Abstract

Abstract: Work motivation is a set of attitudes and values that influence an individual to achieve more detailed things according to his/her goal. This study aimed to obtain the determinants of work motivation of health workers at Manado Adventist Hospital during the Covid-19 pandemic. This was a quantitative study. Population consisted of all health workers totaling 238 meanwhile samples were 70 people. Data were analyzed using the chi square test and logistic regression test. The results showed that physical working condition was 64.3% good and 35.7% poor; interpersonal relationship was 44.3% good and 55.7% poor; company policy was 30% good and 70% poor; salary was 62.9% good and 37.1% poor; and work motivation was 44.3% good and 55.7% poor. The chi square test showed that there were significant relationships between physical working condition and work motivation (p=0.013); interpersonal relationship and work motivation (p=0.000); company policy and work motivation (p=0.019); salary and work motivation (p=0.028). The logistic regression test confirmed that interpersonal relationship had the closest relationship with work motivation. In conclusion, work motivation determinants in this study are physical working condition, interpersonal relationship, company policy, and salary. Interpersonal relationship has the closest relationship to work motivation.Keywords: physical working conditions; interpersonal relation; policies; salary; work motivation Abstrak: Motivasi kerja yaitu serangkaian sikap dan juga nilai yang memengaruhi individu untuk menggapai hal yang lebih detail sesuai dengan tujuannya. Penelitian ini bertujuan untuk mengetahui determinan motivasi kerja tenaga kesehatan Rumah Sakit Advent Manado di masa pandemi Covid-19. Jenis penelitian ialah kuantitatif. Populasi ialah seluruh tenaga kesehatan berjumlah 238 orang dengan sampel berjumlah 70 orang. Analisis statistik menggunakan uji chi square dan regresi logistik. Hasil penelitian mendapatkan variabel kondisi kerja fisik baik sebesar 64,3% dan kurang baik 35,7%. Hubungan interpersonal didapatkan kategori baik 44,3% dan kurang baik 55,7%. Kebijakan perusahaan didapatkan kategori baik 30% dan kurang baik 70%. Gaji didapatkan kategori baik 62,9% dan kurang baik 37,1%. Motivasi kerja didapatkan kategori baik 44,3% dan kurang baik 55,7%. Hasil uji chi square menegaskan adanya hubungan bermakna antara: kondisi kerja fisik dengan motivasi kerja (p=0,013); hubungan interpersonal dengan motivasi kerja (p=0,000; kebijakan perusahaan dengan motivasi kerja (p=0,019); dan gaji dengan motivasi kerja (p=0,028). Hasil uji regresi logisitik menunjukkan bahwa hubungan interpersonal yang paling berhubungan erat dengan motivasi kerja. Simpulan penelitian ini ialah determinan motivasi kerja yang didapatkan ialah kondisi kerja fisik, hubungan interpersonal, kebijakan perusahaan, dan gaji. Hubungan interpersonal yang paling berhubungan kuat dengan motivasi kerja.Kata kunci: kondisi kerja fisik; hubungan interpersonal; kebijakan; gaji; motivasi kerja

Abstract

Abstract: Type 2 diabetes mellitus (T2DM) is the most common type of diabetes today. Lifestyle changes are one of the causes of DM problems. Patients with T2DM are usually treated with oral hypoglycemic drugs. The sodium glucose cotransporter 2 (SGLT-2) group is a type of oral hypoglycemic drug groups that plays a role in reducing HbA1c level. Empagliflozin is a drug included in the SGLT-2 group. Previous clinical trials showed that empagliflozin could lead to a reduction in HbA1c level. This study aimed to determine the effectiveness of using empagliflozin on HbA1C values in T2DM patients. This was a literature review study with data searching using three databases namely, Pubmed, Google Scholar, and Science Direct. The results obtained 10 literatures to be reviewed. All literatures showed that there was a significant effect of empagliflozin on HbA1c value of T2DM patients. In conclusion, type 2 diabetes mellitus patients using empagliflozin show significant decreases in HbA1c values. Keywords: type 2 diabetes mellitus; empagliflozin; HbA1c; sodium glucose cotransporter 2   Abstrak: Diabetes melitus tipe 2 (DMT2) merupakan jenis diabetes yang paling umum ditemukan saat ini. Perubahan gaya hidup menjadi salah satu penyebab timbulnya masalah DM. Penyandang DMT2 biasanya diterapi dengan obat hipoglikemik oral (OHO). Golongan obat sodium glucose cotransporter 2 (SGLT-2) merupakan salah satu jenis golongan obat OHO yang berperan dalam menurunkan kadar HbA1c, salah satunya ialah empagliflozin. Penelitian sebelumnya menunjukkan peran empaglifozin dalam uji klinis dapat menyebabkan penurunan kadar HbA1c. Penelitian ini bertujuan untuk mengetahui efektivitas penggunaan empagliflozin terhadap nilai HbA1C pada pasien DMT2. Jenis penelitian ini ialah suatu literature review dengan pencarian data menggunakan tida sumber basis data yaitu, Pubmed, Google Scholar dan Science Direct. Hasil penelitian mendapatkan sebanyak 10 literatur yang dilakukan review. Kesepuluh artikel memperlihatkan adanya pengaruh bermakna dari penggunaan empagliflozin terhadap nilai HbA1c dari pasien DMT2. Simpulan penelitian ini ialah penggunaan empagliflozin pada pasien diabetes melitus tipe 2 menunjukkan terjadinya penurunan nilai HbA1c yang bermakna. Abstract

Abstract: Measuring the level of patient satisfaction in hospitals must be carried out periodically, accurately, and continuously. Patient satisfaction can be achieved if the patient receives services according to what is needed and expected. The level of patient satisfaction describes the quality of services provided by the hospital. This study aimed to analyze factors related to outpatient satisfaction at Prof. Dr. R. D. Kandou Hospital, Manado. This was a quantitative study with a cross sectional approach. This study was conducted at Prof. Dr. R. D. Kandou Hospital with a total sample of 390 patients taken by simple random sampling. Data were obtained from questionnaires and were analyzed univariately, bivariately using the chi-square test, and multivariately using the logistic regression test. Statistical calculation using the SPSS program. The results of the chi-square test showed the p-values of relationships between patient satisfaction and the variable aspects, as follows:  tangibles p=0.000; responsiveness p=0.000; reliability p=0.000; assurance p=0.000; and empathy p=0.000. The results of multivariate analysis using the logistic regression test obtained that the highest Exp (B) value of 586.267 on the tangibles aspect. In conclusion, tangibles, responsiveness, reliability, assurance and empathy have significant relationships with patient satisfaction to the services at Prof. Dr. R. D. Kandou Manado. The most related factor with patient satisfaction is tangibles. Keywords: patient satisfaction; service quality; tangibles; responsiveness; reliability; assurance; empathy    Abstrak: Pengukuran tingkat kepuasan pasien di rumah sakit wajib dilakukan seara berkala, akurat dan berkesinambungan. Kepuasan pasien dapat tercapai apabila pasien menerima pelayanan sesuai dengan yang dibutuhkan dan diharapkan. Tingkat kepuasan pasien menggambarkan kualitas pelayanan yang diberikan oleh rumah sakit. Penelitian ini bertujuan untuk menganalisis faktor yang berhubungan dengan kepuasan pasien rawat jalan di RSUP Prof. R. D. Kandou Manado. Jenis penelitian ialah kuantitatif dengan desain potong lintang. Penelitian ini dilaksanakan di RSUP Prof. R. D. Kandou Manado dengan jumlah sampel 390 pasien yang diambil secara simple random sampling. Data diperoleh melalui kuesioner, kemudian dianalisis secara univariat, bivariat dengan menggunakan uji chi square, dan multivariat menggnakan uji regresi logistik. Penghitungan statistik menggunakan program SPSS. Hasil uji chi-square terhadap hubungan dengan kepuasan pasien mendapatkan untuk aspek tangibles p=0,000; responsiveness p=0,000; reliability p=0,000; assurance p=0,000; dan empathy p=0,000. Hasil analisis multivariat menggunakan uji regresi logistik mendapatkan nilai Exp (B) tertinggi yaitu 586,267 pada aspek tangibles. Simpulan penelitian ini ialah tangibles, responsiveness, reliability, assurance, dan empathy berhubungan bermakna dengan kepuasan pasien terhadap pelayanan di RSUP Prof. R. D. Kandou Manado. Abstract

A hospital without walls or known as a hospital without walls is very useful for patients who come to the hospital for treatment with referrals from the public health center as the entry point for referral diagnosis, this is to minimize the risk of infection transmission and other disease problems. The purpose of this study is to analyze the concept of a hospital without walls and the obstacles and challenges in providing refractive errors. The research was conducted at the UPTD Eye Hospital in North Sulawesi Province, from January to December 2022 using in-depth interviews with five research informants, namely the director, head of medical services, ophthalmologist, family doctor and patient. Instruments in the form of in-depth interview guidelines and a list of questions that have been provided. The stages of data processing are narrating, inferring, reducing, recording, sampling, unitizing. Data analysis is content analysis. The validity of the research results using triangulation of data sources and methods. The results of this study were obtained from the summary of the answers of the research informants that the concept of a hospital without walls all understand and know this concept to mitigate the accumulation of services in hospitals, but this concept has not been implemented because there are several factors such as standard operating procedures, human resources and infrastructure which is not yet possible for some FKTPs to be able to complete services at the community health center or family doctor, the obstacle presented by the informant is that there are still many patients who want to carry out direct services to the hospital without going to the health center or family doctor, another obstacle is the lack of tools for examining refractive errors and competence general practitioners to be able to make referrals in the diagnosis of disease. Hospital income will also increase if implementing this concept, if there is collaboration with related stakeholders such as BPJS Health, the Health Service, and other private parties, so that this will also have a good impact on the quality of service when hospital accreditation is carried out. The sophistication of information technology needs to be implemented as much as possible from the hospital so that it is easier so that patients do not pile up during services by registering online. The conclusion of this study is that the concept of a hospital without walls has not been implemented in the Eye Hospital of North Sulawesi Province because it has not followed standard operating procedures, quality competent resources and adequate infrastructure. FKTP Puskesmas in providing refractive error services have not provided complete supporting tools and the skills of FKTP doctors are still lacking, this will have an impact on assessing hospital accreditation in the quality of health services.
