Abstract

Human immunodeficiency virus (HIV), is a retrovirus family, that attacks T lymphocytes (CD4) cells in the immune system. The purpose of administering antiretroviral (ARV) therapy to HIV/AIDS patients is to increase the cluster of deficiency 4 (CD4). Given that ARV treatment is lifelong, patients must maintain excellent compliance with ARV medication. Side effects can sometimes lead to reduced ARV medication compliance. The quantitative research determines the correlation between the cognitive level and compliance with ARV medicine in HIV/AIDS patients with a cross-sectional approach. The study inadvertently involved 32 respondents. The data collection techniques were questionnaires on cognition, self-effectiveness, and compliance. Based on the Spearman rank, the test results indicate a significant correlation between self-efficacy (p-value: 0.003; r = 0.509), cognitive level (p-value of 0.001; r = 0.542), and the consistency of ARV drug intake in HIV/AIDS patients at the SMC Hospital Telogorejo VCT clinic. The research concludes the correlation between self-efficacy, cognitive level, and compliance. Nurses should expand their role as consultants and must participate in health education on HIV/AIDS patient management.

Abstract

Chronic Kidney Disease Stage 5 is a progressive and irreversible kidney function disorder. This condition necessitates that patients with chronic kidney failure receive kidney replacement therapy, such as haemodialysis. Haemodialysis is a method of treating patients with chronic kidney failure that requires a lot of money, not just 1-2 months. But it also takes quite a long time. Without family support, it is impossible to carry out haemodialysis treatment according to the scheduled schedule. The main problem faced by patients when they first undergo haemodialysis is decision-making. After receiving a diagnosis of chronic kidney failure, patients often feel uncertain about making decisions about haemodialysis therapy. This study aims to determine the relationship between family support and haemodialysis therapy decision-making. This research is a correlational analytical study with a cross-sectional approach. The number of samples in this study was 103, and data collection techniques used random sampling. Based on the Spearman Rank test, the research results showed that there was a relationship between family support and haemodialysis therapy decision-making (p-value = 0.000, p<0.05). This research suggests that families should provide good family support, including self-esteem support, information, instrumentality, social closeness, and motivation, to aid in haemodialysis therapy decision-making.

Abstract

Diabetes mellitus is a chronic metabolic disease characterized by elevated blood glucose levels due to impaired insulin secretion or function. The prevalence of diabetes continues to increase in Indonesia, particularly among adults and the elderly, requiring comprehensive and sustainable management. Diabetes management is not limited to pharmacological therapy but also includes non-pharmacological approaches such as dietary management, physical activity, and health education. One form of physical activity recommended for diabetes sufferers is diabetes exercise. This study aims to determine the effect of diabetes exercise on reducing blood glucose levels in diabetes mellitus patients enrolled in the Chronic Disease Management Program (Prolanis) at the Gunungpati Community Health Center (Puskesmas). The study used a quantitative approach with a one-group pretest-posttest pre-experimental design. A total of 30 respondents were randomly selected from the Prolanis patient population, and blood glucose levels were measured before and after the diabetes exercise intervention. The intervention was carried out routinely and structured over a certain period. The analysis using the Wilcoxon Signed Ranks test showed a statistically significant decrease in blood glucose levels after diabetes exercise, with a p-value of 0.000 (p < 0.05). The average blood glucose level decreased from 260.27 mg/dL to 211.33 mg/dL. This study concluded that diabetes exercise is effective as a non-pharmacological intervention in helping control blood glucose levels. Therefore, routine implementation of diabetes exercise in primary healthcare facilities needs to be promoted as a simple, effective, and affordable promotive and preventive strategy.

Abstract

Type 2 Diabetes Mellitus (DM) is characterized by hyperglycemia, which can lead to microangiopathic and macroangiopathic complications. One of the chronic complications is diabetic foot ulcer (DFU), which carries a high risk of infection and amputation. Preventing amputation can be achieved by accelerating wound healing. Dalethyne, a herbal product extracted through ozonization, contains essential oils, fatty acids, iodine, and peroxides. In wound care, Dalethyne acts as an antimicrobial agent that eliminates bacteria and promotes the formation of new tissue. This study aimed to examine the effect of modern Dalethyne dressings on wound healing among patients with diabetic foot ulcers. A quasi-experimental study with a non-equivalent control group pretest–posttest design was conducted at a wound care clinic in Semarang. Thirty participants with grade 2–3 diabetic foot ulcers were divided into an intervention group (Dalethyne dressing) and a control group (standard care). Wound healing was assessed using the DMIST scale before and after one week of treatment. Both groups showed improvement in DMIST scores post-treatment. However, the intervention group demonstrated significantly greater improvement (mean DMIST reduction of 48.50) compared to the control group (mean reduction of 16.50) (p = 0.001). Photographic evidence indicated more advanced granulation and epithelialization in the Dalethyne group. Modern wound care using Dalethyne effectively enhances wound healing in diabetic foot ulcers.
