Abstract

ICD coding is usually done by a coder who assigns the ICD code according to the Doctor's clinical diagnosis. However, because coders need to master specific skills, such as knowledge in the field of medicine, coding rules, and medical terminology, manual coding can be costly, time-consuming, and inefficient. Based on this, developing a computationally accurate approach to automatic ICD encoding is imperative. This literature study aims to provide an overview of automatic ICD in terms of the dataset and classification method used. The literature study results show that automatic ICD research to improve the accuracy of diagnosis coding has been done and is still a challenge today. Most of the datasets used are the public MIMIC-III dataset, while automatic ICD classification is the current research trend with deep learning. The deep learning algorithms that are widely used include CNN, RNN, and LSTM. The resulting accuracy based on the dataset and classification method is very diverse. Future research still has many opportunities to contribute and improve the correct classification method to improve automatic ICD accuracy.

Abstract

Diagnosis codes are used for policy-making and determining healthcare costs, so they must be of high quality. Therefore, evaluation of diagnosis codes is necessary. The evaluation includes consistency (reliability), accuracy (validity), completeness, and timeliness. This study aimed to evaluate the quality of diagnosis coding for pediatric cases in inpatient medical records at Bagas Waras Regional General Hospital, Klaten, in 2023. This was a descriptive study with a retrospective approach. The sample used was 95 pediatric medical records, using a simple random sampling technique. The research instruments were ICD-10, observation guidelines, and interviews. Data processing involved collecting, editing, classifying, tabulating, and presenting data. Data analysis was descriptive. The quality of diagnosis codes can be seen from indicators such as code consistency (97.89%) and inconsistency (2.11%). Code accuracy (91.57%) and inaccuracy (8.43%). The code completeness was 96.84% and the code incompleteness was 3.16%. The code timeliness was 98.94% and the code inaccuracy was 1.06%. The diagnosis accuracy with examination and action was 100%. Factors that influence the quality of diagnosis codes are medical personnel (doctors), medical records personnel (coders), and other health workers. The author recommends establishing regulations regarding time limits for coding diagnoses and actions. Hospitals should hold training or seminars on coding more regularly to improve the skills and knowledge of coding staff and support code quality.

Abstract

Pelaksanakan pengkodean yang dilakukan seorang coder membutuhkan informasi yang lengkap, salah satu sumber informasi bagi coder adalah informasi yang tercatat di dalam formulir termasuk penulisan terminologi medis. Kasus genitourinary merupakan kategori penyakit tidak menular, penyakit tidak menular di Indonesia menjadi salah satu isu kesehatan prioritas Kemenkes.penelitian yang digunakan dalam penelitian ini adalah analitik dengan menggunakan metode retrospektif, pengumpulan data menggunakan observasi dan wawancara, teknik sampling yaitu sampling quota dengan jumlah populasi 466 dokumen dan didapat 100 sampel.Hasil  pembahasan menunjukkan terdapat hubungan antara ketepatan penulisan terminologi medis terhadap keakuratan kode diagnosis utama dengan persentase ketidaktepatan yaitu 48% disebabkan oleh ketidaksesuaian dengan terminologi medis pada ICD-10, penggunaan Bahasa Indonesia serta penggunaan singkatan yang tidak sesuai dengan buku singkatan dan ICD-10. Sedangkan persentase ketidakakuratan yaitu 47% yang disebabkan oleh tulisan dokter yang tidak terbaca dan keterbatasan ilmu pengetahuan coder. Data tersebut dianalisis dengan menggunakan SPSS versi 25.0 sehingga dapat diketahui p=0,003, berdasarkan hasil tersebut maka p<0,05.Kesimpulan  dari dua variabel diatas yaitu bahwa terdapat hubungan antara ketepatan penulisan terminologi medis tehadap keakuratan kode diagnosis utama pada kasus sistem genitourinary  Saran dari penelitian ini adalah sebaiknya petugas coding memperbarui ilmu dengan mengikuti pelatihan dan lebih teliti lagi dalam mengkode diagnosis, dokter dalam menuliskan diagnosis lebih jelas dan konsisten sehingga memudahkan coder dalam melakukan pengkodean.
