Abstract

Selama masa pandemi COVID-19 pendaftaran pasien rawat jalan di RSUD Soreang diberlakukan secara online sejak Agustus 2020. Pada bulan Januari 2021 ditemukan ketidaktepatan pendistribusian berkas rekam medis ke poliklinik sebanyak 95 rekam medis atau sebesar 2%. Terkadang rekam medis pasien tidak langsung dikirimkan ke poliklinik karena menunggu menumpuknya rekam medis, sehingga mengakibatkan pasien lama untuk dilayani. Tujuan penelitian ini untuk mengetahui pelaksanaan pendistribusian rekam medis rawat jalan guna meningkatkan efektivitas pelayanan pada masa pandemi COVID-19 di Instalasi Rekam Medis RSUD Soreang. Metode penelitian yang digunakan adalah metode desktriptif dengan pendekatan kualitatif. Teknik pengumpulan data yaitu dengan observasi, wawancara, dokumentasi dan studi kepustakaan/studi literatur. Instrumen penelitian menggunakan pedoman wawancara, alat tulis dan kalkulator. Hasil penelitian penulis, Ditemukan 92 rekam medis yang salah dalam pendistribusian dari jumlah rekam medis pada bulan februari 2021 sebanyak 4.730 berkas, terjadinya penumpukan rekam medis pasien sebelum dikirim ke poliklinik. Terdapat 35 rekam medis yang tidak di temukan pada rak penyimpanan rekam medis rawat jalan, hal ini mengakibatkan rekam medis tersebut lama untuk di kirim karena petugas membutuhkan waktu yang lama, sehingga berpengaruh terhadap kelancaran pelayanan kesehatan. Petugas harus menjalankan tugas sesuai prosedur yang ada, petugas harus lebih teliti dan fokus pada pekerjaannya, pengecekan kembali rekam medis saat di simpan, melakukan evaluasi dan rapat sebulan sekali mengenai pendistribusian. Abstract

A room or space ergonomic and equipped with adequate facilities and infrastructure will create a sense of security and comfort to improve the quality of medical services in hospitals Cibabat Cimahi. As for the problems that researchers found in a field that is of a room the ergonomic, yet the distance between the officers too close thus limiting the space motion of the officers, as well as the facilities and infrastructure are not yet adequate such as the lack of number of seats currently available with the number of as many as 30 seats and a patient waiting room area that is less extensive given the large number of patients coming in each day approximately 380 patients.The research method used is descriptive research methods with quantitative approach that is a bunch of objects aims to see the description of the phenomena that occur in a specific population dealing with numbers and analysis using statistics.From the results it can be concluded that the registration counters BPJS 1 outpatient in hospital Cibabat Cimahi in ergonomic yet with broad the children are 42.38 m2, it is clearly visible from a distance of between attendant adjacent to the average distance such as 21.2 cm, room temperature range 320C aircraft as well as the facilities and infrastructure are not yet sufficient to become supporters in the improved quality of service for the hospital.Suggestions from the author : petition for filing, smoothing files patients claim BPJS piled, sorting clerk, the addition of facilities and infrastructure, as well as the efficiency of the use of the room.

Abstract

Based on preliminary observations, in February 2020 the writer found the problems that were closely related to the outpatient services, especially at the Puskesmas Pasirlangu as follows; (1) Lack of ergonomics in each room there are still spaces that are still narrow resulting in a lack of effectiveness; (2) There are still leaks and humidity in the room, causing mold; (3) improper layout. The purpose of this study was to determine the layout of outpatient services based on the concept of ergonomics in order to support the effectiveness of work in the Pasirlangu Community Health Center. The method used is descriptive with a qualitative analysis approach. Data collection techniques used were observation and interviews. The research instrument used was stationery and interview guidelines. Spatial research results are still not in accordance with the concept of ergonomics because of each room there are shortcomings ranging from lighting, ventilation, room temperature, room humidity and layout that are not yet neat and the effectiveness of work there is not yet fully effective work effectiveness.

Abstract

Abstrak Penyimpanan rekam medis dikatakan baik apabila ruangan dapat menjamin sistem penyimpanan dan pemeliharaan, Apabila data disimpan dengan baik dan benar maka dapat meningkatkan pelayanan kesehatan di rumah sakit tersebut. Berdasarkan studi pendahuluan penulis menemukan permasalahan pada pendistribusian danpenyimpanan yaitu ditemukan 2 berkas belum dikembalikan lebih dari 1x24 jam, 3 berkas rekam medis yang salah simpan, berkas rusak dari 1 rak kecil. Penelitian ini bertujuan untuk mengetahui tentang tinjauan pelaksanaan sistem penyimpanan rekam medis rawat jalan guna menunjang mutu pelayanan di rumah sakit umum pindad. Metode Penelitian yang digunakan adalah Kuantitatif pendekatan Deskriptif. Teknik pengumpulan data pada penelitian antara lain: Observasi, wawancara, dokumentasi, dan studi kepustakaan. Salah satu tolak ukur dalam meningkatkan mutu pelayanan adalah ketepatan waktu. Berdasarkan penelitian di Rumah Sakit Umum Pindad Bandung sudah mempunyai Standar Operasional Prosedur, namun pelaksanaanya belum memenuhi dengan standar yang telah ditetapkan oleh rumah sakit dalam sistem penjajaran menggunakan sistem semi terminal tidak sesuai dengan standar prosedur sistem terminal digit. Terdapat 6 berkas belum dikembalikan, 7 berkas salah simpan, 213 berkas rusak dari 1 rak kecil. Juga hasil rekapitulasi penyediaan rekam medis rawat jalan kurang dari 10 menit sebanyak 85 berkas dan yang lebih dari 10 menit sebanyak 14 berkas dan di peroleh rata-rata satu orang dalam penyediaan berkas yaitu ≥10 menit 30 detik , belum sesuai dengan standar yang telah ditentukan. Saran penulis adalah, pengembalian rawat jalan 1x24 jam setelah dilakukan pelayanan pasien, dilakukan sosialisasi kembali standar prosedur operasional, mengganti berkas yang rusak. Abstract

Hasil studi pendahuluan terdapat laporan adanya petugas yang terpapar Covid-19 sebanyak 4 orang dengan keterangan 2 petugas di bagian pendaftaran, 1 petugas di bagian Assembling dan 1 petugas di bagian Filling pada bulan yang berbeda. Tujuan Penelitian ini adalah untuk mengetahui bagaimana pengelolaan berkas rekam medis guna mengurangi paparan infeksi melalui berkas medis pada masa pandemi Covid-19 di Rumah Sakit Tk. II Dustira Cimahi. Metode penelitian yang digunakan adalah metode deskriptif dengan pendekatan kualitatif. Teknik pengumpulan data yang digunakan adalah teknik wawancara, observasi dan studi kepustakaan.Hasil penelitian pada tanggal 12 – 28 Maret 2021, penulis menemukan beberapa SOP sudah resmi diperbarui setelah pandemi dan terdapat SOP yang masih belum diresmikan mengenai pengembalian berkas rekam medis pasien Covid-19, dan beberapa kendala seperti terbatasnya persediaan Alat Pelindung Diri, kurangnya kedisiplinan petugas dalam penggunaan Alat Pelindung Diri, kurangnya SDM dan terbatasnya ruang gerak bagi petugas dalam mengelola rekam medis. Saran dari penulis sebaiknya segera dikeluarkan SOP resmi mengenai pengembalian berkas rekam medis pasien Covid-19 dan disosialisasikan kepada seluruh petugas rekam medis, pengawasan penggunaan Alat Pelindung Diri, dan perlu ditingkatkan kembali ruangan Instalasi Rekam Medis Rumah Sakit Tk. II Dustira Cimahi untuk bisa berjaga jarak antar petugas. Kata kunci : Pengelolaan, Rekam Medis, Pandemi Covid-19.

Abstract

HIV/AIDS is a global health problem that is increasingly widespread today. The number of cases of Human Immunodeficiency Virus (HIV) in 2021 is 36,902 cases and the majority of sufferers are of productive age. Most HIV case sufferers come from the age range of 25-49 years with a percentage of 69.7% in 2021 then 20-24 years of age 16.9% and HIV sufferers over 50 years of 8.1%, ages 15-19 years by 3.1%. The number of cases (of AIDS) was 5750 cases and the majority of sufferers were in the age range of 30-39 years with the aim of knowing whether there was a relationship between pregnant women's knowledge about HIV/AIDS and the attitude of pregnant women towards voluntary counseling and testing of HIV/AIDS (KTS). ) at the Cipageran Health Center, Cimahi City. The research method used is descriptive-analytic with a cross-sectional approach. The population in this study were all pregnant women at the Cipageran Health Center in May as many as 76 pregnant women. The sample in this study used a total sampling technique. Measuring the knowledge and attitudes of pregnant women using a research instrument (questionnaire). The results showed that of the 76 pregnant women, the majority of pregnant women who had less knowledge about HIV/AIDS, 10 pregnant women (83.3%) had a negative attitude towards KTS. asked her to get tested for HIV/AIDS as early as possible to prevent transmission of HIV/AIDS from mother to child.

Abstract

The results of the Preliminary Study when searching for medical record documents there were missfile incidents of 10 medical record documents, with 4 medical record files in March, 3 medical record files in April, and 3 medical record files in May. This is because the storage room at Sulaiman Air Force Air Force Base is still a mess The research method used is descriptive method with a qualitative approach. The subjects in this study were medical record officers, the object of this study was the procedure for using medical record file tracers. Data collection techniques used are observation, interviews, and documentation. The results of the research conducted by the author, show that the data for taking medical record files in March were 150 medical record files, the occurrence of missfiles was 4 medical record files due to an error in the sequence of numbers and file storage errors, in April there were 120 medical record files, 3 medical record files due to errors in the order of numbers, reading errors, and mistaking numbers, while in May there were 100 medical record files with missifles as many as 3 medical record files due to sequence errors and file storage errors. The tracer used is 10.5 x 32 cm in size, yellow and orange. The constraints on using a tracer to prevent missfile incidents are the time factor, the accuracy factor, the expedition book and the storage rack. Efforts were made to overcome the problem of using a tracer in preventing missfile incidents, namely if the medical record file is not found, the storage officer will create a medical record file to replace the missing medical record file.

Abstract

The rapid digitalization of healthcare systems has accelerated the adoption of Electronic Medical Records (EMRs) globally, yet implementation challenges persist across diverse healthcare settings. This study evaluates EMR implementation at RS Bhayangkara TK II Sartika Asih Bandung using the HOT-FIT (Human, Organization, Technology, Benefit) model to assess whether the hospital meets the criteria for effective digital transformation. A descriptive qualitative research design was employed, utilizing observation and semi-structured interviews as primary data collection methods. The study population comprised 25 medical record information officers (PMIK) from outpatient registration, inpatient registration, emergency department, and casemix units, with key informants selected through purposive sampling. Data were analyzed using thematic analysis to identify patterns across the HOT-FIT framework components. Results revealed strong performance in Human and Organization aspects, characterized by high user adaptability, positive acceptance rates, effective "learning by doing" approaches, and robust managerial support through comprehensive training programs. However, the Technology component demonstrated significant vulnerabilities, including persistent system errors, severe lag during peak hours, unstable internet connectivity, and frequent power outages necessitating manual fallbacks. These technological challenges substantially compromised EMR benefit realization, creating operational inefficiencies and data integrity risks. The study concludes that while human and organizational readiness are exemplary, technological infrastructure improvements are critical for achieving optimal EMR implementation outcomes and unlocking the system's full potential for enhanced healthcare delivery.

Abstract

Incomplete medical documentation at hospitals poses a significant risk to patient safety and service quality. This study aimed to evaluate the completeness of inpatient medical records at Welas Asih General Hospital using a SWOT (Strengths, Weaknesses, Opportunities, Threats) analysis to formulate strategic recommendations. Employing a qualitative descriptive method, the research focused on inpatient medical records and key personnel, including medical record officers and clinical staff. Data were collected through observation, documentation review, and interviews, and analyzed by grouping findings into the four SWOT categories. Results revealed that the overall completeness rate was 64%, with critical forms such as surgical informed consent and Attending Physician forms showing particularly low rates (41% and 42%, respectively). The analysis identified key challenges, including a hybrid documentation system, low digital literacy, and high staff workload. In conclusion, the hospital's medical record completeness is suboptimal due to internal weaknesses and external threats, despite its existing strengths and opportunities. The findings underscore the urgent need for a systematic improvement plan focusing on structured digitalization and continuous staff training to enhance accountability and quality of care.
