Abstract

Background: Identifying organizational culture is crucial for an organization as it can boost overall productivity. Organizations in the economic, social, and educational sectors have extensively identified their cultures, but few hospitals in Indonesia have followed suit.Objective: To determine the culture that is currently felt and preferred by employees at AN-NISA Hospital Tangerang.Methods The research method used quantitative descriptive analysis by distributing questionnaires to 266 respondents who were permanent employees of AN-NISA Hospital Tangerang.Result: The results of the study show that the current organizational culture at AN-NISA Hospital Tangerang is Clan, with an average of 38.42, and the preferred culture at AN-NISA Hospital Tangerang is Clan and Market, with an average of 27.11 and 25.49.Conclusion: AN-NISA Hospital Tangerang will continue to consider the institution as a second home, family, and open to telling stories about themselves. However, AN-NISA Hospital Tangerang hopes to have leaders who can set an example as individuals who are serious, focused, aggressive, and results-oriented. Growth, innovation, and achievement of goals were aggressively developed to make AN-NISA Hospital Tangerang an excellent hospital among the others.

Abstract

Background: The COVID-19 pandemic has been over yet; one of the effects of this pandemic is plenty of hospitals in Indonesia have been applying for temporary health service closure. Objective: This study aimed to analyse the effect of health service closure during the COVID-19 pandemic. Design: Simple study literature is the method for this study. Data Sources: This study used two different search processes. The first search process came from online news platforms (detik.com, kompas.com, and okezone.com), which used a combination of the words “COVID-19”, “emergency room”, “hospital”, and “closed” during November 2020. The second search was from the database (CINAHL, ProQuest, ScienceDirect, PubMed, SpringerLink, and Wiley Online). Review Methods: The guidelines of the second search used Preferred Reporting Items for Systematic Review and Meta-Analysis. Both searching processes applied COVID-19 and hospital closed or emergency department closed as the keywords from November 2020-January 2021. Results: The first finding showed more than 300 news about health service closure, and the second showed 13 selected articles. There were 42 health service unit closures from 16 provinces in Indonesia. These closures had disadvantages effect for patients, nursing care and nursing education side. Conclusions: Total or partial health service closure could be happened due to increased patient capacity, delays in the treatment of the patient, many nurses getting infected by COVID-19, and nursing students cannot practice directly. Therefore, this issue needs innovative solutions to keep the nursing student placement going well by considering safety for the patient, nurses, nursing students, and trainer/ facilitator.

Abstract

Background: Clinical Leadership (CL), carried out by Clinical Nurse Specialists (CNS), plays a crucial role in improving the quality of care and patient safety through their clinical knowledge and experience. Evaluation of the CNS's role from a peer perspective is necessary to assess the effectiveness of their contribution to healthcare. This study aims to describe the role of the CNS as a CL based on the perspectives of the CNS, Professional Care Providers (PPA), and management at AN-NISA Hospital, Tangerang. Methods: This research uses a descriptive quantitative approach involving 177 respondents to find out the perceptions between CNS, PHCP, and management regarding CNS leadership practices. Results: The results of the input aspect of the study indicate that the CNS is ready to implement organizational strategies, address healthcare gaps (M=4.63), understand the necessary structure and workflow (M=4.43), and possess leadership competencies (M=4.06). However, the results of the process and output aspects of the study indicate a gap between the perspectives of the CNS, PHCP, and management. The CNS felt they had not yet engaged in providing support to their colleagues (M=3.55), while PHCP assessed that the CNS were sufficiently involved in providing support to their colleagues (M=4.19). Another gap can be seen in the output aspect, where PHCP felt satisfied with the presence of the CNS, while the CNS felt that their colleagues were not satisfied with their performance results. One factor that can cause tension is reduced trust in the CNS. Conclusion: The CNS performance process and results show that CNS has the ability to carry out its role as a CL. CNS can improve patient outcomes by focusing on their roles and responsibilities, PHCP increasing support and collaboration with CNS, and management providing support to CNS to increase confidence

Abstract

Pendahuluan: Fenomena pasien yang melakukan transfusi darah, tetapi tidak menunjukkan peningkatan Hemoglobin (Hb) yang signifikan memicu untuk dilakukan penelitian lebih lanjut. Penelitian ini bertujuan untuk mengetahui hubungan volume darah, turnaround time (TAT) transfusi darah, dan hasil crossmatch terhadap peningkatan dan status pencapaian target Hb pasien berdasarkan rumus kebutuhan darah. Metode: Penelitian ini menggunakan desain kohort retrospektif dengan mengambil data EMR meliputi kadar Hb pasien, TAT transfusi darah, hasil crossmatch, dan volume darah. Populasi penelitian ini adalah pasien pelayanan BDRS RS AN-NISA Tangerang pada periode Januari-Mei 2025. Sampel dalam penelitian ini berjumlah 435 pasien. Hasil: Volume darah memiliki hubungan yang signifikan dengan perubahan kadar Hb (r=0,505; r²=0,253; p-value<0,001) dan status pencapaian target Hb berdasarkan rumus kebutuhan darah (OR=1,001; 95% CI=1,000–1,002; p-value=0,030). Hasil crossmatch menunjukkan hubungan yang tidak signifikan terhadap perubahan kadar Hb (p-value=0,237) maupun status pencapaian target Hb (p-value=0,862). Turnaround time transfusi darah tidak menunjukkan hubungan yang signifikan terhadap perubahan Hb (p-value=0,507) maupun pencapaian target kadar Hb (p-value=0,709). Kesimpulan: Volume darah menjadi faktor utama yang terbukti berpengaruh signifikan secara statistik terhadap peningkatan status pencapaian target kadar Hb berdasarkan rumus kebutuhan darah, sedangkan TAT transfusi darah dan hasil crossmatch tidak menunjukkan hubungan yang signifikan.
