Abstract

Klinik muhammadiyah medical center membutuhkan sistim pencatatan dan pelaporan yang tepat dan akurat. Saat ini, pencatatan dan pemrosesan data berjalan kurang optimal, karena dijalankan dengan sistem manual dengan menggunakan sreadsheet. jadi ketika petugas akan membuat laporan mereka mengalami kesulitan dalam mencari dan menyusun data yang dibutuhkan. Tujuan dari penelitian ini adalah untuk mengembangkan draf dari sistim pencatatan dan pelaporan data registrasi. Jenis penelitian ini adalah pengembangan penelitian dengan menggunakan pendekatan kualitatif . Dalam penelitian ini, pengumpulan dan pengambilan data menggunakan teknik purposive sampling . Dari hasil identifikasi pengguna , peneliti mulai merancang diagram alir data ( DFD ) , diagram hubungan entitas ( ERD ), database dan desain antarmuka sistem , serta yang dicetak dari laporan yang dihasilkan . Dengan desain sistem ini , diharapkan menjadi masukan yang berguna bagi lembaga . Tapi desain sistem ini masih banyak kekurangan , seperti subsistem tidak lain dan integrasi antara desain yang ada terbatas pada bagian pendaftaran saja

Abstract

Introduction: Balai Besar Kesehatan Paru Masyarakat (BBKPM) Surakarta is a place providing health service including tuberculosis (TB) and human immunodeficiency virus (HIV). The application of recording and reporting running well will support providing, analysis and good of health service. Thus, the system analysis including input, process, and output are needed to provide it. Method: this study is descriptive with qualitative approach undertaken from Mei-July 2015 in BBKPM and distric health office, Sukoharjo. Data were obtained from in-depth interview and observation. The information were collected from 5 informans who choosed with purposive sampling technique. The validation of result was undertaken with triangulation method. Result and discussion: recording system of tuberculosis and human immunodeficiency virus in BBKPM had been using 3 kinds of software: SIM-RS, SITT and SIHA. In addition, the reporting data had been using the software from ministry of health, Indonesia. From them, there was no data spatial among BBKPM and distric health office in input and output. Disparity of software influence the work load, inefficiency in the data input and the redundancy occurs. Beside, the officers recorded manually to check the data input. Conclussion: there was no spatial data and work load in officer to record and report the data (input and process). In partiarcular of output, the officers were difficult to summary and present the data. The integration system in BBKPM and distric health office, Sukoharjo including spatial data were needed to support the surveillance process of TB/HIVKeyword : Tuberculosis, HIV, Recording, Reporting

Abstract

Health information system is one of the six building blocks or key components of the health system. MMC is a health care unit at the Muhammadiyah University of Surakarta. This study purposed to determine the application of Health information system in MMC. This study was a qualitative research with System Development Life Cycle (SDLC) approach. Data collections were obtained by in-depth interviews and direct observation. The results of the research that has been done, that the existing health information system is not running well, because in the process of recording and reporting is still done manually, the lack of security of data, lack of data integration, as well as the information generated is not in accordance with the need for decision-making.Keywords : HIS, reporting, input, SDLC

Abstract

Pelayanan yang cepat kepada pasien tidak lepas dari kerjasama antar unit dalam Puskesmas. Berdasarkan hasil obsevasi, ternyata masih ditumukan keterlambatan pendistribusian berkas rekam medis pasien rawat jalan. Tujuan dari penelitin ini, adalah untuk mengetahui pelaksanaan penyediaan berkas rekam medis rawat jalan berdasarkan Standar Operasional Prosedur (SOP), serta hambatan dalam pelaksanaannya. Metode yang digunakan dalam penelitian ini adalah jenis penelitian deskriptif kualitatif dengan rancangan cross sectional. Pelaksanaan penyediaan berkas rekam medis berdasarkan SOP distribusi rekam medis. Hambatan penyediaan berkas rekam medis: (1) belum adanya standar waktu pada SOP distribusi rekam medis, (2) perawat mengambil sendiri berkas rekam medis ke pendaftaran, (3) keterbatasan SDM.Kata kunci : Rekam Medis, SOP, Penyediaan Berkas Rekam Medis.

Abstract

The incompleteness in filling out medical records will greatly affect the SNARS 1stedition accreditation assessment.The results of the assessment from the Hospital AccreditationCommittee survey of the MIRM 15 standard at Muhammadiyah Selogiri Hospital, namely thesummary of going home with the completion level of filling in 2019 has not reached the 100%standard. The purpose of this study was to review the completeness of the return summary form infulfilling the elements of the MIRM 15 SNARS 1st edition assessment at the MuhammadiyahHospital Selogiri. This research used a descriptive research method, with a qualitative approach,the data collection method used observation, interviews, documentation study with the subject of 4officers, the object was all documents of inpatients in February 2020 totaling 571 documents. Thesurvey results revealed the completeness of the summary forms return in February at the reviewsof identification of 100%, review authentication 81%, Review of recording by 64%, reviewsreporting in accordance SNARS element 1 are met incomplete, element 2 is fulfilled complete, theelement 3 is met, the elements 4 are met complete, element 5 is fulfilled completely, element 6 ispartially fulfilled. The conclusion of this study is that the level of completeness of the summaryform home at Muhammadiyah Selogiri Hospital in February 2020 is not according to the standard,namely 11%.

Abstract

Information system evaluation is the process of knowing the extent of the information system.From a preliminary study of researchers in the health center gatak about the number of humanresources has been found that the lack of human resources is competent in the it field, not all officersunderstand the use of the Health Center Management Information System (HCMIS), sometimes anerror server causes a breakdown in service processes. The purpose of this study is to evaluate theapplication of Health Center Management Information System in the health center gatak and knowthe use of quality services indicator Health Center Management Information System by explainingthe compatible systems component that is human, technology and organization. This type of researchis a descriptive analytic study with a qualitative approach. Research subject exist six is the head ofmedical records, one operator polyclinic, Hcmis coordinator, one Hcmis operator, head of themedical center, and one pharmacist. The sample retrieval technique used is purposive sampling. Theresults of this study, is known to what extent the Health Center Management Information Systemprogram is running, to provide input for an evaluation at health center gatak for the front becausehuman, technology, and organization compatibility really does affect information systems. It ishoped that there will be periodic training so that the implementation of Health Center ManagementInformation System use becomes easier.

Abstract

The Workload Indicator Staff Need (WISN)is an indicator that shows the amount of staffneeded at health facilities based on workload, so that location or relocation will be easier andmore rational. There is medical record officer at the UPTD health center Weru in the outpatientregistration section who continues his education so that in his work duties must be replaced byofficers in the filing division. Filing officers concurrently have duties as registration officers.Cause the workload of officers is high and officers in carrying out their duties are not inaccordance with the established SOP. The purpose of this study was to calculate availableworking time, workload standards, allowance standards and the need for medical record personelat the UPTD health center Weru. This type of research is a descriptive study with a cross sectionalapproach.. The subjects in this study were 3 medical record staff and the object was the medicalrecord staff. Data collection was carried out by observation and interviews with medical recordofficers at the UPTD health center Weru. The research instrument was an observation cheklist andinterview guidelines.The calculation is done using the WISN method. It is known that the availableworking time is 1,925 hours / year or 115,500 minutes / year, the need for the workload foroutpatient registration is 452,375 minutes, the filing section is 2.749.542 minutes, the inpatientregistration section or the emergency room is 420,063 minutes. The standard calculation ofallowance is 0.15 hours / year, the calculation of the need for personnel is 5 personnel, so it isnecessary to add 1 officer in the outpatient registration section of the UPTD healt center Werut.

Abstract

The aims of this research were to: 1) improve the quality of medical records and health workers related to health service facilities in order to be able to collect the data communicatively, 2) do the data collection correctly, 3) do the qualitative and quantitative analysis, 3) improve the management of medical record’s work unit based on the applicable standard to produce information in gaining accurate medical information. The development of medical records worker’s knowledge and competency in the local clinic was accompanied by the improvement of medical record’s management in the local clinic.Dealing with the dedication for society, the implementer team used analysis study and gave the medical record workers intervention service in the local clinic. The service was held into two stages: 1) portraying the partners’ characteristic, problems and needs of medical record workers in the local clinic, 2) giving intervention in the form of medical records management in the local clinic based on the applicable rule and standard. The interactive method was held in this service. The materials were theoretical and practical. The participants’ ability was measured by Pre-Test dan Post-Test. The implementer team monitored and evaluated the training given. The results of dedication program in “Weru” Local Clinic: 1) the employee understood more about the “UKRM” management, 2) there were many problems regarding to the duplication of medical record number and the lack of quality and completeness, 3) the implementation of this activity got positive response from the head of local clinic and the participants (employee of UKRM).Keywords: Medical record, Local clinic, Training, Competency

Abstract

Ruang lingkup layanan BPJS sangat luas, tentu saja ada banyak masalah kompleks yang terjadi dalam proses layanan. Permasalahan tersebut antara lain antrian panjang yang timbul karena jumlah pasien yang tinggi, beban kerja petugas yang semakin meningkat karena menjadi entri data lebih dari satu kali, yaitu pada SIMRS dan SEP, sumber daya manusia yang tersedia menjadi kewalahan untuk dapat memberikan layanan cepat untuk semua pasien BPJS sehingga kinerja petugas menurun. Dalam rangka meningkatkan kualitas layanan perawatan, terutama dalam penerimaan rawat jalan. Oleh karena itu, peneliti ingin mengetahui perbedaan kualitas layanan di Rumah Sakit X sebelum dan sesudah sistem bridging diimplementasikan. Mengetahui perbedaan kualitas layanan sebelum dan sesudah sistem bridging diimplementasikan pada penerimaan pasien rawat jalan Rumah sakit X. Pendekatan kuantitatif analitik komparatif observasional dengan desain studi cross-sectional. Kualitas layanan sebelum sistem bridging 3% petugas menilai sangat baik, 65% baik, 31% tidak baik, 1% tidak terlalu baik. Kualitas layanan setelah sistem bridging menerapkan tingkat petugas 38% itu sangat baik, 57% baik, tidak baik 5%, 0% tidak terlalu baik. Dari uji Wilcoxon menunjukkan bahwa koefisien korelasi -3,727ᵃ maka Asymp. Sig. (2-tailed) adalah 0,000. Ini menunjukkan bahwa ρ <0,05 berarti Ho ditolak. Ada perbedaan yang signifikan dalam kualitas layanan sebesar -3,727ᵃ dan Asymp. Sig 0,000 antara sebelum dan sesudah sistem bridging diterapkan di Rumah Sakit X.

Abstract

Sukoharjo is a regency in Central Java that is having high rank of TB case. In 2016, the sufferers of TB were452 with the total of death were 16 people. In 2017, the sufferers were 335. One of the control indicators was Case Detection Rate (CDR). The established-policy needed medical support facility. The professional employees will support the continuity of health service for the concerned society. The aim of this research was to analyze the rule and fact of TB’s tackling in Sukoharjo regency by looking at the side of government organization needs and the plan had made. The approach was needed in examining the plan of strategy and tackling policy by having framework related to the method and systematic technique. The long-term target of this research was the result of the TB’s situation analysis that could be the input for the stakeholders dealing with designing the priority scale as the arrangement of Regional Action Plan “Rencana Aksi Daerah (RAD)” or as the advocacy material for civil society group in tackling TB so that its prevention and treatment could be managed and supported by all stakeholders in Sukoharjo regency. The method used by the researcher were GAP Analysis and CSF (Critical Sucses Factor). The cause of the high and low of TB’s finding in Sukoharjo regency were categorized into direct and indirect factors. One of them was the absence of special law in regency or regional. The other issue was about human resources. Besides, many health service facilities had not fulfilled the DOTS (Directly Observed Treatment Short-course) which influenced TB’s achievement in Sukoharjo regency.Keywords : Tuberculosis, TB’s Policy, CSF, GAP Analysis
