Abstract

X is a manufacturing company engaged in galvalume processing. Hollow Galvalume is a leading product of CV. X. In each production process, there was product failure or defective products. If defective products occurred frequently, it could harm the company. Therefore, the objectives of study were to identify and analyse the causes of product defects and provide recommendations for improvements to minimize defective products. The methods used were Six Sigma and New Seventools. The Six Sigma method has 5 stages, namely Define, Measure, Analyse, Improve, and Control. The results of the calculation showed value of Defect per Million Opportunity was 14721 and sigma value of 3.69. Thus, the production of hollow products reached the Indonesian average industrial level, but the company wants to make improvements continuously. The results of improvement stage using New Seventools provided recommendations for improvements, among others, training for workers to understand work procedures better, regular schedule of machine maintenance, material inspection process monitoring, making a list of materials planned to be used, addition of special area and facilities for material warehouse.

Abstract

One of the products produced by PT. XYZ is in the form of a white plastic spoon in the production process, and problems are often found in the form of products having defects that significantly affect the quality, management system, and expenses of the company. To overcome this, an analysis was carried out using the Six Sigma method, and from the calculation results obtained a sigma value of 3.9 with a defect value of 10531 for a million production. From the results of the analysis carried out to push the company towards zero defects by seeking improvements to machine and human factors as the main factors for product defect problems, some of the recommendations for improvements given are to carry out maintenance on the machines and tools that exist in the company, to rearrange the machines damaged by changing parts so as not to hinder the ongoing production flow, supervising worker operators using Production Operations Standards (SOP), conducting a worker appraisal system according to applicable regulations so that workers are motivated to perform better. Abstract

PT XY, a precast concrete manufacturer, especially for electric poles, faces product quality problems in the form of defects, which harm quality, management, and production costs. This study aims to identify the root causes of defects and provide improvement solutions using the Seven Tools and Failure Mode and Effect Analysis (FMEA) methods. FMEA is used to identify potential and risk of failure and to formulate priority improvement actions. The results of the study showed three main types of defects: porous fins (53.53%), skin sticking (39.56%), and mottled (6.92%). The causal factors include methods, materials, humans, and machines/tools. FMEA analysis produces the highest Risk Priority Number (RPN) for porous fins (192) caused by imprecise molds (machine/tool ​​factors). The recommended solution is to inspect and repair the moulds periodically. Skin sticking defects have an RPN of 210, caused by poor oil quality (material factors); replacing the oil with an oil-based type is the solution. Mottled defects with an RPN of 144 are caused by the low slump of the casting mixture; the solution is to guide the mixing of raw materials according to standards. This study provides specific recommendations to reduce defects in electric pole products at PT XY, focusing on mould improvement, replacement of oil types, and standardization of raw material mixing.

Abstract

Abstract Production machine maintenance is an activity to maintain the machine's reliability to operate as planned. Apart from reliability, evaluating the effectiveness of the machine is also equally important. Therefore, this research has two main objectives: determining the system's criticality and increasing the effectiveness value of using forklifts using reliability analysis and machine's effectiveness (ME). The research began with collecting operation and maintenance data to calculate the initial conditions of machine reliability and effectiveness. The initial reliability calculation results were 36.79%. Availability is 98.89%, performance is 93.53%, and ME value is 92.49%. The performance value is still below world-class standards, so it needs to be improved. Next, rank the problem using the Pareto diagram for determining system criticality. Efforts to increase performance values to reach world-class standards are carried out by analyzing maintainability and implementing a policy that maintainability values are at least 50% for forklifts and their critical systems. As a result, the availability value increased to 99.74%, the performance value increased to 95.57%, and the ME value increased to 95.32%. So availability, performance, and ME meet the requirements because they exceed world-class standards. Therefore, companies can implement a forklift maintenance policy using maintainability of at least 50% reliability to increase the value of availability, performance, and ME.
