Abstract

Background: The National Health Insurance is one of the government’s efforts to improve community access to health services. The government has fixed the premiums to be paid by community, except for underprivileged community. The aim of the study was to identify Ability to Pay (ATP) and Willingness to Pay (WTP) of the national health insurance premium.Methods: A descriptive study which involved 210  housewives who were chosen by rapid survey method was conducted from September to November 2013 in Cipacing village, Jatinangor, Sumedang, West Java. Data collection was using questionnaire to obtain level of ability and willingness to pay the health insurance premium. The results were compared to the required premium by the government (Rp 22,000,-).Results: Most of the respondents were only housewives, but there were still respondents who were private workers. Most of them were 20–39 years old. About 57.6% of the respondents were able to pay for the required premium, but Only 17.4% of the them were willing to pay according to the required premium.Conclusions: The ATP of the respondents are higher compared to the WTP, meaning that most of the respondents are able to pay the requires premium but are not willing to pay it. Abstract

Background: Systemic Lupus Erythematosus (SLE) is an autoimmune disease with wide range of clinical symptoms. The patients frequently complain musculoskeletal involvement during the active state of the disease. Musculoskeletal manifestation in SLE patients is an important sign in making early diagnosis and monitoring treatment response. This study aims to determine the presentation of musculoskeletal involvement of SLE patients in Dr. Hasan Sadikin General Hospital Bandung.Methods: a descriptive cross-sectional quantitative study done by interviewing SLE patients concerning musculoskeletal manifestation as the primary data and tracking their medical record as the secondary data. Study was conducted between September to November 2016 in Rheumatology Clinic Dr. Hasan Sadikin General Hospital Bandung.Result: Ninety-seven SLE patients, 91 females (93,81%) and 6 males (6,19%), were enrolled in this study with mean age 35.12 (±10.91) years. The three highest proportions of muskuloskeltal manifestations were arthritis of the knee (84,5%), myalgia of upper back(40,2%), and muscle weakness (15,5%). We did not find any Jaccoud’s Arthropathy (JA) and tendinitis manifestation. Osteoporosis were occured in 4 patients (4,12%), whereas gout arthritis, spondytlitis, osteoarthritis, rotator cuff syndrome, and rhupus wereonly occured in one patient (1,03%), respectively.Conclusion: The common musculoskeletal manifestation in SLE patients were arthritis of knee, myalgia of upper back, and muscle weakness. Only small portion of patients suffered from osteoporosis, gout arthritis, spondytlitis, osteoarthritis, rotator cuff syndrome, and rhupus.Keyword: musculoskeletal manifestation, musculoskletal involment, SLE

Abstract

Background: Osteoarthritis (OA) is the most common musculoskeletal disease. World Health Organization (WHO) estimates that 10% of the aged over 60 year population have this disease. The aim of OA treatment is to reduce pain, which is the most OA patients chiefcomplaint. People in Indonesia are very interested in use of herbal therapies from original traditional plant to treat pain now, one of the traditional plants that are known havea benefit is Phaleria macrocarpa or Mahkota Dewa fruit. Phaleria macrocarpa has been shown to decrease the degree of inflammation of OA animal model experiments. In order to know what is the effect of this fruit extract to reduce degrees of pain and change the levels of IL-1, IL-6, TNF-α in the blood as the marker of inflammation of patients with knee OA, and what is the effects to liver, kidney and haematology in the Indonesian population hasnot been investigated Methods: The research method is an experimental study and the research design is PROBE (Prospective Randomized Open End Blinded Evaluation), to evaluate the efficacy of the extract Mahkota Dewa fruit (Phaleria macrocarpa fructus) 330 mg (Super Mahkota POMTR 053 345 491) compared to meloxicam 7.5 mg in patients with knee osteoarthritis. The study population was outpatients with knee OA at Rheumatology Clinic Dr Hasan Sadikin Hospital Bandung. Patients are given Phaleria macrocarpa 330 mg or meloxicam 7.5 mg once a day for 14 days. Observations were made to evaluate the degree of pain as measured by VAS and Lequesne index at day 0, day 14 and measured again at day 28,after they are not taking the extract anymore.Results: Phaleria macrocarpa 330 mg is equal to meloxicam 7.5 mg in reducing the degree of pain as measured by VAS (p=0.78) and the Lequesne index (p=0.51). Our finding, t here is no effect of decreasing the proinflammatory cytokine IL-1 (p=0.72), IL-6(p=0.53) and TNF-α (p=0.07) in the blood of both groups. Safety analysis shown that this extract is safe for consumption.Conclusions: Phaleria macrocarpa 330 mg equal to meloxicam 7.5 mg in reducing the degree of pain however there is no effect on reducing proinflammatory cytokines in the blood of OA patients who had received therapy for 14 days in both groups. There is no adverse effects found on hematological, liver function and kidney function after consumption this plant’s fruit extract.Keywords: osteoarthritis, mahkota dewa, phaleria macrocarpa

Abstract

BackgroundDengue virus infection that most frequently occur in tropical and subtropical regions can cause many symptoms, one of which is a decrease in thrombocyte count. Recent studies showed that guava leaf extract can increase the thrombocyte count in rats. The present study aimed to determine the effect of guava leaf juice in increasing the number of megakaryocytes in the bone marrow of thrombocytopenic mice.MethodsThis study was of experimental design. The study subjects were 24 mice (Mus musculus). The mice were randomly divided into four groups that were subjected to intervention for 14 days. Group 1 was given guava leaf juice (56 mg/kg) and quinine (14 mg/kg), group 2 guava leaf juice (56 mg/kg) only, group 3 was given quinine (14 mg/kg) and group 4 distilled water. After 14 days, from the bone marrow of the femoral bones of each of the mice, hematoxylin eosin stained histologic preparations were made. Anova test was used to analyze the data.ResultsThe mean megakaryocyte count per field of view in group 1 (2.83), group 2 (3.30), group 3 (2.24) and group 4 (2.93). Anova test results for all groups showed significant differences between groups (p<0.05). The mean megakaryocyte count was increased in group 1 compared to group 3, but the difference was statistically not significant (p=0.206).Conclusion Guava leaf juice can increase the megakaryocyte count in the bone marrow of thrombocytopenic mice. This suggests a potential role of guava leaf juice in improving the platelet count in thrombocytopenic disorders.

Abstract

Background: Osteoarthritis is a joint degenerative disease characterized with structural changes of joint. The structural changes can be assessed with radiological assessment. Major clinical manifestations may include pain, inflammation, and stiffness, resulting in limitations for performing daily activities. The purpose of this study was to determine the correlation between radiographic grading of osteoarthritis, pain severity, and lower extremities functional status in patients with knee osteoarthritis.Methods: The study involved 29 patients with knee osteoarthritis in Dr. Hasan Sadikin General Hospital, Bandung and Bandung District General Hospital during the period of August to October 2014 using the cross sectional method. This study used NRS (Numeric Rating Scale) for pain severity assessment, Kellgren-Lawrence criteria for radiographic grading of osteoarthritis, and Modified Lower Extremity Funcional Scale for functional status assessment. The data were analyzed using statistical analysis tools. If the data were normally distributed, the correlation test will be carried out  by using the Pearson correlation test and if the data were not normally distributed then the Spearman’s Correlation test will be used.Results: Correlation coefficient between severity of pain and functional status is r= -0,439 with p value= 0.17 (p<0.05). There was no significant correlation between radiographic grading of osteoarthritis and functional status (p > 0.05).Conclusions: The severity of pain correlates with the functional status of patients with osteoarthritis of the knee, while radiographic grading of osteoarthritis does not correlate with functional status. Keywords:

Abstract

Objective: To determine the correlation between serum levels of IL-6 and MRSS in systemic sclerosis patients.Methods: This was a crossectional observational study on patients with SSc visiting the Rheumatology Clinic of Dr. Hasan Sadikin General Hospital Bandung, Indonesia from January 2019 to December 2020. Registered SSc patients, with validated MMRS by a rheumatologist, and available biological materials for IL-6 measurement were included. Patients with other autoimmune diseases, acute infection, diabetes mellitus, and obesity were excluded from this studyResults: Of the 51 SSc patients, 42 patients met the inclusion criteria. Women comprised the majority of these patients (95.2%), while patients’ mean age was 43±12 years. The mean duration of disease was 20 months, with diffuse SSc (78.6%) as the most frequent type. Bivariate analysis showed that IL-6 was positively correlated with MRSS (100-scale) with r=0.397 and p=0.005.Conclusion: There is a medium positive correlation between MRSS and interleukin-6 serum among systemic sclerosis patients visiting Dr. Hasan Sadikin General Hospital Bandung, Indonesia.

Abstract

Systemic lupus erythematosus (SLE) is an autoimmune disease which causes chronic inflammation and may impact various organs. Hematologic abnormality, including thrombocytopenia, is a common clinical manifestation in SLE, ranging between 7-30%. Thrombocytopenia in SLE has been proven to correlate with a more active disease and a worse prognosis. Most of the time, it gets hard to determine the underlying cause of thrombocytopenia. Immature platelet fraction (IPF) examines immature thrombocyte at peripheral blood and can be used to determine whether thrombocytopenia happens because of a decreased production or increased peripheral thrombocyte destruction. This study was done to evaluate immature platelet fraction value in SLE patients with thrombocytopenia. This was a cross-sectional descriptive observational study. Sample was taken from SLE inpatients and outpatients at Dr. Hasan Sadikin Bandung Hospital. There were 24 subjects included in this study, which counts 7.4% of SLE population. The mean platelet count was 56,870 ±28,933 /mm3. IPF values ranged from 0.9-3.2%, with median 5.7%. The median IPF in moderate-severe thrombocytopenia group was 7.5%, higher than that of mild thrombocytopenia group (4.2%). It can be concluded that IPF values were increased in most SLE patients with thrombocytopenia compared to normal population. It suggests that increased platelet destruction plays an important role in the pathogenesis of SLE thrombocytopenia. A wide range of IPF values shows multifactorial nature of thrombocytopenia causes in SLE patients.
