Abstract

Latar belakang : masalah pulpa dan periodontal menyebabkan lebih dari 50% kehilangan gigi. Laporan kasus ini merupakan kasus lesi primer endodontik dengan lesi sekunder periodontal yang secara klinis terdapat sinus tract. Kondisi ini merupakan kasus yang secara kolaborasi dikerjakan oleh bidang konservasi gigi, prostodontik dan bidang periodonsia untuk mendapatkan hasil yang maksimal. Laporan kasus : terdapat tiga kasus lesi endodonti-periodontal, dua merupakan kasus (gigi 21 dan 37) yang setelah beberapa tahun dilakukan perawatan saluran akar (PSA) mengalami pembentukan sinus tract, dan satu kasus (gigi 47) yang setelah PSA namun tidak memeperlihatkan perbaikan sinus tract yang terbentuk di gingiva. Tindakan bedah flap periodontal dilakukan untuk mencari penyebab, menghilangkan jaringan granulasi, dan memperbaiki kerusakan tulang alveolar yang terjadi. Seluruh kasus mengalami kerusakan tulang anguler di daerah furkasi pada gigi posterior (37 dan 47), dan daerah interdental gigi 12-22; sedangkan satu kasus (gigi 12-22) juga mengalami fenestrasi di fasial gigi 21. Defek tulang anguler dan daerah fenestrasi setelah dibersihkan dari jaringan granulasi yang terinfeksi, diisi dengan graf tulang dan ditutup oleh membran pericardium sebagai guided tissue regeneration (GTR), kemudian ditutup degan penjahitan. Kontrol 14 hari setelah tindakan bedah, sinus tract  telah hilang dan warna gingiva normal. Kesimpulan : lesi endodontik-periodontal yang memperlihatkan sinus tract yang persisten setelah perawatan saluran akar merupakan indikasi adanya kerusakan periodontal yang kompleks. Terapi bedah regeneratif setelah perawatan saluran akar dapat berperan dalam menghilangkan sinuc tract.

Abstract

Background: Periodontitis is an inflammatory disease caused by periodontal pathogens and influenced by multiple risk factors such as genetics, smoking habit, age and systemic diseases. The inflammatory cascade is characterized by the release of C-reactive protein (CRP). Periodontitis has been reported to have plausible links to increased level of CRP, which in turn has been associated to elevated risk of  cardiovascular disease (CVD). Purpose: The purpose of this study was t o investigate the relationship amongst the severity of periodontitis, CRP level in blood and CRP (-717 C>T) gene polymorphism in male Indonesian smokers and non-smokers. Method: The severity of periodontitis was assessed for 97 consenting male Indonesian smokers and non-smokers. The CRP level of the subjects was determined by using immuno-turbidimetric assay performed in PARAHITA Diagnostic Center Laboratory ISO 9001: 2000 Cert No. 15225/2. The rate of CRP (-717C>T) gene polymorphism was determined by using PCR-RFLP in Oral Biology Laboratory, Faculty of Dentistry, Universitas Indonesia. Result: The results suggest that the CRP protein level is not significantly associated with the tested CRP gene polymorphism (p>0.05). Also, while the severity of periodontitis increased significantly with subject age, the CRP level in blood serum was not significantly related to the severity of  periodontitis. The genotypes of the tested polymorphism did not show significant association with the severity of periodontitis either in smokers or in the combined population including smokers and non-smokers. The results naturally do not exclude such associations, but suggest that to discern the differences the sample size must be considerably increased. Conclusion: The CRP (-717C>T) gene polymorphism and CRP level in blood serum were not found to be associated with the severity of periodontitis in male smokers or in the combined population of smokers and non-smokers.

Abstract

Background: Dental implant is one of an alternative solutions reconstruction therapy for missing teeth. Complication of dental implant could occurs and leading to implant failure. In order to restore the complication, surgical treatment with guided bone regeneration (GBR) is indicated. The potential use of bone substitutes is widely known to be able to regenerate the bone surrounding the implant and maintain bone volume. Purpose: The study aimed to demonstrate the effectiveness of implant-bone fully coverage by using sandwich technique of biphasic calcium phosphate (BCP) and demineralized freeze-dried bone allografts (DFDBA) bone substitutes combined with collagen resorbable membrane. Case: A 24-year-old male came with diagnosis of peri-implantitis on implant #11. Clinical finding indicated that implant thread was exposed on the labial aspect. Case management: After initial therapy including oral hygiene improvement performed, an operator did a contemporary GBR to correct the defect. Bone graft materials used were 40% β-tri calcium phosphate (β-TCP)-60% hydroxyapatite (HA) on the outer layer and DFDBA on the inner layer of the defect. Resorbable collagen membrane was used to cover the graft. Conclusion: GBR with sandwich technique could serve as one of the treatment choices for correcting an exposed anterior implant that would enhance the successful aesthetic outcome.

Abstract

Objective: Periodontitis is an inflammatory disease of the supporting tissues of the teeth caused by specific microorganisms, resulting in decay of the teeth and damaging of supporting tissues. Systemic conditions and increasing age will affect the periodontal tissue due to amongst others changes in the immune defense system. The classification for periodontal diseases used today is the 1999 American Academy of Periodontology (AAP) classification.  The latest development was in 2017, when the AAP published a new classification that classifies periodontitis based on stage and grade and is the classification that is widely used today. Research on determining the stage and grade of periodontitis using the 2017 AAP classification is still very rare in Indonesia.Material and Methods: In this study, a cross sectional analytical descriptive approach was used to determine the stage and grade of periodontitis according to the classification of periodontal disease based on the 2017 AAP in relation to the age of patients with systemic conditions and diseases. A total of 331 medical records from patients treated at the RSKGM FKG UI in the 2017 – 2019 visit were researched.Results: The results have shown that the highest stage and grade in all research subjects was stage III grade C, , this stage and grade was found in subjects who smoke. Different results were found in research subjects with Diabetes Mellitus or Hypertension namely stage III grade B.Conclusion: Patients with hypertension without any systemic disease and non smoking should be advised to request a test for diabetes mellitus.

Abstract

Introduction: The treatment of advanced periodontitis has been a challenge for clinicians. Platelet-rich fibrin (PRF) is a second-generation platelet concentrate that contains various growth factorss and cytokines that promote tissue regeneration. The use of PRF in combination with other regenerative materials and techniques has been shown to be effective in the treatment of intrabony defects. This case series aims to evaluate the efficacy of PRF combinations in the regenerative treatment of patients with advanced periodontitis. Case report: Two patients diagnosed with advanced periodontitis (stage III/IV grade C) were selected for this study. After the completion of periodontal initial therapy, there were still pockets with a probing depth ≥ 5 mm (5–10 mm). Therefore, surgical interventions were planned. The first case used the combination of PRF with bone graft and guided tissue regeneration (GTR), while open flap debridement (OFD) with the addition of PRF and bone graft to create sticky bone was done in the second case. Three to five months of follow-up showed favorable outcomes, as evidenced by reductions in probing depth (PD), gains in clinical attachment level, and filling of bone defects, as well as improvements in patient comfort and rapid wound healing. Conclusion: The combination of PRF with grafting material and/or GTR has been shown to be a beneficial therapeutic approach for periodontal regenerative therapy. Platelet-rich fibrin in periodontal surgery may accelerate the tissue healing process.

Abstract

Human beta-defensin-1 (HBD-1), a peptide released by the immune system, has been investigated for its association with periodontal disease. Several studies have found positive findings of its expression related to disease progression, whereas others have reported some tendencies. This review highlights studies associated with HBD-1 and periodontal disease in both primary and clinical investigations, as well as the underlying mechanism and discusses further research possibilities for HBD-1. HBD-1 acts as an innate immune apparatus and mediating the adaptive immune system; therefore, its role in the pathogenesis of a periodontal disease is indisputable. HBD-1 is mainly expressed in the oral stratified epithelium and sulcular epithelium, where the barrier junction properties (e.g., E-cadherin and β-catenin in the adherent junction) of the gingival epithelial cells serve as the first line of defense against periodontal infection. The HBD-1-reinforced epithelium therefore provides both a mechanical and a chemical barrier action. Critical issues that arise in the gingival epithelium can therefore be alleviated by modulation of HBD-1 production to enhance its protective function. The antimicrobial, anti-inflammatory, and wound healing properties of HBD-1 support its use as a promising clinical treatment strategy.

Abstract

According to Basic Health Research, dental and oral health is still one of the main health problems that occur especially in small cities due to lack of accessibility. Poor oral hygiene scores can lead to complex diseases such as caries and periodontal disease. If this condition persists, it will have an impact on aspects of the patient's life such as disruption of productivity, and declining general health of patients, resulting in disruption of social aspects. Until now, no solution can improve the oral health of the community. Whereas Indonesia needs to prepare a competent and healthy workforce to reach Indonesia Gold 2045. Short messages sent by hospitals containing reminders and education have the potential to increase public awareness of the importance of compliance in maintaining dental and oral hygiene. To analyze the implementation of short messages in support of modification of reminders and education in dental practice. The main references were taken from the PubMed, Science Direct, and Wiley databases last accessed on 16 April 2022. 31 search results that had been filtered using PRISMA 2009 became the source with 5 main data that met the inclusion criteria. in the form of clinical studies in the form of randomized controlled trials that have been processed using the study tool from the NIH. From a systematic study, it was found that short messages can be used as reminders and education in the fields of public dental health, pediatric dentistry, orthodontics, and periodontics. This method is carried out with follow-up and evaluation for up to 6 months. Countries such as France, Italy, India, America, and Japan have used this method. The main outcome of this study was an improvement in the oral hygiene score (OHI-S), behavior, and oral health literacy. The use of reminders in the form of short messages has the potential to increase public awareness of the importance of compliance in maintaining dental and oral hygiene. The short message approach as a dental support service has proven to have many advantages and can be implemented in Indonesia.

Abstract

Melalui Pemeriksaan Terpadu merupakan hasil kerja sama antara Universitas Bani Saleh dan Fakultas Kedokteran Gigi Universitas Indonesia yang dilaksanakan di Desa Pantai Harapan Jaya, Kecamatan Muara Gembong. Kegiatan ini bertujuan meningkatkan derajat kesehatan ibu hamil dan masyarakat umum melalui pemeriksaan dan edukasi terpadu. Layanan yang diberikan meliputi pemeriksaan hemoglobin (Hb) pada ibu hamil untuk deteksi dini risiko anemia, pemeriksaan kesehatan gigi ibu hamil sebagai bagian dari perawatan antenatal yang komprehensif, serta pemeriksaan kesehatan umum bagi ibu hamil dan seluruh warga. Metode pelaksanaan meliputi skrining kesehatan, pemeriksaan fisik, edukasi, dan rujukan bagi peserta yang memerlukan perawatan lanjutan. Hasil kegiatan menunjukkan ditemukannya terdapat kasus anemia pada ibu hamil, masalah kesehatan gigi dan mulut, serta keluhan kesehatan umum pada masyarakat, yang kemudian ditangani dengan intervensi awal dan penyuluhan kesehatan. Kolaborasi lintas institusi ini memperkuat sinergi perguruan tinggi dalam mendukung upaya promotif dan preventif di wilayah pesisir yang memiliki keterbatasan akses layanan kesehatan.
