Abstract

Kapitasi telah menjadi salah satu mekanisme penting dalam pengelolaan Jaminan Kesehatan Nasional (JKN) di Indonesia oleh Badan Penyelenggara Jaminan Sosial (BPJS) Kesehatan. Kapitasi Berbasis Kinerja (KBK) merupakan indikator yang harus dipenuhi Puskesmas dalam penerimaan kapitasi. Kekurangan sumberdaya manusia dan pemenuhan indikator KBK yang tidak tercapai merupakan masalah yang sedang dihadapi oleh Puskesmas. Tujuan penelitian ini untuk menganalisis kalkulasi kapitasi BPJS untuk Puskesmas, evaluasi indikator KBK, dampak dan disparitas kapitasi terhadap kebutuhan Puskesmas.Penelitian ini menggunakan jenis penelitian kualitatif dengan pendekatan studi kasus. Populasi dalam penelitian ini yaitu 8 Puskesmas di Kota Payakumbuh. Informan terdiri dari Kepala Puskesmas, Tata Usaha dan PIC JKN serta Kepala Dinas Kesehatan. Penelitian telah dilakukan pada bulan Mei – Juli tahun 2024. Teknik pengumpulan data yang dilakukan menggunakan cara trianggulasi atau gabungan. Data dikumpulkan dengan observasi, wawancara, dokumentasi. Analisis data melalui reduksi data, penyajian data, penarikan kesimpulan atau verifikasi. Hasil penelitian menunjukkan bahwa mekanisme kapitasi memiliki kriteria ketersediaan sumber daya manusia yaitu dokter umum yang belum terpenuhi, sarana dan prasarana, lingkup pelayanan sudah memadai. Komitmen pelayanan melalui capaian Indikator KBK diataranya angka kontak dan Rasio Peserta Prolanis Terkendali yang belum terpenuhi. Pengurangan kapitasi berdampak buruk terhadap kinerja pelayanan. Pendapatan kapitasi yang diterima berkisar antara 90–95%, sehingga terjadi disparitas dengan kebutuhan Puskesmas. Kata Kunci: Kapitasi Puskesmas, Indikator KBK

Abstract

Many adolescents engage in sexual behavior, which can be influenced by several factors including the promotion mix, adolescent reproductive health knowledge and the role of parents. The purpose of this study was to determine the effect of the promotion mix, knowledge of adolescent reproductive health and the role of parents on the utilization of Youth Care Health Services (PKPR) in Gema Nusantara Bukittinggi Health Vocational School in 2020. The sample of this study used purposive sampling, obtained as many as 84 respondents. The research instrument used a questionnaire. This type of research used in this research is quantitative research with cross sectional design. The results of multiple linear regression research variable promotion mix with a regression coefficient of 0.074 and a significance value of 0.001 means that there is a significant and positive influence on the promotion mix of utilization (PKPR), adolescent reproductive health knowledge variables with a regression coefficient of 0.357 and a significance value of 0.001 means there is a significant influence significant and positive knowledge of adolescent reproductive health on utilization (PKPR), the variable role of parents with a regression coefficient of 0.269 and the significance value of 0,000 means that there is a significant and positive influence on the role of parents on utilization (PKPR).

Abstract

In the working area of the Andalas Health Center, ARI (Acute Respiratory Infection) ranked first among the ten most common diseases. As of December 2022, 80.43% of houses in this area did not meet health standards. This study aimed to identify factors associated with the incidence of ARI in toddlers in the Andalas Health Center area in 2023. This was an analytical survey using a cross-sectional approach, conducted from March to June 2023. Data were collected through interviews and home environment assessments. Data analysis was performed using univariate and bivariate methods with the Chi-square test.Results showed that 62.8% of toddlers experienced ARI. Among the assessed homes, 60% had inadequate ventilation, 60% had poor natural lighting, 57.1% had improper room temperature, 68.6% had unsuitable humidity levels, and 54.3% had high occupancy density. Bivariate analysis revealed significant associations between natural lighting (p=0.0001; PR=6.667), room temperature (p=0.0001; PR=4.750), and humidity (p=0.0001; PR=9.625) with ARI incidence. However, no significant relationship was found with ventilation (p=0.353) and occupancy density (p=0.073). It is recommended that sanitarians at the Andalas Health Center increase healthy home education and implement preventive measures, including physical home improvements. Cross-sector collaboration is encouraged to support home renovation programs for low-income families, to create a healthy home environment.

Abstract

Background: Hypertension at Andalas Public Health Center, Padang City, reached 33.63 percent in 2023. One controllable risk factor is excessive salt consumption. Housewives' knowledge plays an important role in this matter but remains limited due to the lack of promotional efforts. Purpose: This study aimed to determine the effectiveness of health education using the Emo-Demo method in controlling salt consumption among housewives in Andalas Subdistrict, East Padang District, Padang City. Methods: This research applied a mixed-method approach from January 28 to June 7, 2025. Qualitative data were collected through in-depth interviews with housewives, a nutrition program officer, a linguist, and a design expert. Qualitative sampling was conducted using non-probability sampling with a purposive sampling technique. The quantitative study used a quasi-experimental one-group pre-test and post-test design. Quantitative sampling was conducted using non-probability sampling with a total sampling technique. Data were collected from 70 housewives through a questionnaire and analyzed using univariate and bivariate methods with the Wilcoxon test. Results: The qualitative findings resulted in the development of a salt consumption Emo-Demo module adjusted to the needs of the target audience. Quantitative results showed an increase in the median knowledge score from 9.00 before the intervention to 14.00 after. The median attitude score increased from 44.00 to 49.00. Statistical tests showed significant differences in knowledge and attitude scores with a p-value less than 0.05. Conclusion: This study concludes that the Emo-Demo educational method is effective in enhancing housewives’ knowledge and attitudes toward controlling salt consumption.

Abstract

Background: Diarrhea is a major global health problem that often affects children under five. In West Sumatra, 1,910 cases were reported in 2023, while in Padang City cases increased from 1,199 in 2022 to 1,576 in 2023. In the first half of 2025, Parak Karakah Health Center recorded 50 cases among under-fives. Objective: This study aimed to assess the effectiveness of a booklet in improving mothers’ knowledge and attitudes toward diarrhea prevention. Method: A mixed-method design was used, combining an exploratory case study and a quasi-experimental one-group pretest–posttest design. The study was conducted from December 2024 to June 2025 in Kubu Dalam Parak Karakah, Padang City, using interviews and questionnaires. Qualitative data were analyzed through reduction, presentation, and verification, while quantitative data were tested for validity, reliability, normality, and significance using the Wilcoxon test. Results: The booklet was developed using the P-Process. Mothers’ median knowledge increased from 11.00 to 14.00, and attitudes from 48.00 to 67.00. The Wilcoxon test showed a significant change (p=0.0001). Conclusion: The booklet was effective in enhancing mothers’ knowledge and attitudes about diarrhea prevention and is recommended as a practical educational tool in health promotion. recommended that the booklet be further developed according to the target group’s needs and utilized as an educational tool in promotive and preventive health efforts. Abstract

Tuberculosis (TB) remains a global and national health issue. Sijunjung Regency has seen an increase in TB cases, with fluctuating treatment success rates. Medication adherence is a key factor in treatment success and is influenced by the quality of health care services. This study aims to analyze the quality of services regarding medication adherence among TB patients, explore patients’ perceptions, describe service factors influencing adherence, and understand the role of health workers from the patients’ perspective at Community Health Centers (Puskesmas) throughout Sijunjung District. This study employed a qualitative approach with a phenomenological design. The primary informants consisted of 8 TB patients, plus key informants (2 healthcare workers) and additional informants (2 family members). Data were collected through in-depth interviews, observations, and document reviews. Analysis utilized the Miles & Huberman model.Research results on the quality of healthcare services are related to the level of medication adherence in tuberculosis patients. The dimensions that contribute most to adherence are empathy, assurance, and responsiveness. Positive perceptions are seen in aspects of interpersonal relationships (friendliness, patience, effective communication) and logistical reliability (drug availability), while negative perceptions are related to the lack of privacy in waiting rooms, long administrative waiting times, and inflexible service hours for informal workers. Health service factors that influence adherence include continuous education and information, active supervision and monitoring, ease of service access, and support from healthcare workers. The role of healthcare workers in supporting patient adherence, according to the patient's perspective, is as educators and motivators.Based on this study, high-quality care particularly empathy, assurance, and responsiveness is crucial for patient adherence. Improvements to physical facilities and greater flexibility in service hours are needed.

Abstract

Improving public health requires planned efforts through effective health services, in which Occupational Safety and Health (OSH) is crucial for protecting workers from the risk of accidents and work-related illnesses. Hospitals present high occupational risks; however, the implementation of the Hospital Occupational Safety and Health Management System (SMK3RS) at Dr. Achmad Mochtar General Hospital in Bukittinggi is considered suboptimal, particularly due to constraints related to limited human resources and changes in organizational structure. This study aims to analyze the implementation of the Hospital Occupational Safety and Health Management System (SMK3RS) and identify the barriers affecting its effectiveness. The research methods used include qualitative approaches such as in-depth interviews, observations, and document reviews, as well as a quantitative approach through a questionnaire administered to 39 respondents. The research results regarding the input component indicate that written policies have not been updated since 2015, the number of staff is limited to only 2 people, and there is no specific budget allocation for OSH. In the process component, planning has been based on risk management; however, the implementation of disaster and fire drills, as well as periodic health examinations, has not been conducted routinely and comprehensively. Regarding the output component, internal monitoring and evaluation have not been conducted systematically, and incident documentation remains inactive. In conclusion, the implementation of SMK3RS at Dr. Achmad Mochtar General Hospital in Bukittinggi has not been optimal due to challenges related to management commitment, budget, and staff competence.
