Abstract

Introduction: The prescribing of non-steroidal anti-inflammatory drugs (NSAIDs) as pharmacological therapy for OA was found to be imprecise. In 2018, the rationality of NSAIDs prescribed at Wolter Monginsidi Hospital, Manado showed as much as 100% proper indication, 77% proper dose, and 89% proper medication. This shows that the rationality of NSAIDs prescription is still irrational. Irrational prescribing has the potential to result in side effects or the incidence of drug-related problems Aim: To determine the characteristics and the rationality of NSAIDs prescriptions in elderly knee OA. Methods: This is a descriptive study of 108 medical records for the period January 2020-April 2021 at BS Clinic, Semarang, using the purposive sampling method. Results:  Results showed the characteristics of elderly knee OA patients at BS Clinic were 74.1% women, 47.2% belonging to an obese group, and 41.7% experiencing severe pain. The patient’s age had a median value of 66 years. In the number of comorbidities, the median value was 1. The rationality of NSAIDs prescription was 95,4% proper diagnosis, 100% proper drug, 97.2% proper dose, and 99.1% proper indication with the incidence of side effects of NSAIDs prescribing was 1.9%.

Abstract

Background and Aim: Insomnia and anxiety are common disorders with a high prevalence rate, especially in medical students. Moreover, many confounding factors play a role in influencing anxiety. Most studies do not include or measure other confounding factors of anxiety in the analysis. This requires research that examines insomnia and anxiety assessing the contribution of the factors influencing anxiety.Methods: A cross-sectional study methodology with linear regression analysis on a sample of students from the Faculty of Medicine, University of Pelita Harapan using the DASS-21 questionnaire to assess anxiety and ISI to assess insomnia.Results: Prevalence rates of insomnia was 34.29% and anxiety 32.38% in Pelita Harapan University medical student. From the results of the bivariate linear regression, a weak relationship between insomnia and anxiety (r = 0.322; p = 0.000) is seen. In multivariate results, there were confounding stress variables (r = 0.565; p = 0.000) and trauma (r = 0.250; p = 0.005) which contributed greatly to anxiety (In order of largest).Conclusion: There is a weak correlation and anxiety. Moreover, the high prevalence rate of anxiety in medical students may be contributed heavily not only by insomnia but also by stress and childhood trauma.

Abstract

Peningkatan konsumsi minuman kemasan (soft drink/beverages) tampak nyata di dunia, termasuk Indonesia. Peningkatan ini terutama terjadi di kelompok usia muda. Konsumsi soft drink/beverages ini berkaitan dengan gangguan metabolik, antara lain obesitas, toleransi glukosa terganggu (TGT) dan diabetes melitus. Hal ini disebabkan penggunaan pemanis tinggi fruktosa (high fructose corn syrup, HFCS). TGT terutama menjadi masalah kesehatan yang cukup serius karena umumnya tidak menunjukkan gejala dan terlambat untuk dideteksi sehingga sering kali telah berprogresi menjadi diabetes mellitus. Terdapat banyak faktor yang mempengaruhi terjadinya TGT dengan faktor lingkungan menjadi faktor yang lebih dominan. Penelitian ini dilakukan dengan desain potong lintang yang menggunakan data riset kesehatan dasar (RISKESDAS 2007). Analisis data dimulai dari univariat, bivariat dan dilanjutkan dengan multivariat. Ditemukan bahwa prevalensi TGT pada kelompok usia muda di Indonesia adalah 5,7% dan prevelensi pengkonsumsi fruktosa tinggi sebesar 20,5%. Kontribusi konsumsi fruktosa tinggi terhadap kejadian TGT usia muda adalah 24,3%. Secara umum, prevalensi TGT pada usia muda di Indonesia cukup tinggi (melebihi 50% prevalensi di semua umur), dan konsumsi fruktosa memiliki kontribusi yang signifikan dalam terjadinya TGT.Kata kunci: konsumsi fruktosa, indeks fruktosa, TGT

Abstract

Urinary tract infection (UTI) is one of the most common types of infection among female employees. Some of the factors that make female employees more susceptible to UTI are the anatomy of the female urinary tract that is different from men, such as the urethra being shorter and closer to the anus, making it easier for bacteria to ascend into the urinary tract. In addition, sexual activity, lifestyle and work environment can also play a role in increasing the risk of UTI in female employees. UTI is a common health problem among women and is often found in primary care. UTI in female employees can cause physical discomfort and negatively impact work productivity. Holistic management and a family medicine approach can provide significant benefits in the management of UTIs especially in female employees who generally due to work demands often hold back urination and lack of drinking water. In the family medicine approach, doctors involve the patient's family in planning and supporting the patient's care. Families and patients are given information about UTIs, symptoms to watch out for, and preventive measures that can be taken. In addition, holistic management also includes psychosocial aspects by providing emotional support to patients and families. Through the family medicine approach and holistic management, it is hoped that patients will gain a better understanding of their condition, experience symptom improvement so that patients do not experience chronic UTI and prevent complications so that they do not need to be referred to secondary care. This approach also helps build a strong relationship between the patient, family and family doctor, which contributes to long-term management and prevention of recurrent infections.

Abstract

Major depression is a significant psychiatric disorder and affects an individual's quality of life. This article describes the comprehensive management of a 24 year old woman who experienced severe depression after experiencing problems in aspects of her closeness with her family. Management involves a multidisciplinary approach including psychiatrists, clinical psychologists, and family support. This case shows the importance of holistic and collaborative treatment in dealing with major depression. In addition, this article also highlights the important role of public education in reducing the stigma associated with mental disorders and increasing awareness of the importance of mental health.

Abstract

Internet Addiction Disorder (IAD) is an increasing public health problem among adolescents. As a branch of medical science that plays an important role in handling public health problems, family medicine has a unique perspective in understanding and seeking solutions to the problem of IAD in adolescents. This case report aims to explain the family medicine approach in managing IAD in adolescents, including the identification of risk factors, holistic interventions, and the role of family physicians in preventive, curative, and rehabilitative efforts.  The case study of a 24-year-old adolescent with IAD, using a family medicine approach, obtained an in-depth understanding of the psychological, social, and physiological dynamics underlying IAD in adolescents is key for family doctors in designing effective interventions. A family medicine approach that focuses on the unique needs and context of each adolescent patient can provide more comprehensive solutions. Family physicians play an important role in early screening, education, referral to mental health services, and collaboration with parents, schools, and communities.  Preventive efforts are also made through the promotion of healthy internet use, development of self-regulation skills, and family/community empowerment. The family medicine approach in managing adolescent IAD places doctors as agents of change who focus on promotive, preventive, curative, and rehabilitative efforts. Collaboration between the government, professional associations, and other stakeholders is needed to develop an effective framework to prevent and address IAD in adolescents.

Abstract

Introduction: Sleep is one of the basic human needs, where human uses one third of their time only for sleeping. Some contributing factors towards bad sleep quality are stress, depression, surrounding noise, and the use of a bright light. It is known that white lights cause the activation of melanopsin, causing circadian cycle shift, whereas using yellow light does not. The aim of this research was to analyze whether there is an effect of using either white or yellow light towards sleep qualityMethod: This is RCT on 81 subjects. Sleep quality was measured using PSQI questionnaire on 3 measurements with each measurement taken every week. Result of the analyzed data using the General Linear Model – Repeated Measurement (GLM-RM) method Result: There were significant differences on PSQI score between group that used white light during sleep (7.6 ± 2.3) in comparison to the control group (5.2 ± 1.8) (p = 0.02), but result also showed that there were no significant difference between the group that used yellow light (6.1 ± 1.7) during sleep in comparison with the control group (p = 0.14). Conclusion: There is a significant difference between the uses of white lights during sleep towards sleep quality, but there has not been any significant difference between the uses of yellow light during sleep towards sleep quality

Abstract

Indonesia's Health Development, among others, aims to increase the awareness and ability of individuals and communities to live behavior patterns and a healthy environment. It is stated in the policy that health development requires effective and efficient efforts, resources and management in maximizing the degree of public health. Various characteristics and criteria of the community will require different health management. For example, in the late adult and elderly age categories, extra efforts and management are needed in maintaining a healthy, quality and productive life. Urban communities living in urban areas tend to have a more modern orientation of education and cultural values, but on the other hand, they face the dynamics of socioeconomic factors and environmental changes that affect the degree of well-being and individual health. One of the efforts to achieve a maximum degree of health is done through health promotion and examination. This activity has been carried out in urban and sub-urban Tangerang, involving 40 residents (10 men and 30 women) with an average age of 59.63 years. The results of blood pressure checks showed that only 11 residents were within the normal range, while the average cholesterol level was 208.39mg/dL; uric acid 7.89mg/dL; blood sugar 113.65mg/dL. This Community service activity recommends continuous and structured health checks in order to help achieve the degree of community health.
