkan bahwa usia (r=0.636,p>0.05), status marital (r=0.838,p>0.05), pendidikan terakhir (r=0.149,p>0.05), pekerjaan (r=0.938,p>0.05), hubungan seksual (r=0.669,p>0.05), dan tipe berhubungan seksual (r=0.308,p>0.05) tidak memiliki hubungan yang signifikan dengan self-care. Responden penelitian ini (n=78) sebagian besar dengan usia yang masih muda (remaja dan dewasa awal) (n=67), belum menikah (n=72), pendidikan terakhir SMA (n=55), pekerjaan wiraswatsa (n=28), aktif dalam berhubungan seksual (n=51) dan bergonta-ganti pasangan seks (n=40), akan tetapi hal itu tidak berhubungan dengan perilaku self-care yang dilakukan oleh responden. Perawatan mandiri yang dilakukan oleh responden sudah tinggi (optimal) sehingga menjadi salah satu tujuan asuhan keperawatan, karena dapat merangsang partisipasi aktif pasien dalam pengobatan mereka, yaitu dengan berbagi tanggung jawab atas pelaksanaan dan hasil asuhan keperawatan antara pasien dengan perawat.
