Abstract

Hospital is one of the health service facilities that tries to overcome the health service balance between investment costs and the availability of applicable unit costs. Calculation of unit costs has an important role, including being used for budget planning, cost control, subsidies and as information in decision making. In the era of National Health Insurance (JKN) in Indonesia, Hospitals that cooperate with the Social Security Administering Agency (BPJS) Health are paid using the Indonesian Case Base Groups (INACBGs) package system. The acquisition of claim payments at Dr. Mintohardjo Naval Hospital according to the INACBGs rate compared to Hospital costs has a negative difference value for Bronchopneumonia cases. The purpose of this study was to analyze the difference in unit costs of Bronchopneumonia services based on Clinical Pathway towards the determination of INACBGs Rates and Hospital Rates at Dr. Mintohardjo Naval Hospital. This type of research is quantitative, comparative analytical observation using a cross-sectional research design according to the Hospital perspective. The subjects of the study were Medical Records and BPJS Kesehatan claim files for Bronchopneumonia services for the period April to September 2023, totaling 61 files. The study was conducted from January 2024 to April 2024. Data analysis used the paired t-test statistical method in the statistical test application (SPSS Version 26). The results of the study on calculating unit costs using the Activity Based Costing (ABC) method are as follows: 1) There is a difference between the calculation of unit costs with INACBGs rates of negative 6.2% and unit costs with Hospital rates of positive 11.2%, 2) There is a difference in the unit cost of Bronchopneumonia services based on Clinical Pathway with the Activity Based Costing method against INA-CBGs Rates, 3) There is a difference in the unit cost of Bronchopneumonia services based on Clinical Pathway with the Activity Based Costing method against Hospital Rates. The suggestions in this study are: 1) For the financial management of RSAL Dr. Mintohardjo to improve competence in calculating unit cost rates for services based on clinical pathways with the Activity Based Costing method, one of which is through training, 2) Updating and conducting clinical pathway audits periodically, 3) Re-evaluating hospital rates to increase the margin of each service so that the margin obtained is in accordance with what has been set, 4) Digitizing patient usage data that was previously still written manually to avoid errors in entering details of service costs.

Abstract

The Emergency Department (ED) plays a pivotal role as the frontline of hospital services, requiring rapid and accurate responses to ensure patient safety and optimal care outcomes. This study aims to analyze the influence of nurse competence and inter-team coordination on the effectiveness of patient transfers from ED to inpatient units, with nurse work motivation as a mediating variable. A quantitative approach was employed with hypothesis testing using path analysis (Structural Equation Modeling - SEM). The study was conducted at Graha Juanda Hospital, Bekasi, involving a sample of 55 nurses selected through total sampling from both the ED and inpatient departments. Primary data were collected through structured questionnaires, and data analysis was performed using SPSS 26. The results demonstrate that nurse competence and team coordination significantly affect service effectiveness, both directly and indirectly through work motivation. Specifically, high levels of nurse competence enhance the ability to assess, monitor, and manage patients efficiently, while effective team coordination fosters a collaborative environment that streamlines patient care transitions. However, work motivation plays a crucial role in mediating these relationships. The findings also indicate that low work motivation—linked to factors such as limited professional development opportunities, high workloads, and lack of recognition—contributes to delays in transferring patients to inpatient care, with the average Length of Stay (LOS) exceeding six hours. The study concludes that improving nurse competence, strengthening inter-team coordination, and enhancing nurse motivation are critical strategies to improve the quality and timeliness of emergency services. By offering continuous professional development, balancing workloads, and creating an environment where nurses feel valued, hospitals can significantly reduce delays in patient transfers. This integrated internal performance approach is vital for reducing LOS in the ED, ensuring timely care, and enhancing patient satisfaction. Furthermore, fostering a positive work environment that prioritizes both individual nurse growth and team collaboration is essential for sustaining high-quality emergency services and seamless, patient-centered care transitions.

Abstract

Healthcare-Associated Infections (HAIs) remain a critical challenge in hospital patient safety. In Indonesia, HAIs prevalence reaches approximately 15.74%, significantly exceeding the global average of 8–9%. At Ciputra Hospital CitraRaya Tangerang, HAIs incidence reached 1.2% in 2024 and already approached 1% within the first three months of 2025. A preliminary survey of 10 nurses revealed deficiencies across all competency dimensions, including knowledge gaps (80%), skill deficiencies in hand hygiene and patient isolation (75%), lack of commitment in infection prevention (70%), communication and education issues (75%), risk management deficiencies (85%), and low participation in IPC programs (85%). This study aims to analyze the influence of clinical leadership and teamwork on nurse competency in HAIs implementation, with patient safety culture as an intervening variable. A quantitative cross-sectional design was employed using Structural Equation Modeling – Partial Least Squares (SEM-PLS) with 122 nurses as respondents. Results demonstrated that clinical leadership, teamwork, and patient safety culture simultaneously influence nurse competency in HAIs implementation (R² = 0.52, p = 0.000). Clinical leadership has a significant positive direct effect on nurse competency (path coefficient = 0.341, p = 0.000), while teamwork showed no significant direct effect (p = 0.137). Patient safety culture emerged as the strongest predictor (path coefficient = 0.526, p = 0.000). Both clinical leadership and teamwork significantly influenced patient safety culture (0.394 and 0.321 respectively), and patient safety culture significantly mediated both relationships toward nurse competency. Strengthening clinical leadership and fostering a strong patient safety culture are the most essential strategies for improving nurse competency in HAIs prevention.

Abstract

RS at Kemayoran continues to experience low conversion of antenatal care (ANC) patients to delivery services, with a conversion rate of 6.9% in 2024. The maternity ward Bed Occupancy Rate (BOR) also remains below national standards. Preliminary assessments indicate dissatisfaction with the hospital’s healthscape, low mimetic desire reflected in limited recommendations, and suboptimal perceived value. This study examines the effects of healthscape and mimetic desire on childbirth location decisions, with perceived value tested as a mediating variable. Methods: A quantitative cross-sectional study was conducted using Structural Equation Modeling (SEM). Data were collected from 210 ANC patients at RSUD Kemayoran through structured questionnaires. Variables included healthscape, mimetic desire, perceived value, and delivery location decision. All instruments met validity and reliability criteria. Results: Healthscape and mimetic desire significantly influenced the decision to choose RSUD Kemayoran for childbirth. Healthscape did not significantly affect perceived value, whereas mimetic desire showed a significant effect. Perceived value had a significant direct effect on delivery decisions but did not mediate the relationships between healthscape or mimetic desire and delivery choice. Healthscape emerged as the most influential determinant. Conclusion: Physical healthcare environments play a critical role in childbirth location decisions. Improving healthscape quality and strengthening professional and social recommendations may enhance ANC-to-delivery conversion rates in public hospitals.

Abstract

Increased turnover of employees, particularly nursing staff, in healthcare institutions, especially hospitals, can reduce service quality and even result in losses. The best single predictor of employee turnover behavior is turnover intention. Employee turnover can be avoided by lowering the turnover intention. This study intends to investigate the effect of workload, compensation, and clan organization culture on turnover intentions. The study employed a quantitative approach as well as a questionnaire-based survey method. The population consists of the entire nursing staff at the Sentra Medika Sanggau Hospital. Total sampling is used in this study, with a total sample of 47 respondents. Multiple linear regression analysis is used in data analysis methods. The workload, compensation, and clan organization culture both simultaneously or partially have significant effects on the turnover intention of the nurses at Sentra Medika Sanggau Hospital. Implications for managers: migration of manual medical records to electronic medical records, enhancement of compensation, and enhancement of clan organization culture implementation

Abstract

Patient experience reflects patient’s comprehensive perceptions of service quality, the physical environment, and communication with healthcare providers. This study was motivated by the need to foster positive patient experience in the Pharmacy Installation of Graha Sehat Hospital, Probolinggo through improvements in service quality, servicescape, and effective communication. The objective of this research was to analyze the effects of service quality and servicescape on patient experience, with effective communication serving as an intervening variable. A quantitative approach was employed using Structural Equation Modeling (SEM) with AMOS. A total of 180 outpatient pharmacy patients were selected through voluntary sampling, with the sample size determined based on Hair’s method. The findings indicate that service quality, servicescape, and effective communication simultaneously exert a positive and significant effect on patient experience. Partially, service quality has a positive and significant effect on patient experience, while servicescape does not have a direct significant effect on patient experience. Effective communication, however, demonstrates a positive and significant influence on patient experience. Furthermore, effective communication significantly mediates the relationship between servicescape and patient experience, but it does not mediate the relationship between service quality and patient experience. The managerial implications for hospital management highlight the need to strengthen service quality improvement strategies, optimize spatial layout and environmental comfort in the pharmacy setting, and establish concise communication standards that include essential medication counseling points. Such measures are necessary to ensure that communication quality remains consistent, even during peak patient volume periods with limited-service time.
