Abstract

The role of Aggregatibacter actinomycetemcomitans bacteria can lead to the occurrence of aggressive periodontitis. The bacterium infects periodontal tissue with its virulence factor. Antibacterial administration such as chlorhexidine mouthwash is one of the treatments for periodontal disease due to bacteria. Long-term use of chlorhexidine can cause irritation of the oral mucosa, burning sensation, changes in taste perception and the appearance of stains on the teeth. One alternative is to gargle the guava leaf extract water. The antibacterial effect of guava leaf extract is obtained from its chemical compounds, namely flavonoids, phenolics, alkaloids, ethanol and tannins. This research aims to know the potential of water guava leaf extract (Syzygium aqueum) in inhibiting the growth of Aggregatibacter actinomycetemcomitans bacteria.  A literature review by reviewing research articles from Science Direct and Google Scholar using the keywords "Syzygium aqueum and Aggregatibacter actinomycetemcomitans".   Guava leaves contain many derivatives of compounds such as flavonoids, alkaloids, tannins, saponins, and terpenoids. The content of these compounds can be used as antibacterial because they are effective in inhibiting the growth of bacteria.  Water guava leaf extract (Syzygium aqueum) can inhibit the growth of Aggregatibacter actinomycetemcomitans bacteria.

Abstract

Background: Gingival hyperpigmentation can occur in active smokers with high frequency. The current treatment is surgical. Therefore, researchers found a new breakthrough by creating a minimally extensive treatment, namely gel preparations. Combination of turmeric (Curcuma domestica) and moringa leaf (Moringa oleifera) extract that has the potential as a tyrosinase enzyme inhibitor. The extract will be made in a gel preparation. This study aims to determine the most optimal concentration of the gel. Method: This study used an experimental method. The gel preparation was made with various concentrations of turmeric extract and moringa leaf extract, namely F1 0.1%: 2%, F2 0.5%: 4% and F3 0.9%: 6%. Followed by testing the physical and chemical properties. Chemical properties in the form of pH, physical properties in the form of organoleptic, homogeneity, adhesiveness and spreadability by measuring the diameter of TB (No Load), B50 (Load 50 g), B100 (Load 100 g) and B150 (Load 150 g). Results: The results showed that F1, F2 and F3 were homogeneous with dark brown color, distinctive smell of moringa leaves and increasingly dilute form with each additional concentration with pH (F1) 7.99, (F2) 7.71 and (F3) 7.58. The spreadability of gel preparation F1 (TB) 2.7, (B50) 2.8, (B100) 2.9, (B150) 3.1; F2 (TB) 2.8, (B50) 3.1, (B100) 3.3, (B150) 3.5 and F3 (TB) 4.2, (B50) 4.6, (B100) 4.7, (B150) 5.1 based on these observations it is known that F3 meets the criteria for spreadability at a load of 150 gr. Conclusion: Based on this study, it can be concluded that F3 gel, a combination of turmeric extract and moringa leaf extract, has the most optimal formula because it almost meets all the requirements for gel preparations.

Abstract

ABSTRACTBackground: Gingival enlargement can be treated with periodontal therapy, starting with the initial phase therapy, which includes dental health education and scaling-root planing. When the enlargement persists after the initial treatment is carried out, a surgical procedure, namely gingivectomy, is needed.Method: A 22-year-old male patient came to RSGM UNIMUS complaining that his upper front gums appeared enlarged. The patient had been aware of the complaint since the examination during the scaling procedure two weeks ago. There was no pain in the enlarged gums, but they bled when brushing his teeth. The patient wanted to get treatment for his gums. The patient denied any relevant medical history and allergies.Outcome: The gingivectomy in this patient improved the gingiva appearance and lengthened the clinical crown on the upper anterior teeth. Conclusion: Gingivectomy was proven to be an effective surgical therapy to eliminate gingival enlargement and maintain the proportional gingival appearance, especially on the anterior teeth. However, considering the complexity of the case, more extensive mouth rehabilitation is required.

Abstract

Periodontitis continues to be a debilitating problem when improperly treated. This condition is associated with certain periodontopathological bacteria, including Porphyromonas gingivalis. Chlorhexidine, while proven effective against bacteria in the oral cavity, carries unwanted side effects. Curcuma domestica and Moringa oleifera are domestic herbs that possess several therapeutic effects. This study aims to investigate the antibacterial effect of the combined two herbs in extract gel. This experimental study used three combination formulas of C. domestica: M. oleifera (0.1%:2%, 0.5%:4%, 0.9%:6%) as the test group and chlorhexidine as the positive control group. The antibacterial inhibition disc method was used to determine the antibacterial activity in each group. The statistical analysis was carried out with Kruskal-Wallis and continued with the Mann-Whitney test. In this study, extract gel combination of Curcuma domestica and Moringa oleifera at concentrations of (0.1%:2%; 0.5%:4%; 0.9%:6%) can inhibit the growth of Porphyromonas gingivalis with average inhibition zone diameters of 12.50 mm, 14.49 mm and 16.66 mm, respectively. The inhibition zone falls in the strong category. The control group showed a mean inhibition zone of 20.96, which is classified as a very strong inhibition zone. Although there was a significant p-value between the test and control groups, the results suggested the potential of the extract gel as an alternative for treating periodontitis with lesser side effects.

Abstract

Background: Gingival enlargement is the most common characteristic of various conditions, such as inflammation, medication effects, systemic conditions like pregnancy, leukemia, and can also be caused by orthodontic treatment. There is a positive correlation between the duration of orthodontic appliance use and the incidence of gingival enlargement since it facilitates biofilm accumulation and bacterial colonization, which triggers the inflammation.Case: A 24-year-old female patient came to RSGM UNIMUS with complaints of swollen gums in the posterior maxilla and anterior mandible, causing her teeth to appear shorter. She has experienced this issue for the past five years since using fixed orthodontic appliances. Intraoral examination revealed significant swelling in almost the entire gingival region of both the upper and lower jaws, along with a conical supernumerary tooth in the anterior region of the lower jaw. Physiological pigmentation was noted in the anterior gingiva of the mandible, which is showing a brownish color. Radiographs showedhorizontal bone loss at the alveolar crest in the lower jaw. Gingivectomy with scaling and root planning, extraction of the supernumerary tooth, and depigmentation were planned for this case.Outcome: Following a one-month post-operative evaluation, the treatment outcomes for the patient were promising with minimal pain reported. She noted that her confidence in her appearance has significantly improved. Conclusion: The use of fixed orthodontic appliances is correlated with gingival enlargement due to several factors. These appliances can lead to increased plaque accumulation and biofilm formation around the brackets and wires, which triggers inflammation. The mechanical irritation from the appliances can also exacerbate the condition, making it harder for patients to maintain her oral hygiene.
