Abstract

Background: Several studies have reported that curcumin exerts chemopreventive effects in various type of cancers, through several mechanisms, however, the effect of curcumin on carcinogenesis in patients with hydatidiform mole has not yet been investigated. This study was conducted to evaluate the effect of curcumin on apoptosis, proliferation, and nuclear translocation of endothelial nitricoxide synthase in trophoblast cells induced by estradiol in complete hydatidiform mole (CHM).Methods: In this in vitro study, trophoblast cells were divided into six groups, the control group (trophoblast cells were exposed to 100 pg/mL of 17-β estradiol) and the treatment group (trophoblast cells were exposed to 100 pg/mL of 17-β estradiol in the presence of curcumin with doses: 50, 100, 200, 400, and 800 µM). At the end of study, the cell proliferation was analyzed using MTT assay and apoptosis with TUNEL test in each group thropoblast cell. eNOS translocation was assayed using confocal laser scanning microscopy at the various dose of curcumin.Results: Curcumin at the doses of 200, 400, and 800 µM significantly decreased the proliferation and increased the apoptotic index in curcumin-treated group compared to those in the control group (p<0.05). All doses of curcumin treatment significantly decreased the nuclear eNOS expression compared to that in the control group. The three highest doses of curcumin increased cytoplasmic eNOS expression compared to that in control group.Conclusion: Curcumin inhibits the proliferation and modulates the apoptosis of trophoblast cells induced by estradiol in CHM involvement.

Abstract

ABSTRAKKehamilan dan persalinan menyebabkan perubahan mekanik dan denervasi otot-otot dasar panggul yang menimbulkakeluhan stres inkotinensia, prolaps organ panggul dan gangguan seksual sehingga menurunkan kualitas hidup pasienPenelitian ini dilakukan untuk mengetahui efek latihan otot dasar panggul   pada kekuatan otot dasar panggul. Desairandomized clinical trial dilakukan pada ibu 4 minggu pasca persalinan pervaginam yang memenuhi kriteria inklusi daeksklusi.    Pada  kelompok  eksperimen  dilatih  untuk  melakukan  latihan  otot  dasar  panggul,  sehingga  mereka  dapamelakukan latihan otot dasar panggul di rumah. Evaluasi kontraksi otot dasar panggul dengan perineometer dilakukadiawal  dan  setiap  4  minggu  sekali  selama  12  minggu.  Perbedaan  kekuatan  otot  dasar  panggul  pada  setiap  kelompodianalisis dengan paired t test, sedangkan perbedaan antar kelompok diuji dengan unpaired t test. Pada kelompok kontrotidak  didapatkan  perbedaan  kekuatan  otot  panggul  sesudah  4-12  minggu  bila  dibandingkan  kondisi  awal  (44.4)sebaliknya pada kelompok perlakuan didapatkan perbedaan bermakna. Hasil uji independent sample t test menunjukkaperubahan otot dasar panggul sesudah 12 minggu pada kelompok perlakuan lebih tinggi secara bermakna dibandingkakontrol. Penelitian ini membuktikan latihan otot dasaar panggul secara mandiri memberikan perbaikan kekuatan otodasar panggul sejak 4 minggu pertama hingga 12 minggu. Abstract

Objective: To evaluate the effects of calcium on the Sacrouterine Ligament by evaluating the expression of calmodulin and bFGF/FGF-2.Method: A true experimental study was carried out by assessing the cell cultures of sacrouterine ligament from female patients who underwent hysterectomy. The cell cultures were divided into groups that were exposed to calcium at different concentrations of 100 nM, 300 nM, 500 nM, 700 nM, and control group without any exposure. The expression of calmodulin and bFGF/FGF-2 was subsequently analyzed using immunofluorescence and ELISA method.Results: This study showed that exposure to calcium significantly affected calmodulin expression (p-value <0.05). The concentration found to be the most effective to induce calmodulin expression was at 500 nM. Calsium also significantly affected the bFGF/FGF-2 expression (p-value <0.05) with the concentration found to be the most effective to induce bFGF/FGF-2 expression at 500 nM.Conclusion: This study suggested that calcium had a significant positive effect of increasing extracellular matrix expression. A further in vivo study needs to be conducted in order to enhance the evidence of the potential effects to become a preventive agent for pelvic organ prolapse. Calcium is widely available in tropical countries like Indonesia, so this preparation is considered very easy for Indonesian women to apply.Key words: Calcium, calmodulin, bFGF/FGF-2, Extracellular MatrixPengaruh Kalsium pada Ekspresi Calmodulin dan bFGF/FGF-2: Studi In Vitro pada Ligamentum SakrouterinaAbstrakTujuan: Mengevaluasi efek kalsium pada Ligamen Sakrouterina dengan evaluasi ekspresi calmodulin dan bFGF/FGF-2,Metode: Sebuah penelitian true eksperimental dilakukan dengan menilai kultur sel ligamentum sakrouterurina dari pasien wanita yang menjalani histerektomi. Kultur sel dibagi menjadi beberapa kelompok yang diberi paparan kalsium pada konsentrasi yang berbeda yaitu 100 nM, 300 nM, 500 nM, 700 nM, dan kontrol tanpa paparan. Ekspresi calmodulin dan bFGF/FGF-2 selanjutnya dianalisis menggunakan metode ELISA dan imunofluoresensi.Hasil: Penelitian ini menunjukkan bahwa paparan kalsium berpengaruh nyata terhadap ekspresi calmodulin (p-value <0,05). Konsentrasi yang paling efektif untuk menginduksi ekspresi calmodulin adalah pada 500 nM. Kalsium juga berpengaruh nyata terhadap ekspresi bFGF/FGF-2 (p-value <0,05), dengan konsentrasi yang paling efektif untuk menginduksi ekspresi bFGF/FGF-2 adalah pada 500 nM.Kesimpulan: Penelitian ini menunjukkan bahwa kalsium memiliki efek positif yang signifikan terhadap peningkatan ekspresi matriks ekstraseluler. Penelitian in vivo lebih lanjut perlu dilakukan untuk meningkatkan bukti potensi efek sebagai agen pencegahan prolaps organ panggul. Kalsium banyak tersedia di negara tropis seperti Indonesia, sehingga olahan ini dinilai sangat mudah diterapkan untuk wanita Indonesia.Kata kunci: Kalsium, Calmodulin, bFGF/FGF-2, Matriks Ekstraseluler

Abstract

The levator ani, comprising the pubococcygeus, puborectalis, and iliococcygeus muscles, is crucial for supporting pelvic organs, maintaining continence, and ensuring pelvic stability. Weaknesses in these muscles or ligaments can lead to pelvic organ prolapse (POP), a condition where the pelvic organs descend and protrude through the vaginal introitus. Collagen and elastin, the key constituents of the extracellular matrix in the levator ani muscle, play a significant role in maintaining its structural integrity and can be influenced by parity. This study aimed to determine the differences in elastin and collagen levels of the levator ani muscle of primiparous and multiparous patients. This was an observational analytical study on 18 postpartum female patients consisting of 9 primiparous and 9 multiparous in January-March 2023. Samples were levator ani muscle biopsies from perineal lacerations of at least grade II from the inpatient Obgyn Department of the Faculty of Medicine, Universitas Brawijaya. Examination was done using the immunohistochemical method. Results showed that the percentage of histological area secreting elastin and collagen was higher in the primiparous group than multiparous, thus the levels were higher (p<0.001; p=0.001; p<0.05). In conclusion, elastin and collagen levels were lower in multiparous women compared to primiparous women. Future studies can evaluate factors affecting the decline in elastin and collagen in multiparous women using a larger sample size.
