Indonesia merupakan negara yang mengalami kenaikan kejadian obesitas, hipertensi dan diabetes setiap tahunnya. Keadaan ini membuat meningkatnya prevalensi sindrom metabolik. Sindrom metabolik adalah kombinasi dari sejumlah kondisi, yaitu hipertensi, hiperglikemia, hiperkolesterolemia, hipertrigliserida dan peningkatan ukuran lingkar perut. Asupan energi dan pola makan yang berlebihan, peningkatan kejadian obesitas, aktivitas fisik sedentari dan riwayat risiko penyakit menjadi faktor resiko sejumlah kondisi penyebab terjadinya sindom metabolik. Kabupaten Pangandaran merupakan salah satu daerah pesisir pantai di Provinsi Jawa Barat yang secara demografis dimungkinkan tingginya konsumsi garam yang dapat memicu hipertensi. Pola makan dan aktivitas fisik yang tidak baik bagi kesehatan serta rendahnya tingkat pengetahuan dan kesadaran tentang bahaya sindrom metabolik memengaruhi jumlah kejadian sindrom metabolik. Pengabdian masyarakat ini dilakukan sebagai upaya preventif melalui pengisian kuisioner untuk mengetahui pola makan dan aktivitas fisik masyarakat kecamatan Cijulang Kabupaten Pangandaran serta pengukuran lingkar perut untuk mengetahui prevalensi resiko terjadinya sindrom metabolik. Dari 544 responden rata-rata berusia 30 tahun, 129 orang mempunyai lingkar perut yang tidak normal, 47% mengkonsumsi manis setiap hari 32% mengkonsumsi asin setiap hari dan 56% tidak olahraga setiap hari.

Background: Colorectal cancer is the second leading cause of mortality and the most prevalent cancer worldwide. Most patients, who come with late-stage, have ineffective treatments and some side effects in chemotherapy. Aaptos suberitoides has potential anti-cancer effects due to its bioactive compounds such as aptamine. This study aimed to evaluate the migration inhibition effect of Aaptos suberitoides fraction in HCT-116 cell line.Methods: This study was an experimental study. Aaptos suberitoides specimen was taken in Tinjil Island and fractionated with ethyl acetate. HCT-116 cell line was added with Aaptos suberitoides fraction and cellular migration activity was observed in 48 hours of which the scratch assay was performed. The gap closure area was determined with ImageJ software.Results: The data showed that a low concentration of Aaptos suberitoides fraction inhibited migration activity in HCT-116 cell line as follow; 1 and 5 mg/L Aaptos suberitoides fraction inhibit 3-4 % cancer cell migration in 24 hours, and 10-11% inhibition in 48 hours, respectively. However, 10 mg/L fraction concentration only inhibited 7-14% of the migration effect.Conclusion: Aaptos suberitoides fraction suggests insignificant migration inhibition in colorectal cancer cells and only inhibits less than 15 % HCT-116 cell line.

Background: Snakebite is considered a global health issue, especially in the Southeast Asian region. However, data regarding snakebite cases in Indonesia are still very limited. This study aimed to explore the venomous snakebite cases and the serum anti bisa ular (SABU) antivenom effect in treating venomous snakebite caused by other than the snake species indicated in currently available SABU antivenom formulas.Methods: The analytical descriptive method with a cross-sectional study design was conducted in 2021 using a total sampling from the medical records of 63 patients diagnosed with snakebite at Dr. Hasan Sadikin General Hospital Bandung from 2015 to 2019. Characteristics of patients, clinical manifestation, and correlation between snakes species and outcomes after treatment with or without administration of SABU antivenom were collected.Results: Out of 63 patients, males (79%) were predominant with an average age of 39 years. Fifty-six patients arrived at the Emergency Room less than 24 hours after the occurrence (89%). Most cases were categorized as grade 2 (41.%) where the upper extremities were commonly bitten the body area (64%). Edema (83%) was the most common manifestation. There was no correlation between snake antivenom administered to a specific snake species and the stated outcome (p=0.053), meaning that SABU antivenom might be an effective alternative to treat more types of snakebites.Conclusions: Snakebites are most common in males, attack the upper extremities, categorized as grade 2 with edema. Administration of SABU antivenom provides the similar outcome compared to the group caused by the Javan spitting cobra, Banded krait, and Malayan pit viper.

Objective: To evaluate the cytotoxic effects of ethanol extract from marine sponge Stylissa carteri in both parental and paclitaxel-resistant HeLa cervical cancer cells. Methods: This was an experimental in-vitro study subjected ethanol extract from Stylissa carteri obtained from Pramuka Island, Kepulauan Seribu National Park, Jakarta. Both parental and paclitaxel-resistant HeLa cells were treated with ethanol extract followed by microscopic observation and MTT assay. The IC50 and drug curves were analyzed using four-parametric logistic regression by SigmaPlot for Windows version 12.0 from Systat Software Inc., USA.Results: Ethanol extract from Stylissa carteri triggered cell death-associated morphological changes in parental Hela cells. It demonstrated cytotoxic activities with an IC50 value of 1 ppm. Importantly, this extract also triggered cell death in paclitaxel-resistant HeLa cells. The IC50 of ethanol extract of Stylissa carteri was 4 ppm in paclitaxel-resistant HeLa cells.Conclusions: There is a potential cytotoxic activity of Stylissa carteri that is not only confirmed in parental HeLa cells but also in paclitaxel-resistant HeLa cells.Keywords: Cervical cancer, paclitaxel resistance, HeLa cells, cytotoxicity assay, Stylissa carteri DOI: 10.15850/ijihs.v6n2.1378

Background: Medical error is any kind of unplanned or out of procedure medical action. Some of them are error in selecting and prescribing drug which might cause the disease to remain, worse, or death. Prescribing error can be prevented by giving better education about pharmacology. One popular learning methods is blended learning. Truwinist Interdigital Method is a learning media innovation with bleanded learning concept designed subjects in audiovisual form. Truwinist Interdigital Method was accessible at university’s learning management system. This study aimed to evaluate the improvement of student’s pharmacological learning outcome upon Truwinist Interdigital Method. The learning outcome was students knowledge and skill on prescribing asthmatic drug. Methods: A experimental research was conducted with pretest and posttest design. Subjects were 148 medical students of Universitas Padjadjaran who were given Truwinist Interdigital Method. The learning outcome was observed using pre-test and post-test score and tested using Wilcolson test. Results: Students’ knowledge score increased from average of 34.70±15.43 to 86.01±10.80. Students’ skill score increased from average of 25.31±12.76 to 76.93±10.66. The result showed students’ learning outcome increased significantly (p<0,001). Conclusion: Knowledge and prescribing skill Students’ asthmatic drug increase after using bleanded learning with Truwinist Interdigital Method.

Background: The field of education began to shift from conventional methods to the e-learning method. Blended-learning method is a method that combines e-learning and conventional methods. Truwinist Interdigital Method (MTI) is a learning innovation using the blended-learning method. The aim of this study was to determine student perceptions about blended-learning: Truwinist Interdigital Method.Methods: This study is a descriptive studies. The subjects of this study were students of the Medical Undergraduate Study Program of Padjadjaran University in 7th semester, add up to 156 students in total. Subjects were presented with material for 4 days by accessing MTI antibiotics and continued face to face for 1 hour on the fourth day. The research instrument in the form of a questionnaire consisted of 16 statements with 4 aspects of assessment, namely ‘students interest’, ‘accessibility’, ‘content’, and ‘discussion activities in MTI’. The calculation method used likert-scale with index interval analysis.Results: Based on the analysis of the data from questionnaire, all items for aspects of students interest in blended-learning and accessibility were categorized as 'strongly agree'. Index number for the aspect of content is 78.21%; 79.97%; 79.97%; and 82.85% were categorized as 'strongly agree' and 72.28% were categorized as 'agree'. The index number per item for the aspect of discussion activities is 79.65% and 90.54% which is categorized as 'strongly agree', while the index number of 69.55% is categorized as 'agree'.Conclusion: The majority of students have a positive response to learning using blended learning: MTI. Background: The impact of weekly formative assessments on the summative performance of undergraduate medical students is not widely studied. The formative assessment began to be applied in the Undergraduate Medical Study Program, Faculty of Medicine, Padjadjaran University (Program Studi Kedokteran Fakultas Kedokteran Universitas Padjadjaran or PSK FK Unpad) in early 2020. This study aims to determine whether there is a correlation between weekly formative and summative assessment in Multidisciplinary Examination (MDE) and Structured Oral Examination (SOE).Methods: This study was conducted using a numerical correlative bivariate analysis with a cross-sectional design. The data used in this study was weekly formative assessment scores, MDE and SOE scores of Reproductive System (RPS) I and II of 138 medical student batch 2017. Results: Rank-Spearman test shows there was no significant correlation between the formative test scores and the MDE RPS I score (p>0.05) and the SOE score (p>0.05). Meanwhile, the MDE RPS II score with the formative test was found to be a significant correlation (p<0.05) and positively correlated (r=+0.211).Conclusion: This study shows that the use of weekly formative assessments might contribute to the students' achievement of MDE scores. Weekly formative assessments can be integrated into the curriculum of pre-clinical medical programs to improve student summative performance.

EFFORTS TO IMPROVE EARLY DETECTION OF BREAST CANCER IN CIJULANG 1 VOCATIONAL HIGH SCHOOL, PANGANDARAN REGENCY. The high mortality rate of breast cancer (BC) patients in Indonesia is associated with conditions of most patients come to doctor in an advanced stage of BC. This is due to the low BC awareness of Indonesian women. This community service program (PPM) is intended to increase knowledge of adolescent women about early detection of BC. This action was conducted by BC awareness workshop to 114 female students of Vocational High School (SMK) 1 Cijulang, Pangandaran district, West Java. The profile of participants showed that 64% of participants are 17 years old and mostly from Cijulang and Cimerak sub-districts. In addition, the majority of participants were not aware of BC, indicated by lack of knowledge of BC signs and symptoms (97%), and low confident of doing breast self-examination (BSE) (30%). Moreover, there were 7% of the participants were smoking which is known to be one of the major risk factors for BC. These findings point out conducting a program for increasing BC awareness among students. The program was managed by mini-lecture that focuses on BC risk factors, BC early detection and BSE (SADARI) using videos and simulation on a mannequin. This program was expected to have an impact on their families, indirectly. Ultimately, this will increase the finding of new cases of BC which will increase their life expectancy.

Sappan wood (Caesalpinia sappan L.) is a plant used by the community to mix drinking. The antioxidant index of Sappan wood extract has a higher value than commercial antioxidants to counteract oxidative free radicals and improve the body's defense system. The use of Sappan wood drinking in society is often used in daily so it is necessary to do a chronic toxicity test to observe the response to prolonged use of the system in the body. Kupffer cells are an important part of the innate immune system, acting as “scavengers” and phagocytes. A study that aims to see the chronic toxicity studies of the ethanolic extract of Sappan wood on the number of Kuffer cells in the liver has been carried out on 60 Wistar rats divided into 12 groups. The group was the negative control group, doses 100 mg/kgBW, 200 mg/kgBW, 300 mg/kgBW, 400 mg/kgBW and 500 mg/kgBW of extract Sappan wood which were divided into male and female groups. Each group was given ethanol extract of Sappan wood for one year orally. The results showed a significant increase in Kupffer cells in the female group at a dose of 100 mg/kgBW and the male group at 200 mg/kgBW with significant values, respectively, p<0.001 p = 0.004.

Background: HER2+ breast cancer is a very aggressive type of breast cancer. Although trastuzumab, specifically targeted for HER2, has been used for breast cancer treatments, some patients become resistant to trastuzumab. A marine sponge is one of the potential sources of anticancer agents. One of marine sponges commonly found in Indonesia is Stylissa carteri, but it has not been explored extensively.Objective: This study aimed to identify cytotoxic effects of the ethyl acetate fraction of Stylissa carteri on HCC-1954, HER2+ trastuzumab-resistant breast cancer cells, by assessing cell viability and determining IC50 value.Methods: This study was an experimental in vitro study conducted in The Cell Culture Laboratory, Faculty of Medicine Universitas Padjadjaran on February 2018 to June 2018. The Stylissa carteri was collected from Pramuka Island, Kepulauan Seribu National Park Jakarta. HCC-1954 cells were treated by serial concentration of fractions and were incubated for 72 hours. Cell viability were observed under a microscope and analysed with MTT assay. The IC50 value was also determined by using four parametric logistic regression (4PL) method by Sigmaplot version 12.0. Result: Data of this study showed descent of cell viability significantly when exposing the ethyl acetate fraction of Stylissa carteri. There was a decrease of 49% cell viability in 10 µg/ml of the ethyl acetate fraction of Stylissa carteri. The estimated IC50 value was 9.25 µg/ml.Conclusion: This result indicated that the ethyl acetate fraction of Stylissa carteri has cytotoxic effects on HER2+ trastuzumab-resistant breast cancer cells.
