Abstract

Antidiabetic effects of water extract of ciplukan (Physalis angulata L.) herbs has been done with the test method of alloxan diabetes induction. Water extract was fractionated using n-hexane, ethyl acetate and water.The results showed that ciplukan water extract dose of 10 mg/kgbw and the water fraction dose of  4.84 mg/kgbw have similar effect with glibenclamide dose of  0.65 mg/kgbw. The phytochemical screening showed that ciplukan water extract contains alkaloids, flavonoids, saponins, polyphenols, steroids and triterpenoids, monoterpenoids and sesquiterpenoids. ABSTRAK Telah dilakukan uji efek antidiabetes ekstrak air herba Ciplukan (Physalis angulata L.) dengan metode uji diabetes induksi  aloksan. Selanjutnya ekstrak air difraksinasi dengan pelarut n-heksan, etil asetat dan air. Hasil uji membuktikan bahwa ekstrak air herba ciplukan dosis 10 mg/kgbb dan fraksi air 4,84 mg/kgbb mempunyai efek antidiabetes yang sama dengan pembanding glibenklamid dosis 0,65mg/kgbb. Hasil penapisan fitokimia menunjukkan bahwa simplisia dan ekstrak air herba ciplukan mengandung alkaloid, flavonoid, saponin, polifenol, steroid dan triterpenoid, monoterpenoid dan seskuiterpenoid.

Abstract

Hypertension is a non-communicable disease that has a high prevalence. Blumea balsamifera(Asteraceae) is one of the plants that has traditionally been used to treat high blood pressure. The aim of this study is to evaluate antihypertensive effect of ethanol extract of Blumea balsamiferaleaves on animal models induced epinephrine. The extract was made in 50% ethanol. Extraction was done using a set of Soxhlet tools. The antihypertensive effect test was carried out on male Wistar rats using epinephrine 0.25 mg/kg bwas inductor and blood pressure was measured indirectly through the tail using Kent Scientific's CODA system. The dose of extract was 8.75; 17.5 and 35 mg/kg bw, while propranolol at dose dose 1.8 mg/kg bw was used as comparator. The parameters were diastolic and systolic blood pressure. The results showed that the ethanol extract at doses of 8.75; 17.5 and 35mg/kg bw can prevent the increase in diastolic and systolic blood pressure in animals model induced epinephrine when compared to controls (p <0.05), where the best results is shown by the ethanol extract at dose of 17.5 mg/kg bw. The ability of the ethanol extract is equivalent to propranolol (p> 0.05). It can be concluded that Blumea balsamiferaleaves can be developed and have the potential to be antihypertensive.

Abstract

Rheumatoid arthritis is an autoimmune disease characterized with chronic joint inflammation. Chronic inflammation leads to liver function abnormality. Previous study showed that Physalis angulata L. had antiinflammatory, hepatoprotector, and immunosupressan effect. This research aims was to evaluate the effect of ethanol extract of P.angulata as adjuvant on liver function of RA animals model. Animals was induced with 0.1 mL complete freund adjuvant (CFA) intraplantar. The dose of P.angulata extract was 50 mg/kg bw po. The extract was given for 10 days as adjuvant of 0.1125 mg/kg bw methotrexate. Evaluation of liver function consisted of measurement of aminotransferase (AST and ALT), alanine phosphatase (ALP), organ index, and histology. The result showed that CFA could increase the level of AST and ALP and decrease the level of ALT and organ index of control group. The extract of P.angulata as adjuvant of methotrexate could decrease the level of AST and ALP and increase the level of ALT and organ index compare to control group, but could not reach its normal value. Histology showed that the extract also could improve liver regeneration. The extract of P.angulata at dose of 50 mg/kg bw used as adjuvant could improve liver function of RA animals model.

Abstract

Cecendet (Physalis angulata L.) is a herbal plant commonly used for traditional remedies in Indonesia to prevent inflammation in rheumatic disease. Several studies have determined that cecendet has an immunosuppressant activity towards pristane-induced lupus animal model. A preliminary study has proven that its use is safe in animals based on acute and sub-chronic toxicity tests. In management of lupus, methylprednisolone is used for primary immunosuppressant agent. This study aimed to investigate the effect of cecendet ethanolic extract in combination with methylprednisolone on inflammation, urinalysis, and serum creatinine level of rats as pristane-induced lupus animal models. This study was conducted at the Pharmacology Laboratory of Pharmacy Faculty of Jenderal Ahmad Yani University in January 2016. Four groups of female rats were used i.e. normal group, control group, 62.5 mg/kg body weight (BW) cecendet ethanolic extract group, and group receiving 62.5 mg/kg bw cecendet ethanolic extract combined with 0.36 mg/kg body weight methylprednisolone for six weeks, followed by thirty days of 0.5 mL pristine-induced. Examinations on anti-inflammation effect, urinalysis, and serum creatinine level  were performed in week 3 and 6 during the intervention. The results showed that the administration of cecendet ethanolic extract alone and in combination with methylprednisolone could inhibit inflammation and reduce proteinuria and serum creatinine level when compared to the control group.Key words: Anti-inflammation, cecendet, creatinine, methylprednisolone, Physalis angulata L. Pengaruh Pemberian Kombinasi Ekstrak Cecendet (Physalisangulata L.) dan Metilprednisolon terhadap Penurunan Radang dan Perbaikan Fungsi Ginjal Pada Model Tikus Lupus yang Diinduksi PristanaTanaman cecendet (Physalis angulata L.) sering digunakan sebagai obat tradisional di Indonesia, antara lain sebagai antiradang untuk mengobati reumatik. Penelitian terdahulu menunjukkan bahwa cecendet memiliki aktivitas imunosupresan pada hewan model lupus yang diinduksi pristana dan aman berdasar s ata uji toksisitas akut dan subkronis. Pada penanganan lupus, metilprednisolon digunakan sebagai obat utama imunosupresan. Penelitian ini bertujuan menguji khasiat ekstrak etanol herba cecendet dalam penggunaan tunggal dan kombinasi dengan metilprednisolon terhadap radang, urinalisis, dan kadar kreatinin serum pada tikus yang diinduksi pristana sebagai model hewan lupus. Penelitian dilaksanakan di Laboratorium Farmakologi Fakultas Farmasi Universitas Jenderal Ahmad Yani pada bulan Januari 2016.  Tikus betina sebagai hewan uji dibagi menjadi 4 grup, yaitu: grup normal, grupkontrol, grup diberi sediaan ekstrak etanol herba cecendet 62,5 mg/kg BB tunggal, dan grup hewan uji diberi sediaan ekstrak etanol herba cecendet 62,5 mg/kg BB kombinasi dengan metilprednisolon 0,36 mg/kg BB selama enam minggu, setelah tiga puluh hari diinduksi 0,5mL pristana. Tiga dan enam minggu setelah pemberian sediaan uji, dilakukan uji efek antiradang, pemeriksaan urin, dan pengukuran kadar kreatinin serum. Hasil penelitian menunjukkan bahwa pemberian ekstrak etanol herba cecendet tunggal maupun sebagai komplemen dengan metilprednisolon selama enam minggu memperlihatkan pengaruh penghambatan pembentukan radang, menurunkan proteinuria dan menurunkan kadar kreatinin serum jika dibanding dengan kelompok kontrol.Kata kunci: Anti-radang, Cecendet, kreatinin, metilprednisolon, Physalis angulata L.

Abstract

Abstract Background: One of the symptoms of metabolic syndrome is an increase in cholesterol levels in the blood. Ginger (Zingiber officinale) is a herbal plant that has several active compounds that can help improve lipid levels in the blood. Aim: This systematic study was conducted to examine the potential of ginger as an anti-hypercholesterolemia to prevent metabolic syndrome. Methods: The study was conducted on Indonesian-language articles through the Google Scholar Indonesia database. The inclusion criteria used were articles in Indonesian, preclinical and clinical experimental research, and complete articles that were accessible free of charge. Results: The results of the study show that the anti-cholesterol effect of ginger has been proven both preclinically and clinically. To reduce blood cholesterol levels, ginger can be used combined with other herbs. Some of the active ingredients in ginger are phenolic compounds, including gingerols and shogaols. The suspected mechanism of action is to affect the HMG-CoA reductase enzyme by inhibiting the oxidation pathway and as antiinflammation. Abstract

Latar belakang : daun sirih merah (Piper crocatum) mengandung senyawa kimia yang bermanfaat seperti alkaloid, saponin, tanin, dan flavonoid yang memiliki sifat antibakteri dan antiinflamasi. Penelitian terdahulu menunjukkan bahwa daun sirih merah memiliki efek bakterisid lebih baik dibandingkan dengan sirih hijau. Ekstrak daun sirih merah telah terbukti menghambat pertumbuhan bakteri penyebab penyakit periodontal pada penelitian sebelumnya. Setiap bahan atau zat kimia yang terkandung dalam obat maupun makanan harus menjalani uji toksisitas sebelum diperbolehkan penggunaanya secara luas. Tujuan : penelitian ini bertujuan untuk mengetahui efek toksisitas akut ekstrak daun sirih merah sebagai bahan terapi poket periodontal. Metode : pengujian toksisitas akut ekstrak daun sirih merah dilakukan terhadap hewan uji mencit galur Swiss webster yang dikelompokkan menjadi 6 kelompok yang terdiri dari 4 ekor jantan dan 4 ekor betina. Dosis yang diberikan pada hewan uji merupakan dosis tunggal secara peroral sebesar 2 kali dosis maksimal yaitu 10000, 5000, 2500, 1250, 625 mg/kgbb dan kelompok kontrol tanpa dosis. Penelitian ini dilakukan dengan menimbang berat badan selama 14 hari dan dibedah pada hari ke 15 untuk menghitung indeks organ relatif. Data dianalisis secara statistik dengan menggunakan uji One Way ANOVA dan dilanjutkan dengan uji Post-Hoc Tukey. Hasil : berdasarkan analisis menunjukkan bahwa ekstrak daun sirih merah (Piper crocatum) dengan dosis 10000, 5000, 2500, 1250, dan 625 mg/kgbb yang diberikan secara akut tidak menimbulkan perubahan berat badan dan indeks organ relatif yang signifikan pada hewan mencit galur Swiss webster. Kesimpulan: hasil penelitian menunjukkan bahwa ekstrak daun sirih merah (Piper crocatum) tidak bersifat toksik.

Abstract

Kulit merupakan salah satu bagian tubuh yang sering terpapar benda asing dan membutuhkan nutrisi dan pembersihan rutin menggunakan sabun. Susu sapi mengandung protein dan lemak yang berkhasiat untuk melembabkan dan menutrisi kulit. Penelitian ini bertujuan untuk membuat sediaan sabun cair dari susu sapi yang berfungsi untuk meningkatkan kelembaban pada kulit. Penelitian dimulai dengan pengumpulan susu sapi dari usaha kecil menengah di kota Cimahi dan uji karakteristik secara organoleptik. Selanjutnya dibuat serangkaian seri formula sabun cair yang mengandung susu sapi sebanyak 0, 5, 10 dan 20%, kemudian dievaluasi meliputi evaluasi fisik (organoleptis, pH, viskositas, dan uji stabilitas) dan pengujian terhadap responden (pengujian daya emolien dengan alat skin analyzer, uji kesukaan, dan efektivitas pembersihan). Data yang diperoleh dianalisa statistik menggunakan uji t-berpasangan. Hasil evaluasi fisik menunjukkan bahwa formula sabun cair yang mengandung susu sapi stabil selama penyimpanan 28 hari. Hasil pengujian terhadap responden menunjukkan sediaan yang paling disukai adalah sabun cair yang mengandung susu sapi 15% dengan alasan nyaman digunakan, lembut dan tidak lengket pada kulit. Hasil pengujian efektivitas pembersihan juga menunjukkan sabun cair yang mengandung susu sapi 15% dapat membersihkan kotoran dan make up.

Abstract

Psoriasis is an autoimmune disease that is characterized by the appearance of lesions on the skin. The current treatment aims to control the symptoms. The efficacy of Stachytarpheta jamaicensis (L.) Vahl’s for autoimmune rheumatoid arthritis and systemic lupus erythematosus has been tested in animal models. The aim of this research was to evaluate the effect of the water extract of S. jamaicensis leaves on psoriasis model animal (male Balb/c mice) induced topically by imiquimod. The water extract of S. jamaicensis leaves is made by boiling. The animal was divided into groups: normal, control, methotrexate 0.2 mg/kgBW, the extract (doses 25, 50 and 100 mg/kgBW). The measured parameters were the Psoriasis Area and Severity Index (PASI) score and skin histopathology. The results showed that all doses of the extract could reduce the PASI score when compared to the control group. Histological results showed that there was a decrease in keratin growth in test animals that were given the extract. Extracts at doses of 25 and 50 mg/kgBW can reduce the thickening of keratin in the epidermis of the back and ears. It can be concluded that the water extract of S. jamaicensis leaf has the most effective activity to prevent psoriasis recurrence in the dose range of 25 and 50 mg/kgBW.Keywords: Psoriasis, Stachytarpheta jamaicensis leaf water extract, PASI, keratin, imiquimod

Abstract

Rheumatoid arthritis (RA) adalah penyakit autoimun yang ditandai inflamasi kronik sistemik dan menyerang berbagai jaringan terutama sendi. Pertimbangan utama dalam pemilihan jenis obat RA adalah keberhasilan terapi dan efek samping. Penelitian ini bertujuan untuk mengevaluasi hubungan pemilihan jenis obat dan keberhasilan terapi RA pada pasien rawat jalan di salah satu rumah sakit di Bandung. Penelitian ini bersifat analitik cross-sectional, dimulai dengan mengumpulkan data secara retrospektif pada 30 pasien usia produktif yang melakukan terapi RA selama tiga bulan menggunakan metilprednisolon, metotreksat, kombinasi keduanya, atau dengan obat lain. Evaluasi keberhasilan terapi dilakukan dengan membandingkan keberhasilan perbaikan nilai Disease Activity Score 28 (DAS28) sebelum dan setelah terapi, dan monitoring efek samping dilihat dari kadar hemoglobin, laju endap darah, jumlah trombosit, dan leukosit. Hasil penelitian menunjukkan bahwa 90% pasien yang berobat memperoleh terapi metilprednisolon tunggal dan/atau kombinasi dengan metotreksat atau Disease-modifying Antirheumatic Drugs (DMARDs) lain. Pasien yang menerima metilprednisolon tunggal dan metotreksat tunggal mengalami penurunan nilai DAS28 sebanyak 26,8% dan 15,4% jika dibandingkan dengan kondisi awal (sebelum terapi). Pasien yang menggunakan kombinasi metotreksat, metilprednisolon, dan DMARDs lain mengalami peningkatan kadar hemoglobin tertinggi sebesar 3,51% dan penggunaan metotreksat tunggal dapat meningkatkan kadar hemoglobin sebesar 2,42%. Pasien yang menerima metotreksat tunggal mengalami penurunan nilai laju endap darah tertinggi sebesar 38,46%, penurunan trombosit tertinggi sebesar 27,16%, serta penurunan leukosit tertinggi sebesar 48,80%. Dapat disimpulkan bahwa meskipun sebagian besar pasien menerima terapi metilprednisolon tunggal dan/atau kombinasi dengan obat DMARDs lain, terapi menggunakan metotreksat tunggal masih merupakan pilihan utama untuk mencegah terjadinya remisi dan menurunkan risiko efek samping.Kata kunci: DAS28, metilprednisolon, metotreksat, rheumatoid arthritis Correlation between Drug Selection and Therapeutic Goals of Rheumatoid Arthritis PatientsAbstractRheumatoid arthritis (RA) is an autoimmune disease characterized by chronic systemic inflammation and attacks various tissues, especially the joints. The main consideration in choosing the type of RA drug is the success of therapy and side effects. This study aimed to evaluate the relationship between drug selection and the success of RA therapy in outpatients in a hospital in Bandung. The study was a cross-sectional analytic study, starting with collecting data retrospectively on thirty productive age patients who had three-month RA therapy using methylprednisolone, methotrexate, a combination of both or with other drugs. Evaluation of the success of therapy was done by comparing the success of the improvement of Disease Activity Score 28 (DAS28) values before and after therapy, and monitoring of side effects was seen from hemoglobin levels, blood sludge rate, platelet counts, and leukocytes. The results showed that 90% of patients seeking treatment received single and/or combination methylprednisolone therapy with methotrexate or other Disease-modifying Antirheumatic Drugs (DMARDs). Patients who received a single methylprednisolone and a single methotrexate experienced a decrease in DAS28 values of 26.8% and 15.4% compared to the beginning (before therapy). Patients who used a combination of methotrexate, methylprednisolone, and other DMARDs experienced the highest increase in hemoglobin level of 3.51% and the use of a single methotrexate could increase hemoglobin levels by 2.42%. Patients who received a single methotrexate experienced a decrease in the value of the highest blood sludge rate of 38.46%, the highest decrease in platelets of 27.16%, and the highest decrease in leukocytes of 48.80%. It can be concluded that although most patients received methylprednisolone therapy alone and/or in combination with other DMARDs, therapy using single methotrexate was still the main choice to prevent remission and reduce the risk of side effects.Keywords: DAS28, methylprednisolone, metothrexate, rheumatoid arthritis

Abstract

Rheumatoid arthritis is a chronic systemic autoimmune disease of the joints with unknown etiology. treatment therapy is carried out to reduce the symptoms of inflammation, one of which is by using methylprednisolone and methotrexate. treatment is carried out in the long term. this study aims to analyze the cost-effectiveness of combined methylprednisolone in rheumatoid arthritis patients. the study used a retrospective descriptive analytic design. the data were taken from 31 outpatient medical records in hasan sadikin hospital from january 2017-december 2018. the research data included the das28 score, doctor visit fees, medical expenses, and laboratory fees. cost-effectiveness analysis is done by calculating the average cost effectiveness ratio (acer) and incremental cost effectiveness ratio (icer). the results showed 17 patients using methylprednisolone+methotrexate, 6 patients using methylprednisolone+sulfasalazine, 1 patient using methylprednisolone+cyclosporine, 2 patients using methylprednisolone+methotrexate+chloroquine, 3 patients using methylprednisolone+chloroquine+cyclosporine and 2 patients using methylprednisolone+chloroquine. the lowest acer value was obtainedwhen the combination of methylpredisolone+chloroquine was rp. 453,716 per effectiveness with an icer value of rp. 11,686 per effectiveness. this study concludes that the combination of methylprednisolone + chloroquine is the most effective therapy compared to other combinations of methylprednisolone.
