Abstract

Background: Leprosy has been known as the greatest imitator disease that can mimic a disease condition, including autoimmune disease. This case report aims to demonstrate and remind healthcare providers that leprosy infection may manifest with various autoimmune phenomena reminiscent of classic autoimmune disease, including systemic lupus erythematosus (SLE), and to prevent delayed treatment and further complications. Case report: A 28-year-old female patient with ulcers all over her body and amputation of her fingers and toes. Based on the positive antinuclear antibodies (ANA) test results, weak positive ANA profile band, anticardiolipin antibodies immunoglobulin M (ACA IgM), and complement C4 test, the patient has been diagnosed with systemic lupus erythematosus (SLE) since eleven years ago. There was no improvement during SLE therapy as the years passed. The patient developed leprosy symptoms such as madarosis, saddle nose, and facies leonine. Slit skin smear for leprosy acid-fast bacilli (AFB) from forehead and chin showed bacterial index +6, skin biopsy examination concluded lepromatous leprosy with bacterial index +5, and IgM anti-PGL-1 serological test examination 3185 u/ml. She was treated with steroids at the hospital and multidrug therapy for multibacillary leprosy (MDT MB) for one year. She had significant improvement after receiving medication. Conclusion: The diagnosis of leprosy is delayed because of the clinical similarities between the disease and SLE. The clinicians should know leprosy-specific symptoms to avoid inaccurate diagnoses and treatment delays.

Abstract

Land sales and purchases must be completed prior to the PPAT that intended to give legal clarity and protection to the owner of a land parcel or other registered right so that he may readily establish his ownership of the relevant right and for the efficient administration of property. Fundamental guidelines that must be followed when purchasing and selling land are the transaction procedure and the certificate document's validity. The government released Presidential Instruction of the Republic of Indonesia (Inpres) Number 1 of 2022 on Optimum Implementation of the National Health Insurance Program (JKN). The Presidential Instruction regulates the requirements for managing a number of public services such as buying and selling land, making SIM, STNK, SKCK, Haji and Umrah which must be registered as BPJS Health participants. In the Inpres Number 1 of 2022, the government instructed 30 ministries/agencies to require JKN-KIS for various purposes to ensure that all people are protected by health insurance. Some of these policies seem to have no relationship or connection, but they are actually very closely correlated. The government wants to ensure that all levels of society have health insurance, especially the upper middle class who have not been registered with the JKN-KIS program. BPJS Health participant cards is one of the requirements for buying and selling land. The BPJS Health participant card is a requirement in the application for registration services for the transfer of land rights or ownership rights to flat units due to buying and selling.

Abstract

Postherpetic neuralgia (PHN) is a common and debilitating complication of shingles, characterized by persistent pain that lasts for months to years after the resolution of the herpes zoster rash. While various treatments, such as anticonvulsants and opioids, are commonly used, they often fail to provide adequate pain relief and are associated with significant side effects. Botulinum toxin type A (BTX-A), traditionally used for muscle spasticity, has emerged as a promising treatment for PHN due to its ability to reduce pain through its neurotoxic effects. This literature review evaluates the safety and efficacy of BTX-A in treating PHN by reviewing 30 studies sourced from databases such as PubMed, Medline, and PubMed Central. The results indicate that BTX-A injections significantly alleviate PHN-associated pain, with many patients experiencing a reduction of up to 70% in pain severity. Additionally, BTX-A has been shown to improve patients' quality of life by enhancing sleep quality and reducing the need for oral medications. Although the results are promising, the review calls for further research to determine optimal dosing protocols and the long-term effects of BTX-A. Despite its limitations, BTX-A presents a potential therapeutic option for managing PHN, particularly for patients who do not respond well to traditional treatments.

Abstract

Premature graying of hair is defined as the occurrence of gray hair, depending on ethnicity, before the age of 30 in Africans, 25 in Asians, and 20 in Caucasians. Premature graying is an important cause of low self-esteem, which often interferes with sociocultural adjustment. Premature graying, known as premature canitis, the exact etiopathogenesis is still unknown, although it has been associated with disorders of premature aging, atopy, and autoimmune diseases. Many pathological conditions have been discussed in relation to premature canitis such as certain medications, deficiencies of Vitamin B12, vitiligo, hypothyroidism, and progeroid syndromes. Premature canitis may occur as an autosomal dominant condition only or associated with premature aging syndromes or various autoimmune. This condition needs to be differentiated from the various genetic hypomelanosis hair disorders. Patients with PGH should be evaluated for metabolic diseases and syndromes. Among the various pharmacotherapeutic management options, calcium pantothenate, PABA, and a combination of calcium pantothenate + PABA can be given. Some herbal agents such as Anu-tailam has been evaluated in a clinical research setting. Hair dye remains the primary treatment modality for cosmetic problems after nutritional supplementation. Lastly, addressing underlying pathological conditions often results in a reversal of the condition in many cases.

Abstract

Leprosy, is a chronic infectious disease caused by Mycobacterium leprae, an airborne intracellular bacillus and is considered a disease of poverty. Leprosy, with its associated physical and social morbidity, results in economic conditions that lead to malnutrition. The purpose of this study was to explore the role of vitamins, minerals, and nutrition in leprosy patients. The method used in this study was to explore articles and theories related to vitamins, minerals, nutrition and leprosy patients. The results of this study are that decreased antioxidant levels can lead to increased oxidative stress and complications of infectious diseases and leprosy. Because these nutrients have regulatory effects on innate and adaptive immune responses, a perfect balance in their concentrations is important to improve the immune response to pathogens.
