Abstract

Performance is the result of quality and quantity of work accomplished by each employee based on the jobdescription or responsibility. The standard of a good performance are at least precision, intelligence, quickness, and orderliness. Kalisat Public health center has a problem which is underoptimal performance in providing service to patients so that affecting to the pastient’s satisfaction. This research was aimed to analyze employee’s performance within outpatient service in Kalisat Public health center Jember district. The method is analityc observational cross sectional design as the research method. There were 20 employees in outpatient service as the research sample obtained by saturated sampling. It was analyzed by applying the data tabulation software. This result showed that most of the employees were average in their good motivation (60%), they were good at the oppurtunity (50%),they were good at the ability (55%), and the performance of outpatient service belongs to a good category (50%). It shows that there were significant relationship between motivation, oppurtunity, ability, and performance of outpatient employees with significancy value < 0,05 and  X2 value > X2 table. The improvement efforts can be made by developing information system, division of labor improvement, good delegation of competency, and increasing training.

Abstract

The medical resume form is a summarize of all the patient's treatment that is conducted by healthcareworkers and signed by a doctor. The filling completeness of the medical resume at RUMKITAL Dr.Ramelan Surabaya only reached 82.5%. The purpose of this research was to analyze e the completenessof medical resumes in RUMKITAL Dr. Ramelan Surabaya. This research was qualitative descriptive. Thedata collection used observation. The sample of this research as many as 20 medical resume forms inFebruary of 2020. The research results indicated the highest percentage of patient identification reviewincompleteness is 45% which is the inpatient’s rank or class item. The highest percentage ofincompleteness on important report's review is 55% which is the inpatient’s duration item. The highestpercentages of incompleteness on authentication are the patient's date item, the filling time, the patient’sor family’s name, also the patient’s or family’s signature. The highest percentage of incompleteness oncorrect documentation review is 20% which is the plain and legible registration. In conclusion, the highestincompleteness is in the identification review and the correct documentation. The research suggestion is tocreate the SOP of medical resume, the SPO socialization, and to sign a commitment about theconsistency of filling medical resumes.

Abstract

Completeness informed consent form of RUMKITAL Dr. Ramelan Surabaya is still less than the MinimumHospital Service Standard of 100%, it is based on a preliminary study of researchers that the incompleteinformed consent form on the identity component is 60%, important reporting is 80%, authentication is40%, and the correct documentation is 0%. The purpose of this study was to conduct a quantitativeanalysis of the completeness of the informed consent form of RUMKITAL Dr. Ramelan Surabaya. Thisstudy uses descriptive research with a cross-sectional approach to identify the completeness of filling outinformed consent. Data collection used is observation and documentation on the informed consent form.The results show the identification component with the highest completeness by 90% and the highestincompleteness by 55%; The important reporting component with the highest completeness is 80% andthe highest incompleteness is 80%; The authentication component with the highest completeness is 90%and the highest incompleteness is 50%; The correct documentation component with the highestcompleteness is 85% and the highest incompleteness is 50. Some suggestion to fix the problem is bymaking the Statement of Commitment, the SOP of evaluating and monitoring the medical recorddocuments completion, improving the Informed Consent SOP and Medical Records Completion SOP byadding completeness standard of informed consent filling which is must be achieved by RUMKITAL Dr.Ramelan Surabaya.

Abstract

The first time retention at RUMKITAL Dr. Ramelan Surabaya were conducted in 2008. Based on data patient visits of Dr. RUMKITAL Ramelan Surabaya from 2015 to 2019 had increased it caused the storage shelf could not accommodate medical record files. SPO (Standard Procedure Operational) about implementation of retention and destruction of medical records in Dr. RUMKITAL Ramelan Surabaya not found provisions of the storage inaktif medical record file. The aim of this research was to analyze the implementation of retention and destruction medical record files in Dr. Ramelan Surabaya with identifying using 5M (Man, Money, Material, Machine, Method) and the USG method (Urgency, Seriousness, Growth) to determine the priority of problem. This research is qualitative research and collecting the data using observations, interview, and documentation. The subject of study is 1 scan officer of medical record and 2 officer of medical record. The results of this research is got 3 priority cause of problem the implementation of retention and destruction of medical record are elements of method, material and machine. The suggestion of this problem are improving the SPO (Standard Procedure Operational) and implementation the system information of retention.

Abstract

Based on a preliminary study at RUMKITAL Dr. Ramelan Surabaya in March 2020, there was a problemwith the unavailability of medical record files in the filling due to files not returned from poly, and there weremisfiles. It caused longer file provisioning time and delayed service in poly because the officer needs timeto search files that did not exist in storage. The purpose of this study was to determine the factors causingthe unavailability of medical record files in storage. The type of research used a qualitative method. Datacollection techniques used interviews and observation. The results obtained; the factors that causeunavailability of medical record files in the filling seen from 5M factor were as follows. Based on man factor,there is education and officer work experience. The money factor, there is no special fund for providing lostfile folders. The method factor, there is medical record missfile. The material factor, there are broken filefolders so that the contents of these folders could be separated from the file folders. The machine factor,tracer utilization is not optimal. So, the suggestion is to add the color code on the numbering section ofmedical record folder so that it will be easier to store and align medical record documents

Abstract

The online registration system is important component to improve hospital service quality. Regional GeneralHospital K.R.M.T. Wongsonegoro City of Semarang has implemented online registration system since 2017and never been evaluated. An online registration system that is often used by patients is via the MYRSWNapplication, can be downloaded at the play store and app store. Online registration system users in 2019totaling 15,437 users from 199,188 patients. Based on data, it can be seen that users of the online registrationsystem are only 8% of the total number of patients. This shows the user's interest in using the onlineregistration system is very lacking. This research was aimed to analyze the acceptance of an onlineregistration system. The research showed that overview of the online registration system based on aspects ofperceived ease of use can be assessed as easy to learn, quite flexible and easy to use, aspects of perceivedusefulness is considered to have considerable benefits in increasing productivity, making work effective andfast, last aspects of behavioural intention to use tend to be satisfied using the online registration system.Based on aspects of perceived ease of use, perceived usefulness,and behavioural intention to use applicationof an online registration system has been running according to user needs, but it is not free from variousproblems, so there needs a efforts to improvement and development of online registration system through therecommendation of researchers so that online registration system can maintain quality and improve servicequality to patients.

Abstract

RSUD K.R.M.T Wongsonegoro Semarang City is one of the hospitals that have implemented an electronic medical record at the end of December 2017. Information systems that support management and service to patients are electronic medical records, especially in outpatient coding units. One factor that plays an important role in the successful application and use of information technology is the user factor. The level of user readiness to accept information technology has a major influence in determining the success or failure of the application of the technology. The purpose of this study was to evaluate the application of electronic medical records in outpatient coding units based on the Technology Acceptance Model (TAM) method. This type of research is qualitative research with the data collection methods, namely interviews, observation and documentation. The subjects of this study were outpatient coding officers. The results showed that based on the aspects of usefulness in the use of electronic medical records in outpatient coding units be very beneficial for officers. Based on the aspect of ease in showing the acceptance of electronic medical records, it is considered easy to understand, flexible enough with the work of officers and easy to used to assist with work. Based on the aspect of interest, it is obtained that outpatient coding officers also show their interest in the system, and plan to use the system in the future, although there are some obstacles. The solution obtained from the problem is making guidelines for the use of electronic medical records, adding electronic signature features to electronic medical records for patient medical resume forms, striving for electricity and electricity networks to remain stable, making updates or updating electronic medical records.

Abstract

Polyclinic at RSAL dr. Ramelan Surabaya which has the most number of patient visits every day is poly surgical oncology than other poly surgery which is ± 55 - 60 patients, here researchers see that with the many requests for medical record files, the supply of medical record files must be as fast as possible and must still meet existing standards. But in reality the supply of medical record files still exceeds the standard. The results of the observations I made to search for 1 file took 5 minutes until the file was recorded in the register of each poly, aiming that the file with no. The RM has been sent and then barcoded or sent via SIMRS, it should be marked that the medical record file must already be in the clinic but here the file is still placed on the table very long which is about 15 minutes and can also be more than it should be sent to the polyclinic. That way the file does not get to the clinic according to the Hospital Minimum Service Standards (SPM). The aim is to find out how long it takes to provide medical records in outpatients for poly surgical oncology patients at RUMKITAL Dr. Ramelan Surabaya. This type of research is qualitative. Data collection techniques in the form of interviews and observations. The population in this study were all outpatient medical record files for oncology surgery patients in March 2020. The results obtained from the study were the length of time the provision of outpatient medical record files for old poly surgical oncology patients exceeded the established standard that required an average time 11 minutes for each file arrived at the polyclinic.

Abstract

Each hospital is required to hold records or records of all services provided to patients called medical records. Increasing the effectiveness of recording medical data that is accurate, fast, can take advantage of current technological advances through the implementation of the Electronic Medical Record (EMR) system in hospitals. RSUD K.R.M.T Wongsonegoro is one of the hospitals that has implemented an electronic medical record (EMR) especially in the inpatient and emergency department registration units. The application of this electronic medical record still has some flaws that must be evaluated. This study aims to evaluate the implementation of electronic medical records at the emergency and inpatient registrations of Wongsonegoro Hospital. Data collection in this study consisted of observations and interviews. Identification of problems using the TAM (Technology Acceptance Model) method by reviewing from 3 aspects namely perceived usefulness, perceived ease of use, and behavioral intention to use. This type of research is descriptive qualitative by narrating the results of research based on the data obtained. The results obtained from this study are based on the aspect of usefulness (perceived usefulness), namely the use of electronic medical records in TPPGD and TPPRI, making the registration officer work faster and more effective.The aspect of ease (perceived ease of use) obtained from the interview results is that it is able to make it easier to accelerate the registration process in the TPPGD and TPPRI. The behavioral intention to use obtained is that the users of this system really need this electronic medical record and the registration officer at TPPGD and TPPRI plans to use the EMR in the future.

Abstract

RSUD K.R.M. T Wongsonegoro The city of Semarang became one of the type B hospitals in the city of Semarang, which have applied the electronic medical record gradually the application of electronic medical records assessed as an effort to improve the quality of services increasing patient satisfaction improves the documentation accuracy reduces clinical error and speeds up patient data access. (Billy Maria in Andriani DKK 2017). One of the service units that require the speed, accuracy and accuracy of access to patient data is the coding part of the hospitalization of the person who handled the medical record and coding (PJRM). In its implementation, there were constraints that made the officers less satisfied with RME. The purpose of this research is to evaluate RME implementation using the PIECES method (performance, information, economic, control, efficiency, service). This type of research is qualitative research, by conducting observation methods and interviews to PJRM officers. The results of RME evaluation conducted using the PIECES method on the performance/performance aspect, RME has already produced a good performance. Based on information/information aspect, RME can provide accurate information quality, according to user needs, and easy to understand. Based on Economy/economy, RME has the value to be integrated with one hospital and have qualified resource. Based on Control/control aspect, RME has good integrity and security because it comes with username and password, has different permissions. Based on the Efficiency/data efficiency aspects of RME can be easily learned, operated, and processed. Based on the Service/service aspect: RME users feel that RME provides convenience for RME users. In conclusion, RME in the coding part of inpatient is good enough in terms of Performance, Information, Economy, Control, Efficiency and Control. The advice that can be given is to make improvements and development on RME to avoid errors, regulatory or regulation regarding steps when errors occur, as well as conducting socialization or training to RME users.
