Abstract

Morfologi saluran akar gigi molar mandibula ketiga memiliki variasi yang lebih kompleks dibandingkan gigi molar lainnya.   Pada gigi molar ketiga sering dilakukan pencabutan, namun dalam keadaan tertentu gigi molar ketiga dapat dipertahankan. Perawatan saluran akar satu kunjungan merupakan pilihan untuk terapi kasus ini. Tujuan laporan kasus ini untuk memperlihatkan keberhasilan dari perawatan saluran akar satu kunjungan pada gigi molar ketiga nekrosis pulpa disertai restorasi resin komposit dengan pasak tapered self theading. Pasien wanita 20 tahun datang ke RSGM Prof Soedomo FKG UGM dengan keluhan sakit saat pengunyahan pada gigi molar ketiga dan positif pada perkusi. Gambaran radiografis menunjukkan restorasi yang tidak sempurna, terdapat celah antara kavitas dengan restorasi. Rencana perawatan pada kasus ini, perawatan saluran akar satu kunjungan dan resin komposit dengan pasak tapered self threading sebagai restorasi akhir. Kesimpulan dari perawatan saluran akar satu kunjungan memiliki rekontaminasi mikroorganisme yang lebih kecil dibandingkan dengan multi kunjungan sehingga menjamin keberhasilan perawatannya. Restorasi resin komposit secara direkdengan pasak tapered self threadingmerupakan restorasi alternatif pasca perawatan endodontikkarena lebih cepat dan kuat.ABSTRACT: Composite Resin Restoration Using Tapered Self Threading Poston Left Mandibular Third Molar. Root canal morphology of mandibular third molar has more complex variation than the other molars. In third molar, the extraction teeth are often executed; however, the third molar can be maintained in other conditions. One visit root canal treatment is a therapy option for this case. The purpose of this case report is to show the success of one visit root canal treatment in third molar with pulp necrosis by restoring the composite resin through tapered self-treading post. A 20 year-old female patient who came to the Prof Soedomo RSGM, FKG UGM complained about the pain when chewing food on her third molar and positive in percussion. The radiographs showed that there was an incomplete restoration. There was a gap between cavities with restoration. The treatment plan for this case was one visit root canal treatment and composite resin with tapered self-threading post as final restoration. From the case, it can concluded that one visit root canal treatment results in a smaller chance for microorganism recontamination than the multi-visit in order to ensure the success of the treatment. Direct composite resin restoration with tapered self-threading dowel is an alternative restoration after endodontic treatment because it works out faster and more retentive.

Abstract

Perforation is one of iatrogenic factors responsible for endodontic failure. Root canal perforation can occur at the cervical, mid-root, or apical levels. Non-surgical (conservative) perforation repair offers less tissue destruction and easy isolation during treatment. Objective: To explain the management of apical third root perforation using the conservative technique. Case Report: This case report describes a 29-year-old patient who came for management of right maxillary lateral incisor with apical third root perforation on the labial aspect. The location of apical third root perforation was evaluated using cone beam computed tomography (CBCT). Root perforation was sealed using mineral trioxide aggregate (MTA). MTA was applied in conservative technique with hand filling. MTA was dispensed into the original canal while maintaining the patency of the perforated canal, followed by dispensed of MTA into the perforated canal. Conclusion: Root perforation should be identified as soon as possible and could be easily examined using CBCT. Non-surgical root perforation treatment is recommended in intact periodontal attachment and in absence of inherent complication. Repairing the root perforation promote the proper healing of the periapical tissue and increase the success rate of retreatment.

Abstract

Immediate Dentin Sealing (IDS) is a conservative procedure performed to protect the exposed dentin surface after tooth preparation, especially for indirect restorations. Indirect restorations are a common procedure in dentistry for replacing the crown structure of teeth, including various types such as jacket crowns, bridge prostheses, endo crowns, inlays, and veneers. The tooth preparation process can expose dentin tubules, which can trigger pulp tissue irritation and the risk of infection, as well as interfere with the adhesion of the final restoration. Post-IDS surface treatment methods can help enhance the bonding strength. The research aims to investigate the influence of different post-IDS surface treatment on the shear bond strength of lithium disilicate.The study involved 30 upper premolar teeth, divided into three groups. After crown portion buccal surface preparation, all samples underwent IDS, followed by immersion in water and 24-hour incubation. Subsequently, each group received a specific surface treatment: Group I with nylon brush, Group II with nylon brush + pumice, and Group III with sandblasting using aluminum oxide. Lithium disilicate was then cemented on all samples, followed by another 24-hour incubation before shear bond strength testing. Shear bond strength values in MegaPascals (MPa) were analyzed using one-way ANOVA and post hoc Games-Howell tests with a 95% confidence level.The statistical analysis revealed a significant influence of the different treatment groups on shear bond strength (p < 0.05). In conclusion, this study demonstrates a significant impact of varying post-IDS surface treatments on the shear bond strength of lithium disilicate.

Abstract

Background: Free radicals produced by hydrogen peroxide after teeth bleaching which diffuse through the enamel and dentinal tubules can trigger inflammation of the pulp.  To overcome this negative side effect, exogenous antioxidants have been proposed.  This research was conducted to determine the effect of natural antioxidants, namely 5% and 10% green tea, which was applied on rat molars teeth that were bleached using 40% hydrogen peroxide. Methods: Fifteen molar teeth of male Wistar rats were bleached using 40% hydrogen peroxide on their occlusal surface. Furthermore, the control group (Group I) rinsed using warm distilled water, while in Group II and Group III, the teeth were rinsed using 5% and 10% green tea extract gel. Wistar rats were sacrificed on the fifth day after treatment, followed by hematoxylin-eosin staining. Histological examinations were observed under a light microscope using 400x magnification and the number of the neutrophil were counted. Result: One-way ANOVA test showed that there was a significant effect of green tea extract gel concentration on the number of neutrophils (p 0.05).  Post hoc test results showed that there were significant differences in all pairs of groups compared (p 0.05). Conclusion:  Group III which was treated using10% green tea extract gel has the lowest number of neutrophil compared to Group II which was treated using 5%  green tea extract hel and the control group.

Abstract

ABSTRACTIntroduction: Root canal treatment (RCT) of mandibular third molars is often considered challenging due to anatomical variability, limited accessibility, and difficulty achieving proper isolation. As a result, extraction is frequently chosen. This case report outlines a successful single-visit RCT on a right mandibular third molar with a curved canal and Class II disto-occlusal caries. Case Report: A 30-year-old female presented with a large cavity on her right lower molar and requested a restorative treatment. Three weeks earlier, devitalization and temporary restoration had been performed by another dentist. Clinical examination revealed no periapical lesion, and percussion testing was negative. Radiographic analysis showed a slight curvature in the distal canal. A pre-curved K-File and the One Curve System were used for instrumentation. The canal was obturated in a single visit, followed by restoration using direct composite and short fiber-reinforced composite. Matrix and PTFE tape were utilized for proper adaptation in the distal area. Conclusion: With accurate diagnosis, understanding of anatomical complexity, and appropriate technique, single-visit RCT on mandibular third molars is achievable. This case supports the viability of conservative treatment for necrotic third molars with asymptomatic apical periodontitis, avoiding unnecessary extraction.Keywords single visit, root canal treatment, third lower molar, curved canal, composite restoration. ABSTRAKPendahuluan: Perawatan saluran akar (RCT) pada molar ketiga mandibula menantang karena variasi konfigurasi saluran, kesulitan akses ke area posterior, dan tantangan dalam isolasi. Akibatnya, pencabutan sering menjadi pilihan. Laporan kasus ini mendeskripsikan protokol RCT kunjungan tunggal pada molar ketiga kanan mandibula dengan saluran melengkung dan karies disto-oklusal kelas II. Laporan Kasus: Seorang wanita 30 tahun datang dengan kavitas besar pada molar ketiga kanan bawah dan menginginkan perawatan tanpa pencabutan. Tiga minggu sebelumnya, dokter lain telah melakukan devitalisasi dan restorasi sementara. Pemeriksaan klinis menunjukkan tidak ada lesi periapikal dan uji perkusi negatif. Radiograf menunjukkan kelengkungan ringan pada saluran distal. RCT dilakukan menggunakan K-file pra-lengkung dengan sistem One Curve. Obturasi dilakukan dalam satu kunjungan, diikuti restorasi komposit langsung yang diperkuat dengan komposit serat pendek. Area distal direstorasi menggunakan matriks dan pita PTFE. Simpulan: RCT kunjungan tunggal pada molar ketiga mandibula dengan pulpa nekrotik dan periodontitis apikal asimptomatik dapat berhasil dilakukan dengan diagnosis akurat dan teknik yang tepat. Pendekatan konservatif ini menunjukkan bahwa pencabutan bukan satu-satunya pilihan.Kata kunci sekali kunjungan, perawatan saluran akar, molar ketiga, saluran melengkung, restorasi komposit.

Abstract

Immediate dentin sealing (IDS) is a technique involving the application of adhesive material to freshly prepared dentin for indirect restorations. This method prevents contamination by saliva, debris, and bacteria from penetrating dentinal tubules, which could otherwise compromise the bonding strength of restorative materials. This study aimed to evaluate the effects of different adhesive types and application techniques in IDS on the shear bond strength (SBS) of lithium disilicate. The adhesive systems investigated included an eighth-generation adhesive and a universal adhesive, each applied using both total-etch and self-etch techniques. Twenty-eight upper premolars were divided into four groups, with buccal crown surfaces prepared for IDS treatment as follows: Group I – eighth-generation adhesive with total-etch; Group II – eighth-generation adhesive with self-etch; Group III – universal adhesive with total-etch; and Group IV – universal adhesive with self-etch. Lithium disilicate was cemented onto the dentin surfaces treated with IDS and then immersed in distilled water for 24 hours prior to SBS testing. The results showed that both adhesive type and application technique significantly influenced SBS (p < 0.05). No significant differences were observed among Groups II, III, and IV. However, Group I demonstrated the lowest SBS. In conclusion, the findings indicate that both the choice of adhesive and application technique in IDS significantly affect the shear bond strength of lithium disilicate.
