Abstract

Clinical phase of medical education includes understanding theory, mastery of skills and affective learning. The learning pro-cess were evaluated using a workplace-based assessment (WBA) approach. Direct observation of procedural skill (DOPS) was one of the WBA assessment methods. This research used qualitative methods with a phenomenological approach. The study po-pulation were clinical supervision lecturers and students of the clinical phase. The research method began with the pre-research stage; training for lecturers on how to implement DOPS, followed by the research stage; document study, observation, and focus group discussion (FGD). The results of the study found three major themes, namely concepts, ways of implementing, and obs-tacles to implementing DOPS. According to the teaching staff and students at FKG UK Maranatha, DOPS was an assessment of student procedural skills by direct observation of each stage of the procedural work. The teaching staff's perceptions about DOPS affected the way lecturers provide feedback to participants. As conclusion, full observation is needed to generate useful feedback for students. The effectiveness of DOPS is influenced by the availability of patients, the presence of examiners, and the availability of sufficient time to run DOPS.

Abstract

Abstrak Kusta dapat menyebabkan disabilitas permanen yang memengaruhi fisik dan psikis pasien. Stigmadiri dan stigma sosial menyebabkan pasien mengalami dehabilitasi yang dapat menurunkan kualitashidup penyandangnya. Penelitian ini merupakan studi pendahuluan yang bertujuan untuk mendapatkanprofil dehabilitasi pasien kusta termasuk orang yang pernah mengalami kusta (OYPMK). Penelitian inimenggunakan metode potong lintang untuk menilai profil dehabilitasi pasien kusta dan OYPMK menggunakanskala dehabilitasi Anandaraj di RS Kusta Alverno Singkawang, bulan Juli 2019. Delapan puluh delapan subjekdiikutkan dalam penelitian ini. Analisis data dilakukan menggunakan uji Mann Whitney untuk menilai hubungantipe kusta dan derajat dehabilitasi, serta uji Kruskal Wallis untuk melihat hubungan antara reaksi kusta danderajat disabilitas terhadap skala dehabilitasi. Analisis dilakukan menggunakan perangkat lunak SPSS formac. Subjek terbanyak adalah laki-laki, usia produktif, bekerja sebagai petani, berpenghasilan kurang dari 1,5juta per bulan, tidak bersekolah, sudah menikah, etnis Tionghoa, tinggal di dalam rumah yang padat, memilikikeluarga serumah yang menderita kusta, terdiagnosis kusta tipe multibasiler, pernah mengalami reaksi dantelah menyelesaikan pengobatan. Dari penelitian ini didapatkan bahwa tipe kusta dan disabilitas derajat noldan dua berhubungan dengan skala dehabilitasi Anandaraj (p<0.05). Stigma terhadap kusta hingga kinimerupakan masalah yang belum dapat diatasi. Stigma dan disabilitas akibat kusta berperan penting terhadapkejadian dehabilitasi yang menyebabkan penurunan kualitas hidup pasien kusta dan OYPMK. Kata kunci: Anandaraj, dehabilitasi, kusta, Singkawang.   Dehabilitation Profile of Leprosy Patients in Alverno Singkawang Hospital: a Preliminary Study Abstract Leprosy causes permanent disability that affects both physical and psychological aspect of the patient.Self-stigma and social stigma cause dehabilitation; therefore, reducing the quality of life of the patients. Thisresearch is a preliminary study to assess the dehabilitation profile of leprosy patients, including people whohave had leprosy (OYPMK). A cross-sectional method was used to assess the dehabilitation scale of leprosypatients in Alverno Leprosy Hospital, Singkawang, using the Anandaraj dehabilitation scale. Eighty-eightsubjects were included in the analysis. Mann-Whitney test was done to analyze the association betweentype of leprosy and dehabilitation scale, and Kruskal Wallis test was done to analyze the association betweenleprosy reaction and grade of disability with dehabilitation scale. Most subjects are male, productive age,farmers, less than 1.5 million wages per month, uneducated, married, Chinese ethnicity, living in a crowdedhouse, had a family with leprosy, diagnosed with multibacillary leprosy, had an episode of leprosy reaction danreleased from treatment. Type of leprosy and WHO disability grade 0 and 2 are related to the dehabilitationscale. Leprosy stigmatization is still a worrisome problem. Together with the disability, stigmatization affectsthe dehabilitation scale of leprosy patients leading to reduced quality of life. Keywords: Anandaraj, dehabilitation, leprosy, Singkawang.

Abstract

Sebagian kecil mahasiswa fakultas kedokteran adalah mahasiswa dengan riwayat pernah mengikuti program akselerasi ditingkat pendidikan dasar dan menengah, dan kelompok ini disebut sebagai akseleran. Akseleran merupakan siswa berbakat dan memiliki inteligensi serta kemampuan akademik yang tinggi, namun, akseleran umumnya imatur baik sosial, emosional maupun fisik. Dengan demikian dapat diprediksi akan mengalami penurunan kompetensi ketika berada pada tahap rotasi klinik. Tujuan penelitian ini adalah untuk melakukan evaluasi kinerja akseleran dalam pencapaian kompetensi di rotasi klinik. Penelitian ini menggunakan rancangan penelitian campuran, dengan pendekatan kualitatif lebih dominan. Responden penelitian ialah mahasiswa fakultas kedokteran di semester tujuh dan delapan dengan riwayat pernah mengikuti program akselerasi. Hasil penelitian ini mendapatkan bahwa kemampuan dasar akseleran berada pada skala baik dan baik sekali; sikap kerja cukup hingga baik, sedangkan kepribadian pada komponen stabilitas emosi serta daya tahan stres berada pada skala kurang hingga cukup. Nilai median kompetensi klinik akseleran sedikit lebih tinggi dibandingkan non-akseleran, tetapi secara satistik perbedaannya tidak bermakna. Kesimpulan penelitian adalah, kinerja akseleran dalam rotasi klinik cukup baik, namun akseleran menghadapi masalah non-akademik terutama aspek kepribadian yang dapat memengaruhi nilai akademik. Perlu pertimbangan seleksi calon mahasiswa kedokteran terhadap akseleran dengan menggunakan pemeriksaan psikologik untuk menilai kematangan kepribadian. Abstract

Indonesia ranks third in terms of countries with the highest number of leprosy in the world. This fact hasn't changed since 20 years ago. This shows the magnitude of leprosy management problems, both at the world level and national level. These problems range from the problem of finding new cases, continuity of the provision of various drug regimens, and control of the spread. In addition to problems related to the detection and handling of new cases, there are also problems in leprosy survivors. Survivors generally have disabilities due to the disease. The disability can continue to be aggravated over time, even though leprosy itself has healed long since. These disabilities vary from numbness, paralysis, shortened and claw-like fingers on the hands and feet, wounds resulting in amputations of the nose, ears, fingers and limbs. Such problems require a complete and continues management. Meanwhile, the management of leprosy, both in patients and survivors, has challenges both in terms of medical, policy, financing, and culture. Not to mention the challenges related to low socialistism, not many Indonesians know the problem of leprosy, especially knowing the magnitude of the problem. Pandemic conditions make things worse. This case report will explain the journey of leprosy survivor who has been amputated and has rehabilitation related to the maintenance and replacement of leg prosthetic in pandemic situation. It will describe the stigma, medical, and socioeconomic challenges that he has to go through. This case report is expected to be an inspiration in improving public, academics, policy makers, and stakeholder awareness, related to leprosy handling problems in Indonesia, both problems for patient and survivors. Thus, it is ultimately expected to reduce the level of disability, promote equal rights for people with disabilities, and support the realization of leprosy elimination nationally and internationally.

Abstract

Yaws, a neglected tropical disease, is still become the health problem in several countries. World Health Organization has proposed 2030 goals for yaws eradication after the previous target has not been achieved. Indonesia is still one of the largest contributors to yaws cases in Southeast Asia. The variety of lesions and the development of atypical clinical features make the yaws diagnose as a challenge. WHO guideline is used as a reference to determine the clinical manifestations, divided into early stage (infectious) lesions to advanced (non-infectious) yaws lesions. Yaws clinical features should be mastered by health workers in Indonesia, considering that the establishment of suspected cases begin with a clinical examination then confirmed by serological test. Several recent clinical surveillance had not reported early-stage lesions as the most common clinical finding, but rather had scars which was previously removed from the modified WHO guidelines. Missed diagnosis causes continually transmission.
