Abstract

Background: Development of infection in burned patients is a crucial matter because it may increase morbidity and mortality. Factors contributing to high risk for infections are disrupted skin barrier, dysfunction of the immune system and prolonged hospitalization. Crowded hospital condition and transfer of patients from one unit to another can affect development of infection caused by environmental factors. The aim of the study was to identify the antimicrobial suspectibility of bacterial isolated from Burn Unit in Dr. Hasan Sadikin General Hospital, Bandung.Methods: A descriptive study was carried out in Burn Unit Dr. Hasan Sadikin General Hospital Bandung, Indonesia, from June to September 2013.  A total of 30 environment samples were collected and cultured to identify the bacterias. Futhermore, the colonies  found, underwent antimicrobial susceptibility test.Results: Most common bacteria found from the environmental samples in Burn Unit was Staphylococcus saprophyticus (24%). Other bacterias found were Klebsiella pneumonia (17%), Acinetobacter baumanii (15%), Staphylococcus epidermidis (12%), Bacillus sp.(12%), Pseudomonas sp.(7%), Staphylococcus aureus (5%), Enterobacter aerogenes (5%) and Serratia sp. (2%). Some bacteria still had good sensitivity to antibiotics while the Enterobacteriaceae were almost completely resistant to antimicrobial used in the study.Conclusions: The most common bacteria found from the environment samples in Burn Unit is Staphylococcuss aprophyticus. Additionally, Klebsiella pneumoniae as one of the Enterobacteriaceae groups, appears as an emerging hospital associated infection pathogen with their resistant to many antimicrobials. DOI: 10.15850/amj.v2n3.592

Abstract

Background : Ptosis has a reputation as a cosmetic problem, but it is more often as a functional deficit that can have real impact on patients’ lives. Ptosis results from the dysfunction of one or both of the upper eyelid retractors and often blocks peripheral vision. Ptosis can be congenital or caused by aging. Frontal suspension is one of few techniques for correction of ptosis especially for severe ptosis. Patient and Method : We reported one case of severe ptosis. The patient had bilateral congenital ptosis at both of upper eyelids. The upperlid excursion was 4 mm and marginal reflex distance (MRD) was 2 mm. A frontal suspension procedure using frontalis muscle was done for correction. Result : After frontal suspension procedure, MRDare 5 mm for both eyes. There wee no difficulty for closing the eyes. The cosmetic appearance was good. Patient was satisfied with the result. Conclusion : Frontal suspension procedure using frontalis muscle gives a good function and cosmetic result for severe ptosis.

Abstract

Penelitian dilakukan untuk mengetahui tingkat pengetahuan, sikap dan perilaku orang tua pasien peserta Bakti Sosial Operasi Bibir dan Langit-Langit Sumbing melalui kuesioner dan analisis deskriptif terhadap hasilnya. Responden merupakan orang tua pasien penderita bibir dan/atau langit-langit sumbing yang mengikuti operasi bakti sosial di Gunungkidul D.I. Yogyakarta pada 14 Januari 2017.Responden memiliki cukup pengetahuan perihal bibir dan langit-langit sumbing. Faktor lingkungan lebih berperan daripada faktor genetik di masyarakat ini. Seluruh responden baru mengetahui memiliki anak yang menderita sumbing setelah kelahiran.Walaupun keterbatasan dana merupakan kendala utama, sebagian besar responden tetap ingin anaknya tampak normal. Kesulitan utama adalah dalam proses pemberian makan. Peran interaksi langsung masyarakat dalam sosialisasi bakti sosial lebih bermanfaat dibandingkan media tulis atau media sosial. Hampir seluruh responden puas akan hasil operasi dan berharap acara serupa dapat rutin. Hasil penelitian ini diharapkan dapat menjadi data dasar dan umpan balik untuk persiapan dan kinerja tim bakti sosial selanjutnya.Research was done by questionnaire to assess the knowledge, attitude and behavior among parents of children with lip and/or cleft palate who attended plastic surgery charity event at Gunungkidul DI. Yogyakarta on January 14th, 2017; the results were analyzed descriptively. Respondents seemed quite knowledgeable about these birth defects. Environmental factors play a bigger role than genetic factors in this society. All cases were recognized only after birth. Although limited fund is the major restraint, the majority of respondents still wanted their children to look normal. The main problem is feeding difficulty. Social direct interaction is more effective than written or social media in the socialization of the event. Most respondents were satisfied with the results and hoped that this activity can be held regularly. The results of this study are expected to serve as basic data and feedback for the preparation and practice of other surgical charity team.

Abstract

Background: Snakebite is considered a global health issue, especially in the Southeast Asian region. However, data regarding snakebite cases in Indonesia are still very limited. This study aimed to explore the venomous snakebite cases and the serum anti bisa ular (SABU) antivenom effect in treating venomous snakebite caused by other than the snake species indicated in currently available SABU antivenom formulas.Methods: The analytical descriptive method with a cross-sectional study design was conducted in 2021 using a total sampling from the medical records of 63 patients diagnosed with snakebite at Dr. Hasan Sadikin General Hospital Bandung from 2015 to 2019. Characteristics of patients, clinical manifestation, and correlation between snakes species and outcomes after treatment with or without administration of SABU antivenom were collected.Results: Out of 63 patients, males (79%) were predominant with an average age of 39 years. Fifty-six patients arrived at the Emergency Room less than 24 hours after the occurrence (89%). Most cases were categorized as grade 2 (41.%) where the upper extremities were commonly bitten the body area (64%). Edema (83%) was the most common manifestation. There was no correlation between snake antivenom administered to a specific snake species and the stated outcome (p=0.053), meaning that SABU antivenom might be an effective alternative to treat more types of snakebites.Conclusions: Snakebites are most common in males, attack the upper extremities, categorized as grade 2 with edema. Administration of SABU antivenom provides the similar outcome compared to the group caused by the Javan spitting cobra, Banded krait, and Malayan pit viper.

Abstract

Burns causes a significant number of mortality and morbidity globally with a higher number found in developing countries, such as Indonesia. A study conducted in Indonesia discovered that the majority of burns were caused by flame LPG-related (liquefied petroleum gas) burns (33%), which is understandable because LPG is the main cooking fuel used in Indonesian homes. Since the first management of burn will influence the outcome of burn injuries, health promotion efforts to improve knowledge, attitude, and practice to prevent burns and to provide first aid for burn are considered important to reduce burn-related morbidity and mortality. This was a descriptive study conducted in Babakan Sari Sub-district, Antapani, Bandung, Indonesia which involved a one-day community health promotion event as the intervention. The aim of this study was to compare the level of knowledge regarding burn prevention and first management before and after a one-day community health promotion event. As many as 61 respondents were recruited through purposive sampling and these respondents were asked to fill out a pre-event questionnaire consisting of questions regarding demographic data and knowledge on burn prevention and management. Demographic data collected showed that 31% of participants had experienced burns; 100% of participants used 3kg LPG gas tank in their home; 98% of participants had never been taught on how to handle an emergency situation or how to arrange an evacuation route; and 100% of participants felt that their neighborhood did not provide facilities for burn management. A post-event questionnaire was distributed after the one-day health promotion event, which was conducted in October 2018, and the results were compared with the pre-event questionnaire results. A significant improvement of knowledge was seen, as evident from the increase of a median score from 2.0 before the event to 9.0 after the event (p<0.001). In conclusion, despite the fact that all participants of this study used 3kg LPG gas tank, they do not have proper knowledge regarding prevention or management of burns and that a one-day health promotion event can increase this knowledge significantly. Thus, an effective community health promotion program should be implemented at the community level in Indonesia, especially for areas where the majority of residents use 3 kg LPG gas tank, in order to reduce the incidence of burns due to 3 kg LPG tank explosion.Peningkatan Pengetahuan Melalui Penyuluhan Mengenai Pencegahan dan Penanganan Dini pada Luka BakarLuka bakar merupakan sebuah masalah yang menyebabkan morbiditas dan mortalitas tinggi di negara berkembang, termasuk Indonesia. Mayoritas luka bakar (33%) diakibatkan oleh api dari ledakan tabung gas LPG (liquefied petroleum gas) 3 kg dan mayoritas rumah tangga menggunakannya. Pelaksanaan penyuluhan mengenai pencegahan dan penanganan awal diharapkan dapat menurunkan angka morbiditas dan mortalitas akibat luka bakar. Penelitian deskriptif ini bertujuan membandingkan tingkat pengetahuan tentang pencegahan luka bakar dan manajemen pertama sebelum dan setelah acara promosi kesehatan Penelitian ini dilakukan di Kelurahan/Desa Babakan Sari, Antapani, Bandung pada bulan Oktober 2018 untuk mengedukasi dan meningkatkan pengetahuan responden. Dari total 61 responden (45 wanita dan 16 pria) dengan mayoritas responden berusia di atas 60 tahun (45.9%), didapatkan 32.8% responden pernah mengalami luka bakar, 100% responden menggunakan gas LPG 3 kg, 98% peserta tidak pernah mendapatkan penyuluhan mengenai penanganan luka bakar, dan 100% peserta merasa tidak ada fasilitas penanganan kebakaran di wilayah mereka. Pengetahuan responden dinilai sebelum dan sesudah dilakukan penyuluhan dan didapatkan peningkatan secara signifikan dan bermakna (p<0.001). Simpulan, meskipun semua responden yang menggunakan tangki gas LPG 3 kg tidak memiliki pengetahuan yang memadai pencegahan atau pengelolaan luka bakar, penyuluhan ini efektif dapat menurunkan angka kejadian luka bakar.

Abstract

Burns can increase metabolic responses of the sufferers, making adequate nutrition important for burn patients and needs to be considered in the management of burns. Counseling on the importance of nutrition for burn patients can reduce the risk of burn complications. This study was an intervention study designed to increase the community's knowledge and awareness of the importance of nutrition in burn patients. In this study, a lecture session and discussion session on nutrition in burn patients were held by the Division of Plastic Surgery, Department of Surgery, Dr. Hasan Sadikin General Hospital, Bandung, Indonesia at the Jatinangor Public Health Center, Sumedang, Indonesia, on August 14, 2019. Of all participants attending this event, thirty-one participants who participated in through the whole agenda of this event were sampled to be the respondents. Data were collected using a pre- and post-test questionnaires consisting of items on basic knowledge, first treatment, and nutrition for burn patients. More women (74%) participated in this study. Most respondents were over 40 years old (39%) and graduated from senior high school (39%). There was a significant increase in knowledge of the respondents before and after counseling the nutrition of burn patients (p<0.05). Thus, counseling is proven to increase knowledge in the general population that it should be performed routinely in various areas. Perlunya Penyuluhan Nutrisi Pada Pasien Luka Bakar Luka bakar dapat meningkatkan respons metabolik penderitanya. Nutrisi adekuat pada pasien luka bakar merupakan hal yang cukup penting dan perlu diperhatikan dalam penatalaksanaan luka bakar. Penyuluhan tentang pentingnya nutrisi untuk pasien luka bakar dapat mengurangi risiko komplikasi luka bakar. Penelitian ini adalah studi intervensi yang dirancang untuk meningkatkan pengetahuan dan kesadaran masyarakat tentang pentingnya nutrisi pada pasien luka bakar. Dalam penelitian ini, sesi kuliah dan sesi diskusi tentang nutrisi pada pasien luka bakar diadakan oleh Divisi Bedah Plastik, Departemen Bedah, Rumah Sakit Umum Dr. Hasan Sadikin, Bandung, Indonesia di Pusat Kesehatan Masyarakat Jatinangor, Sumedang, Indonesia, pada 14 Agustus 2019. Dari semua peserta yang menghadiri acara ini, tiga puluh satu peserta yang berpartisipasi melalui seluruh agenda acara ini dijadikan sampel untuk menjadi responden. Data dikumpulkan menggunakan kuesioner sebelum dan sesudah tes yang terdiri atas pengetahuan dasar, pengobatan pertama, dan nutrisi untuk pasien luka bakar. Lebih banyak perempuan (74%) yang berpartisipasi dalam penelitian ini. Sebagian besar responden berusia di atas 40 tahun (39%) dan lulus dari sekolah menengah atas (39%). Ada peningkatan yang signifikan dalam pengetahuan responden sebelum dan sesudah penyuluhan nutrisi pasien luka bakar (p<0,05). Simpulan, penyuluhan terbukti meningkatkan pengetahuan masyarakat bahwa itu harus dilakukan secara rutin di berbagai bidang.

Abstract

Background: Sepsis is an important cause of mortality in patients with burn, although many factors influence it. Early excision debridement as source control treatment has been done routinely in our center. It was intended to prevent sepsis and decrease mortality rate. Methods: We performed a retrospective, cross sectional study over 4 years (2012-2014) among patients with flame burns in Burn Unit Dr. Hasan Sadikin Hospital who underwent early excisional debridement. The criteria of patients were adult, with full thickness burn, without inhalation injury and co morbid disease. The mortality, length of stay, and incidence of sepsis were analyzed by simple regression linier statistics using SPSS 16.0 for Windows. Results: Mortality rate of all patients was 43.3%, and 42.3% was directly caused by sepsis. Thirty-nine patients matched the criteria, 20 patients had early excision (< 3 days) and 19 patients had late excision (> 3 days). In early excision group, 75% got sepsis and 55% died. Mortality and incidence of sepsis wasn’t significantly different in both groups (p=0.252 and p=0.855). Both groups have same length of stay (mean : 13.1 and p=0.236). Sex as confounding factor wasn’t significantly different (p=0.774). The extent of TBSA and age were significant factors causing mortality rate (p < 0.05). Conclusion: There are many factors that contribute to the success of treating burn patient. Excision debridement was proven by this study not a major factor and ineffective to decrease sepsis and mortality in burned patients.

Abstract

Introduction: The IRF6 rs2235373 (C/T) is a polymorphism in the intron region that has been associated with non-syndromic cleft lip and palate (NS CLP) among some populations. Polymorphism in introns can also affect the transcription that should be detected through some changes in IRF6 mRNA expression. This study was aimed to evaluate the effects of IRF6 rs2235373 polymorphism on mRNA expression changes in NS CLP with its phenotypes which include complete unilateral (CU) CLP, complete bilateral (CB) CLP, cleft lip only (CLO), and cleft palate only (CPO). Methods: This study was a laboratory analytical study, using samples from 198 individuals in Indonesia (136 individuals with NS CL/P consisted of CU CLP (n=42), CB CLP (n=34), CLO (n=33), CPO (n=27), and 62 individuals as controls). DNA was extracted from venous blood, and the segment of rs2235373 was evaluated by PCR and Sanger sequencing. mRNA expressions analysis preceded by RNA extraction from oral epithelium, then real-time (RT) PCR was performed. mRNA expression changes were analysed by Livak method and Kruskal Wallis test followed by the Mann Whitney test through CC, GT, and TT genotype. Result: The mRNA expression of TT genotype from NS CU CLP and control was 3.36 higher value than CC genotype (p=0.008), and the mRNA expression of CT genotype from NS CB CLP and control was 0.16 lower value than CC genotype (p=0.000). Conclusion: The IRF6 rs2235373 polymorphism of TT genotype tend to cause mRNA overexpression in NS CU CLP, and CT genotype tend to cause mRNA underexpression in NS CB CLP.Keywords: Non-syndromic cleft lip and palate, NS CLP phenotype, IRF6, rs2235373, mRNA expression.
