Abstract

Abstrak Faktor risiko seperti usia lanjut, jenis kelamin wanita, penyakit penyerta (depresi dan penyakit lain), status sosial ekonomi rendah menyebabkan insomnia. Penelitian mengenai prevalensi dan faktor-faktor yang mempengaruhi kejadian insomnia di Poliklinik Saraf RS DR. M. Djamil Padang belum pernah dilakukan. Tujuan penelitian ini adalah menentukan faktor-faktor yang mempengaruhi terjadinya insomnia di poliklinik saraf RS DR. M. Djamil Padang. Penelitian ini merupakan penelitian cross sectional. Jumlah sampel dihitung menggunakan rumus yang dikembangkan oleh Snedecor & Cochran dan didapatkan jumlah sampel 100 orang. Pengambilan sampel dilakukan secara acakdimana pasien yang memenuhi kriteria inklusi langsung menjadi sampel penelitian. Pengambilan data menggunakan kuesioner dan beberapa skala, Insomnia Severity Index, dan Beck depression inventory scale. Data dikumpulkan dari t 1 Juli sampai 31 Agustus 2013. Data ditampilkan dalam bentuk tabel distribusi frekuensi dan dilakukan analisis bivariatdan multivariat.  Kejadian Insomnia dialami oleh 38% (38 orang) pasien yang berkunjung ke poliklinik saraf RS DR. M.Djamil Padang dengan jenis kelamin terbanyak pada wanita 24(45,3%) dan pada kelompok umur  61-70 tahun (3,3%). Insomnia berhubungan dengan depresi (p= 0,00) dan tidak berhubungan dengan umur (p=0,472), jenis kelamin (p=0,111), status ekonomi (p=0,075), riwayat insomnia di keluarga (p=0,197). Depresi (p=0,00; OR=9,20) dan nyeri  kronik (p=0,031; OR=4.253) merupakan faktor yang dominan berhubungan dengan kejadian Insomnia. Research on the frequency of insomnia in DR. M. Djamil hospital Padang has never been done. The objective of this study was to determine the factors that influence the incidence of insomnia in neurology outpatient of DR. M. Djamil Hospital Padang. This study was a cross-sectional design. Sampling method was consecutive sampling, in which patients who met the inclusion criteria were included. Data were collected using questionnaires and some scales; Insomnia Severity Index (ISI) and the Beck depression inventory scale. Data were collected from the date of July 1 – August 31 2013. Data were displayed in the form of afrequency distribution table and performed bivariate and multivariate analysis. Insomnia was experienced by 38% (38 people) of patients who visited Neurology Outpatient of DR. M. Djamil Hospital Padang with the highest incidence in women 24 (45.3%) and in the age group 61-70 years (3.3%). Insomnia associated with depression (p = 0.00) and wasnot associated with age (p = 0.472), sex (p = 0.111), economic status (p = 0.075), family history of insomnia (p = 0.197). Depression (p = 0.00; OR=9.204) and chronic pain (p=0.031; OR=4.253) was the dominant factor associated with the incidence of insomnia.Keywords: sleep, insomnia, insomnia severity index, beck depression inventory scale

Abstract

Miastenia Gravis (MG) merupakan kelainan autoimun yang menyerang reseptor neurotransmitter di tautan neuromuskular dan menghambat terjadinya kontraksi otot. Kelemahan otot yang terjadi menyebabkan keterbatasan aktivitas fisik serta mempengaruhi setiap aspek dari kualitas hidup. Tujuan penelitian ini adalah untuk mengetahui gambaran kualitas hidup pada pasien miastenia gravis di RSUP Dr. M. Djamil Padang. Penelitian ini merupakan studi deskriptif dengan desain cross sectional melibatkan 38 pasien yang telah menyetujui informed consent dan berusia 20-79 tahun. Pengambilan sampel menggunakan teknik consecutive sampling. Instrumen yang digunakan adalah kuesioner WHOQOL-BREF. Kualitas hidup baik jika didapatkan skor akhir 51-100 dan kualitas hidup kurang jika didapatkan skor akhir 0-50. Analisis data menggunakan analisis univariat. Pengumpulan data dilakukan pada data rekam medis.Hasil penelitian menunjukkan responden paling banyak berusia 20-49 tahun (76,3%), berjenis kelamin perempuan (84,2%), MGFA kelas II (63,2%), dan lama menderita rata-rata 56,87 bulan dengan median 45 bulan. Kualitas hidup 89,5% baik, rata-rata skor 63,7. Kesimpulan penelitian ini adalah secara umum kualitas hidup pasien MG adalah baik, terdapat kecenderungan penurunan skor akhir kualitas hidup seiring dengan lamanya mederita dan meningkatnya kelas MGFA.

Abstract

Peningkatan angka harapan hidup menyebabkan pergeseran pola penyakit dari penyakit infeksi ke penyakit degeneratif seperti hipertensi, diabetes melitus, serta aterosklerosis. Aterosklerosis pada tahap awal yang dikenal dengan atherosclerosis asimptomatis sering ditandai dengan adanya Peripheral Arterial Disease (PAD). Ankle Brachial Index (ABI) adalah ratio tekanan sistolik ankle dan brachial yang dapat digunakan untuk menilai severitas oklusi arteri perifer yang merupakan gambaran penyumbatan arteri secara umum. Tujuan penelitian adalah untuk mengetahui hubungan antara nilai ABI dengan fungsi kognitif. Penelitian dilakukan pada 75 pasien lanjut usia yang berobat ke poliklinik saraf RS DR M Djamil Padang dengan metode consecutive sampling. Pemeriksaan fungsi kognitif menggunakan Indonesia Montreal Cognitive Assessment (InaMoCA). Data dianalisa dengan uji χ2 dengan nilai p<0,05. Hasil penelitian didapatkan gangguan sebanyak 48 orang (64%). Nilai ABI yang rendah sejumlah 31 orang (41,3%), normal 44 orang (58,7%). Tidak didapatkan hubungan langsung antara nilai ABI dengan fungsi kognitif (p>0,05). Namun setelah sampel dispesifikasikan berdasarkan jenis kelamin, hubungan antara ABI dengan fungsi kognitif pada wanita bermakna secara statistik (p<0,05). Kesimpulannya, tidak terdapat hubungan antara nilai ABI dengan fungsi kognitif pada lanjut usia, namun terdapat hubungan antara nilai ABI dengan fungsi kognitif pada lanjut usia berjenis kelamin wanita.

Abstract

Latar belakang: Revolusi digital memiliki pengaruh penting di bidang pendidikan seperti pada bidang lainnya. Pengaruh ini juga menyebabkan perubahan radikal di bidang pendidikan, seperti dalam hal pendekatan pengajaran dan pembelajaran. Flipped classroom merupakan suatu strategi pembelajaran yang tergolong baru. Strategi pembelajaran ini semakin berkembang dengan kemajuan teknologi, seperti akses internet serta software yang pendukung lainnya. Pada pembelajaran tradisional pendidik menyampaikan materi, lalu untuk menambah pemahaman materi tersebut maka siswa akan mengerjakan tugas di sekolah dan diberikan pekerjaan rumah.  Pada flipped classroom, peserta didik berpartisipasi dalam mempersiapkan pembelajaran melalui tontonan video, memahami powerpoint dan mengakses sumber belajar yang disediakan oleh pendidik baik melalui media online seperti e-learning. Beberapa penelitian membuktikan bahwa penerapan Flipped classroom dapat meningkatkan motivasi belajar peserta didik. Tujuan: Tujuan: Untuk meninjau secara singkat semua informasi terkini tentang flipped classroom di era digital. Kesimpulan: Revolusi digital memiliki pengaruh penting di bidang pendidikan seperti di banyak bidang lainnya. Pengaruh ini juga menyebabkan perubahan radikal di bidang pendidikan, seperti dalam hal pendekatan pengajaran dan pembelajaran, sehingga pendidik perlu menciptakan  bentuk pembelajaran aktif yang sesuai dengan karakteristik peserta didik zaman ini.

Abstract

Objectives: Although stroke is often viewed as occurring primarily in the elderly, it also strikes infants, children, young adults. Stroke in children are rare but increasingly important conditions due to the severity of their complications and diverse diagnostic differentials. It is not yet clearly understood and with multifactorial etiologies. When it comes to stroke, children are not just little adults. Stroke risk factors, symptoms, prevention efforts, and treatment are often different in children than in adults. Up to 80 % of children with ischemic stroke have cardiovascular disease. Case report: RS, An infant, aged two and half years old was admitted to DR. M. Djamil hospital Padang, with right hemiplegy and developed aphasia after activity. One year ago, he had history of cyanotic skin but neglected by his parents and never sought medical treatment. Physical findings on this patient are cyanosis and clubbing fingers. Cardiac auscultation revealed a pansystolic murmur grade III-IV best heard at the upper left sternal border. Chest radiograph showed a characteristic “bootshaped” heart, which is a reflection of Right Ventricular (RV) hypertrophy. The ECG showed right axis deviation. Brain CT Scan revealed a large hypodense lesion on the left cerebral hemisphere suggested as large brain infarction. Echocardiogram revealed tetralogy of fallot with a larged malaligned ventricular septal defect (VSD) with 60 % aortic override. Patient was managed conservatively. After two weeks of admission, patient was discharged with improvement.Conclusion : Ischemic stroke in this infant with tetralogy of fallot occurred via paradoxical embolism mechanism.

Abstract

Myasthenia gravis (MG) is a disorder of the neuromuscular junction that causes fluctuating muscle weakness. The etiology of this disease is due to the presence of antibodies that attack acetylcholine receptors on the postsynaptic membrane (autoimmune). Myasthenia gravis is a chronic disease and most MG patients fall into Generalized Myasthenia Gravis (gMG) within 2 years of onset. Myasthenic crisis is a serious condition and requires high-cost management. Abstract

Miller Fisher syndrome (MFS) is a rare variant of Guillan Barre Syndrome (GBS), with clinical symptoms of ophthalmoplegia, ataxia, areflexia. The diagnosis of MFS in this patient is based on a clear history supported by supporting examinations such as electrophysiological examination and CSF analysis. CSF analysis is often unsupportive at the onset of the disease, in which case the validated Brighton criteria may serve as a reference for establishing the diagnosis of GBS and its variants.

Abstract

Background: Disturbance sleep quality is often found in sufferers of human immunodeficiency virus (HIV). Disturbed sleep quality can affect immunity, which ultimately can increase patient morbidity and mortality. Impaired sleep quality in HIV sufferers is related to neurotoxicity due to the HIV virus, which damages sleep architecture. HIV infection can cause an increase in brain extracellular glutamate. Elevated glutamate plays a role in neuronal and glial damage and death. This study aimed to assess the relationship between plasma glutamate levels and sleep quality in HIV sufferers. Methods: The research uses a cross-sectional design. The samples were HIV sufferers in a polyclinic voluntary counseling test (VCT) internal medicine of Dr. M. Djamil General Hospital Padang, who met the inclusion and exclusion criteria. Samples are selected by consecutive methods. Sleep quality was assessed using a questionnaire called the Pittsburgh sleep quality index (PSQI). Plasma glutamate levels were measured using the ELISA method. Statistical analysis using SPSS with a p-value <0.05 was considered statistically significant. Results: The research sample consisted of 82 people. The median plasma glutamate level was 16.39 µg/mL. Impaired sleep quality was found in 45 (54.9%) HIV sufferers. There was no significant relationship between plasma glutamate levels (p= 0.506), age (p=0.795), gender (p=0.547), education (p=0.358), occupation (p=0.255), disease duration (p=0.348), stage (p=0.309) and type of ARV therapy (p=0.791) with sleep quality in HIV sufferers. From this research, a significant relationship was found between sleep quality, body mass index (BMI) (p= 0.015), and marital status (p= 0.039). Conclusion: There is no relationship between plasma glutamate levels and sleep quality in HIV sufferers. There are other factors that influence sleep quality, namely BMI and marital status.

Abstract

ABSTRACT   Myasthenia Gravis (MG) is an autoimmune neuromuscular disease that cause weakness due to the presence of autoantibodies that affect acetylcholine receptors (AChR) at the post synapse. Myasthenia gravis is a rare disease, but its prevalence is increasing. Classic clinical symptoms are characterized by fluctuating weakness. Diagnosis of myasthenia gravis requires a combination of clinical symptoms, physical examination and confirmatory tests in the form of bedside tests, serological tests, electrodiagnostics and imaging. Among the confirmatory tests available, serological tests has high sensitivity and specificity. In most cases of myasthenia gravis due to the presence of autoantibodies against AChR, other endplate proteins, such as muscle-specific receptor tyrosine kinase (MuSK) or lipoprotein-related protein 4 (LRP4) can be targets of autoantibodies. There are several examination methods for detecting autoantibodies in myasthenia gravis, several tests have been developed and commercialized such as radioimmunoprecipitation assay (RIPA), enzyme-linked immunosorbent assay (ELISA), there are also tests using antigen-expressing cells/cell-based assay (CBA), which is still being developed. The RIPA examination method is still the examination of choice due to its high specificity and sensitivity. ELISA can be an alternative to RIPA examination. CBA is more sensitive than RIPA but its use is still limited.

Abstract

Spasmofilia merupakan keadaan patologis dimana terjadi hiperiritabilitas saraf dan otot (neuromuskular) akibat adanya gangguan keseimbangan elektrolit,  terutama ion kalsium (Ca2+) dan ion magnesium (Mg2+) yang ditandai dengan munculnya kedutan otot, kesemutan dan spasme karpopedal. Hipoparatiroidisme merupakan keadaan dimana produksi hormon paratiroid yang tidak adekuat. Keadaan ini jarang ditemukan dan umumnya disebabkan oleh kerusakan atau pengangkatan kelenjar paratiroid pada saat operasi tiroid. Laporan kasus ini  membahas mengenai pasien perempuan berusia 25 tahun dengan spasmofilia  dan hipoparatiroidisme sekunder  ec tiroidektomi.
