Abstract

Stroke merupakan salah satu penyebab kecacatan terbanyak di dunia. Pasien dengan stroke iskemik memiliki perbaikan fungsional yang berbeda dari pasien dengan stroke hemoragik. Alat yang digunakan adalah kuesioner Functional Independence Measure (FIM). Penelitian ini bertujuan untuk mengetahui perbedaan skor FIM pada pasien rawat inap dengan stroke iskemik dan stroke hemoragik di rumah sakit di Kota Bengkulu. Penelitian ini merupakan penelitian analitik observasional dengan desain potong lintang. Penelitian ini melibatkan pasien stroke yang dirawat di RS Bhayangkara Kota Bengkulu, RSUD Kota Bengkulu dan RSUD dr. M. Yunus Kota Bengkulu pada bulan Januari - April 2018 dengan total sampel 22 pasien stroke iskemik dan 22 pasien stroke hemoragik yang memenuhi kriteria inklusi. Skor FIM sebagai variabel bebas, sedangkan tipe stroke sebagai variabel tergantung. Perbedaan kedua variabel tersebut dianalisis dengan uji t tidak berpasangan dan uji Mann-Whitney. Pasien dengan stroke iskemik memilki skor FIM masuk RS yang lebih tinggi dari pasien dengan stroke hemoragik (26 vs 13,5; p=0,006). Tidak ditemukan perbedaan skor FIM keluar RS pada pasien di kedua tipe stroke (1,67 vs 1,58; p=0,081). Tidak terdapat perbedaan pada peningkatan skor FIM antara pasien dengan stroke iskemik dengan pasien dengan stroke hemoragik (4,12 vs4,36; p=0,444). Pasien dengan stroke iskemik memiliki skor FIM yang tinggi saat masuk RS dibandingkan dengan pasien dengan stroke hemoragik, namun tidak terdapat perbedaan yang bermakna pada peningkatan skor FIM selama perawatan antara pasien stroke iskemik dan stroke hemoragik di rumah sakit di Kota Bengkulu.

Abstract

BACKGROUND: Intestinal microbial dysbiosis and its metabolites can affect the immune activity of intestinal mucosal cells, causing insulitis and pancreatic β-cell death. Probiotic Lactobacillus acidophilus plays an important role in reducing inflammatory cytokines, hence improves oxidative stress that affects pancreatic β-cell apoptosis. Current study examined the feature of pancreatic histopathology affected by the administration of probiotic L. acidophilus in rats with type-1 diabetes mellitus (DM) induced by streptozotocin (STZ).METHODS: Twelve rats were induced by STZ at double dose of 50 mg/kgBB before administered with probiotic L. acidophilus at a dose of 1.5x10 8 or 1.5x10 9 CFU/mL/day, while other 4 rats were used as control. After 21 days of the L. acidophilus treatment, the average of fasting blood glucose (FBG) levels of rats were measured, then the pancreatic histopathology was assessed to evaluate the degree of insulitis in islet of Langerhans.RESULTS: The induction of STZ had been succeeded to increase blood glucose levels, which indicate DM condition. The highest FBG level after 21 days of treatment was found in DM group with glucose level of 512±81.51 mg/dL. The administration of probiotic L. acidophilus during 21 days treatment at both dose 1.5x10 8 and 1.5x10 9 CFU/mL/day significantly improved pancreatic histopathology (p=0.04 and p=0.034, respectively), with significant decrease on insulitis scores compared to DM group.CONCLUSION: The administration of L. acidophilus at both dose of 1.5x10 8 and 1.5x10 9 CFU/mL/day for 21 days can improve pancreatic histopathology of type-1 DM rats induced by STZ, therefore probiotic L. acidophilus may be potential as supplementation treatment for type-1 DM.KEYWORDS: Lactobacillus acidophilus, pancreatic histopathology, streptozotocin, type-1 diabetes mellitus

Abstract

Hypertension is one of the most prevalent non-communicable disease and remains a major public health challenge worldwide. Understanding its spatial distribution is important for describing geographical variations in disease burden and supporting evidence based public health planning. This study aimed to describe the spatial and temporal distribution of reported hypertension cases in Bengkulu City, Indonesia during the period 2020-2024. A quantitative approach with a descriptive ecological desgin was employed using secondary data on hypertension cases among individuals aged ≥15 years obtained from 20 public health centers. Spatial analysis was conducted using Geographic Information System (GIS) and hypertension case distribution was classified into low, moderate, and high categories using the Equal Interval method. The findings revealed substantial spatial and temporal variation in reported hypertension cases throughout the study period. Most areas of Bengkulu City were consistently classified in the high category, with complete spatial coverage observed in 2021, 2022, and 2024, whereas limited moderate category areas emerged in 2023. Districts including Selebar, Gading Cempaka, Ratu Agung, Singgaran Pati, Muara Bangka Hulu, and Kampung Melayu repeatedly exhibited higher reported case burdens than other areas. However, considerable fluctuations in the annual number of reported cases were observed and should be interpreted cautiously, as variations in surveillance systems, reporting pratices, and health service utilization may have influenced the reported patterns. Overall, hypertension remained widely distributed across Bengkulu City during the study period. GIS based mapping provides useful baseline information for public health surveillance and may support the development of geographically targeted prevention and control strategies.

Abstract

This study examined the effect of diaphragm strengthening exercise with incentive spirometry on pain and core muscle stability in patient with LBP (Low Back Pain). Twenty respondents aged 25-60 years participated in this research group. This research using a pre-experimental one-group pretest-posttest design. Incentive spirometry training was given to one group 3 times a week for 4 weeks. Pain level was measured using the numerical rating scale and core muscle stability was measured using a sphygmomanometer which had been modified to function as a tool to measure core muscle stability. There was a decrease in pain levels and an increase in core muscle stability in research subjects. The results of the wilcoxon test and paired t-test obtained a significance value of less than 0.05. There is an effect of diaphragm muscle strengthening exercises using incentive spirometry on improving pain and core muscle stability in patients with LBP (Low Back Pain).

Molecular identification and antimicrobial activities of Bacillus subtilis MS-01 isolated from Indonesian ethnic food lemea. Biodiversitas 26: 1069-1074. The community in Bengkulu Province has a variety of traditional foods produced through fermentation, one of which is a bamboo shoot-based dish known locally as lemea. Bacillus subtilis, found in fermented foods, shows great potential as a probiotic due to its ability to inhibit harmful bacteria. Given its genetic diversity, molecular identification is crucial to differentiate between strains. This study represents the first isolation and identification of Bacillus strains from lemea, an ethnically fermented bamboo shoot from the Rejang tribe community in Bengkulu, Indonesia. This study aimed to evaluate the antibacterial activity and perform molecular identification analysis using the 16S ribosomal RNA gene region. The Bacillus isolate was identified using observations of macroscopic and microscopic morphological Lactic Acid Bacteria (LAB). Inhibition tests of Bacillus isolate were conducted using pathogenic bacteria, namely Escherichia coli, Salmonella typhimurium, and Enterococcus faecalis. The isolate demonstrated an exceptionally strong inhibitory effect, particularly against E. faecalis. Molecular identification involved polymerase chain reaction amplifying the 16S rRNA gene, producing an amplicon of approximately 1,300 base pairs. Sequence analysis via BLAST showed 99.84% similarity to B. subtilis strain X2, SSR17, NIBSM OsG1, BaBc-1, and strain LMV in GenBank. Phylogenetic analysis confirmed the isolate’s classification within the order Bacillales and family Bacillaceae. In conclusion, B. subtilis MS-01 was successfully isolated and identified, exhibiting antibacterial potential against both Gram-negative and Gram-positive pathogenic bacteria, with a particularly noteworthy effect against E. faecalis, thereby suggesting its potential as a probiotic with potent antimicrobial properties.
