Abstract

Background: Complete dentures can improve the quality of life of edentulous patients. The selection of a suitable articulator and an impression technique is important to construct a stable and retentive complete denture with good occlusion. The use of a semiadjustable articulator ensures that the dentures can be constructed such that their movement closely resembles the patient’s physiological movements. The effective suction method can ensure the development of the border seal according to the patient’s anatomical condition. The use of a semi-adjustable articulator along with an effective suction method is expected to provide good results in the construction of a complete denture. Purpose: This report aimed to describe the management of individual complete dentures in the flat mandibular ridge using a semi-adjustable articulator along with an effective suction method. Case: A 69-year-old female patient came with a chief complaint of her old dentures being unusable and wanted new dentures made. The patient’s general condition was good, and the last extraction was done three months prior to the patient’s arrival. The mandibular posterior alveolar ridge showed dextral tapering and sinistral flatness. Case Management: A complete denture with an acrylic base was fabricated using a semi-adjustable articulator along with an effective suction method. Conclusion: Flat ridge case management using a semi-adjustable articulator with an effective suction method can improve complete denture retention and stability.

Abstract

Prosthetic rehabilitation of a partially edentulous patient can be established by using wide range of treatment opti-ons. Magnetic attachment retained denture has always been considered beneficial for the patient, because it is gi-ving a more esthetic and functional outlook to the denture. The following case report discusses removable partial denture with no clasps showing by using magnetic attachment retained, thus increase esthetics. A 60-year-old male patient came to Department of Prosthodontics, Airlangga University, for his esthetic problems and chewing inability. Intraoral examination revealed teeth 14, 13, 12, 11, 21, 22, 23, 24, 28, and 46 remained at lower jaw. He never had a denture and needed to make a new one. Clinical and radiographic examination along with the preliminary impress-ion were taken. Teeth preparation and impression for magnet keeper, then cementation of magnet keeper on teeth 12 and 24. Functional impression for upper and lower jaw with individual tray. Denture delivery and one week after followed by insertion of magnet EX 600 in the denture. It was concluded that magnetic attachment retained partial denture could enhance the natural looking for the patient because it has no clasps showing.

Abstract

Background: Metal frame denture design is common used for partially dentate patient, one of the problem was a metal appearance from wrought wire or a metal casting clasp as retainer, which is not really aesthetic. Telescopic overdenture with its double crown will be the best solution for those cases. Purpose: The purpose of this case report was to reported telescopic overdenture design and fabrication where the aesthetic was the main consideration. Case: Sixty years old female patient, a businesswoman, came to Dental Hospital of Universitas Airlangga, complained about her upper partially dentate and 6 years lower fixed partial denture, which is not comfortable to wear and her upper anterior teeth doesn't look quite good. She wanted to make crowns and removable denture. Case management: After the diagnosis were made, the pretreatment plan were remove crown for the upper jaw, remove the bridges on lower jaw, and transitional dentures were made to maintain the VDO. The crowns and veneer were fabricated and inserted while the inner copings were tried up on prepared teeth. After the the dentures were produced, the inner copings were luted to the abutment teeth, and then the dentures were inserted using FGP (Friction GriP) cement for the friction effect. Discussion: Telescopic overdenture is consists of a double crown system known as "the telescopic”, the procedure involves fitting the remaining natural teeth with inner metal crowns, followed by outer crowns as part of an over denture that can be removed by the patient. Conclusion: This technique ensures that telescopic overdentures give natural aesthetic result.

Abstract

Background: Edentulousness resulting from extraction of teeth often leads to psychological problem and social isolation of the patient. The patient has to adapt to the situation with respect to speech, chewing, swallowing, and so forth. A smooth transition from dentulousness to edentulousness is the result of immediate over denture with a few retained teeth. Purpose: to report overdenture prevents alveolar ridge bone absorption, stabilizes a denture, and makes the typical sensory function to be looked closer to natural teeth. Case: A 79-year-old female complaint of missing multiple teeth and desire to replace them. Extraoral examination and temporomandibular joint movement were normal. Five teeth (12,11,21,22,34) exhibited grade II mobility with diagnosis apical chronic periodontitis. Radiographic examination is an overview of diffuse radiolucent on the apical on the teeth 17,13,12,11,21,22,34. Case Management: First, make a diagnostic impressions of the maxillary and mandibular with irreversible hydrocolloid then poured in dental stone. After that, the final impressions were taken and poured in dental stone, casts mounted on articulators. Bite registration was done after using bite rim and bite registration material. The next step is setting up the artificial teeth then tried to the patient. Five teeth (12,11,21,22,34) were trimmed from the casts on the functional model. The patients tooth 12, 11, 21,22 and 34 were reduced to a dome shape only 2 mm above gingival margin then restored with GIC on the surface. The denture was then placed immediately after reduced. Discussion: The selected abutments teeth were reduced to a dome shape only 2 mm above gingival margin. This will create adequate space for the overlying artificial denture tooth and denture base. Also, it reduces the lateral stresses and lever action of the tooth. The retained tooth roots, used for overdenture, transfers occlusal forces to the alveolar bone through the periodontal ligament and maintain alveolar ridge morphology. Conclusion: Overdenture can be used to prevents alveolar ridge bone absorption, stabilizes a denture, and makes the typical sensory function to be looked closer to natural teeth.

Abstract

Background: Complete denture in the lower jaw often has problems. The entire loss of the mandibular teeth often has a large resorption, so it is difficult to obtain retention and stability. Purpose: To report mandibular  magnet retained complete overdenture is can increase retention and stability in the lower jaw. Case: A 60-year-old man came to the Prosthodontics Clinic of RSGM Airlangga University with complaints of the old complete denture that made by previous dentist, is often unstable while talking and chewing, so the patient feel uncomfortable. The denture has been used for approximately four years. Therefore the patient wants to make a new complete denture. Case Management: PFM crown removed and 31,32,33,41,43 are treated with root canal treatment. Magnet keepers are fabricated on teeth 33,43, and metal copings are fabricated on teeth 31,32,41. The complete lower denture is attached to the magnet. Acrylic material denture are chosen. Discussion: Overdenture is a complete or partial denture that is placed above the tooth structure or the root of the original tooth which is the support of the denture. Overdenture treatment can maintain ridge height, reduced load of supporting elements, and avoid mucosal irritation. It's necessary to add retention in the form of magnets to the teeth 33 and 43 to get good retention and stability. Conclusion: Mandibular  magnet retained complete overdenture is can increase retention and stability in the lower jaw so that the patient feels satisfied and comfortable because the dentures are more retentive and stable.

Abstract

Background: Immediate complete overdentures are designed for placement denture immediately following the extraction or decapitation of natural teeth. This procedure overcome the challenges associated of esthetic problems, including functional difficulties and psychological issues. Purpose: The article aims to highlight the advantages of endodontic-prosthodontic approach through immediate overdenture, such as the preservation of alveolar bone and improvement in patient’s psychological well-being and quality of life. Case: A 60-years-old female came into Universitas Airlangga Dental Hospital with the chief complaint of missing multiple teeth with upper front teeth becoming loose and desire to replace them because of difficulty in chewing food. Case Management: Diagnostic impressions were taken using irreversible hydrocolloid material, then poured in the dental stone to create final impression. The treatment plan included endodontic treatment, scaling and root planning for 11. The vertical dimension of occlusion was established using a bite rim and mounted on an articulator. Then, the immediate overdenture was created by arranging artificial teeth. Before the denture insertion, 11 was decapitated and filled the orifice with glass ionomer cement. Extraction of 21, 22, 23 was carried out and after ensuring hemostasis, the immediate overdenture was inserted. To finalize the denture insertion, occlusion was assessed using articulating paper. Control was done after day 1,3 and 7 after denture insertion. Conclusion: This case report describes the effectiveness of immediate overdentures as a valuable treatment option through endodontic-prosthodontic approach. By providing a functional and aesthetic solution while minimizing the edentulous period, immediate dentures significantly enhance patient’s quality of life.
