Abstract

Objective: This case study aims to examine the effect of classical music therapy interventions on patients undergoing caesarean sections. Method: The approach chosen for the study is a descriptive method using a case study approach and nursing care approach that involves assessment, formulating nursing diagnoses, implementing interventions, and evaluating carried out for 3 days in the Lotus 2 room BLUD RSU Banjar City and referring to the SDKI, SLKI, and SIKI books. This case study research used one patient to be used as a subject, and the selection of participants used accidental sampling. Data collection techniques used by the author include interviews, observation, physical examination, and documentation studies using the SOAPIER method. Result: Nursing diagnosis Acute pain related to physical injury agents (surgery, birth canal trauma, episiotomy) with the diagnosis number D.0077 The patient experiences pain in the wound area post-sectio caesarea surgery in the abdominal region, which is indicated by the patient's complaint. They mention that pain is felt when engaging in excessive movement, there is a postoperative wound in the abdominal area, and the patient grimaced when moving. The pain scale is 8 (0–10), and with intervention given for 3 days with a duration of 15 minutes a day, acute pain nursing problems, which were originally a scale of 8 (0–10), have decreased to a scale of 4 (0–10). Conclusion: There is evidence to suggest that the use of classical music therapy can help reduce pain in patients who have had a caesarean section. From May 31, 2023, to June 2, 2023, nursing interventions involving classical music therapy were carried out for 15 minutes each day. During this period, the patient's pain decreased from level 8 on a scale of 0 to 10 to level 4 on the same scale. These changes indicate that the acute pain has been partially relieved.

Abstract

Objective: Congestive heart failure is a condition where the heart is unable to pump adequate blood to meet the needs of the network. If the supply in the blood cannot flow to parts of the body, it can cause the body to experience weakness, lethargy, and usually tire easily, so that it will experience problems, namely activity intolerance. One of the non-pharmacological interventions that can be done is relaxation therapy, one of which is back massage therapy, and measuring fatigue scores. This study was conducted to present nursing care to congestive heart failure clients with activity intolerance problems with back massage interventions and to measure client fatigue scores in the Kenanga Room at Banjar City Hospital. Method: The method in this study is a descriptive case study and a subjective questionnaire that aims to describe the nursing care of congestive heart failure clients with activity intolerance disorders with back massage interventions and measure fatigue scores using FACIT (Functional Assessment of Chronic Illness Therapy) questionnaires for clients in the memory room of RSUD Kota Banjar. The approach used in this case study is the process of nursing care, which includes assessment, intervention, diagnosis, implementation, and evaluation of nursing. Data collection techniques include interviews, observation, physical examination, and documentation studies. Subjects in this study were patients who experienced congestive heart failure with activity intolerance problems. Result: At the assessment stage, it is known that the client complains that the body is tired easily. The intervention given is energy management according to the SIKI book, and adding back massage therapy and measuring the client's fatigue score are carried out for 3 days. After the intervention was carried out, the client said that he was tired, and the client's fatigue score had increased. Conclusion: Nursing care for clients who experience congestive heart failure with activity intolerance problems using back massage therapy is effective in overcoming client fatigue.

Abstract

Objective: This study aims to present care with semi-Fowler's intervention as an effort to overcome shortness of breath in congestive heart failure patients with ineffective breathing patterns in the Kenanga Room, BLUD RSU Kota Banjar. Method: Descriptive analysis plus a case study technique make up the research design. Adult patients with congestive heart failure (CHF) and dysfunctional breathing patterns were the study's subjects. Interviews, observation, physical examinations, and documentation studies are a few of the methods of data collection. Result: The results of the case study at the assessment stage found that the client complained of shortness of breath. The intervention and implementation used to reduce shortness of breath is by giving the semi-Fowler's position by positioning the patient half-sitting or 45 degrees, while the shortness lasts. After the intervention, the client's breathing frequency decreased from 28 x/minute to 22 x/minute, while the subjective data from the anamnesis showed that the client's shortness of breath was reduced. Conclusion: The conclusion of nursing care in patients with congestive heart failure (CHF) with shortness of breath is well resolved. Intervention in the semi-fowler's position is effective in reducing respiratory frequency.
