Abstract

Traditional fishermen and divers are activities that are generally carried out by people in coastal areas. Kuta area, Mandalika is one of the coastal areas located on the island of Lombok. Diving activities and catching marine products are generally carried out in a hereditary manner and do not meet the Standard Operating Procedure (SOP) for safe and correct diving and are not followed by knowledge of the safety and health of diving so that it can cause problems in several organs of the body and the most often is the ear. In addition, health problems in coastal communities, especially in the field of ENT are also influenced by several factors including clean and healthy living behavior and environmental sanitation. Therefore, in this national social service activity, ENT health service activities and ear cleaning are carried out to obtain an overview of the health conditions of the people in coastal areas. The results of the ENT examination obtained in this activity are that in general the community has a good health status, although there are still several health problems that need attention, namely cerumen impaction and middle ear infection (CSOM), so it is necessary to carry out similar activities in coastal communities, especially on the island of Lombok to achieve better health status

Abstract

Latar Belakang: Kelainan refraksi merupakan gangguan pada mata yang menyebabkan fungsi mata menjadi menurun. Prevalensi kelainan refraksi di Indonesia mencapai 61,71% dan menjangkiti semua kalangan usia termasuk anak-anak dengan prevalensi kejadian sekitar 10%. Gambaran refraksi anak di Provinsi Nusa Tenggara Barat masih kurang banyak diteliti. Tujuan dari penelitian ini adalah untuk mengetahui karakteristik pasien berdasarkan demografi, jenis dan klasifikasi kelainan, lateralitas, tatalaksana, dan komplikasi kelainan refraksi pada anak di RSUD Provinsi NTB. Metode: Penelitian dilakukan di RSUD Provinsi NTB menggunakan data rekam medis periode tahun 2019. Metode yang digunakan adalah deskriptif dengan desain penelitian cross sectional. Hasil: Data yang memenuhi kriteria yaitu sebanyak 77 orang. Karakteristik demografi yang paling banyak ditemukan adalah kelompok usia 13-15 tahun sebanyak 31 orang (40,2%) dan jenis kelamin perempuan sebanyak 53 orang (67,5%). Astigmatisma sebagai kelainan refraksi terbanyak (58,5%) dengan jenis astigmatisma miopia kompositus sebanyak 43,7%. Berdasarkan karakteristik klinis mayoritas bilateral sebanyak 65 orang (84,4%), diberi tatalaksana kacamata sebanyak 65 orang (84,4%), dan mengalami ambliopia sebanyak 14 orang (87,5%). Kesimpulan: Karakteristik kelainan refraksi pada anak usia sekolah di RSUD Provinsi NTB memiliki karakteristik yang serupa dengan teori yang ada.

Abstract

Introduction : Silicone tube was the standard for stenting canaliculi in lacrimal canalicular laceration. But it was not always in stock at a hospital in remote area with low resource. Use of intravenous catheter to exchange the unavailable slicone tube was a possible option. Case Illustration : We reported two cases of inferior canalicular laceration at NTB provincial hospital. Both cases were adult, one female and one male. First case caused by nail and second because of traffic accident. There were no globe involvement in both case. Reconstruction surgeries with monocanalicular approach were performed. Intravenous catheter was used for stenting the canaliculus. First case was using a 24G, the other was 22G. The catheters were well inserted and planned to be removed at3 month. At the day 7 follow-up, the 24G catheter was extruded spontaneously while the patient with 22G was not. Both patients did not complain of profuse tearing. Discussion : The diameter and length of 24G iv catheter were 0.7mm and 19mm, while 22G were 0.9mm and 25mm. The diameters were similar with the silicone tube (0.6mm) and the lengths were long enough to pass the lower and common canaliculus which has a total 15mm long. The catheter in the first case was extruded possibly because in 24G there was only 4mm inside th lacrimal sac. A high pressure air from the nose would push the catheter backward and extruded. The 22G would have 10mm inside the sac. Conclusion : Intravenous catheter was an alternative option for inferior canalicular laceration stenting in low resource setting.
