Abstract

Healthcare professionals’ adherence to hand hygiene protocols is paramount to prevent Healthcare-Associated Infections (HAIs) and ensure patient safety. This study evaluates compliance with the World Health Organization's (WHO) "Five-Moment Hand Hygiene" protocol among nurses in two Indonesian hospitals: Regional General Hospital X in Makassar City and Provincial General Hospital Y in South Sulawesi Province. A total of 60 nurses participated, with demographic analysis revealing significant differences between the hospitals. Utilizing a comprehensive observation tool provided by the WHO, data analysis indicated variations in hand hygiene practices, particularly after touching patient surroundings. Despite these differences, the data followed a normal distribution, enhancing the reliability of statistical analyses. Significant disparities in hand hygiene practices were found after touching patient surroundings (p-value = 0.008; α < 0.05), underscoring the urgent need for targeted interventions among nurses to improve their compliance and enhance patient safety. This study recommends comprehensive training, resource provision, regular audits, fostering a supportive organizational culture, and integrating robust hand hygiene modules into nursing education. The study also suggests further research to identify barriers and improvement strategies. Addressing these recommendations collectively will bolster patient safety and reduce healthcare-associated infection burdens in Indonesian healthcare settings. The study's identification of specific deficiencies in hand hygiene practices provides actionable insights for healthcare administrators, infection control practitioners, and frontline healthcare workers, emphasizing the urgency of sustained efforts to promote proper hand hygiene practices.

Abstract

Diabetes mellitus (DM) is a growing global health issue, with dietary management posing significant challenges for patients. The habit of consuming high-sugar and high-fat snacks, as observed at Bajeng Community Health Center, Gowa Regency, increases the risk of serious complications, including diabetic ulcers and cardiovascular diseases. This program aimed to develop and optimize Golden Rice Cookies, a healthy snack enriched with locally sourced Piper crocatum extract, as a nutritional alternative for DM patients. A participatory approach was employed, involving an initial analysis of partner conditions, product development through organoleptic and hedonic testing, and education on diabetes dietary management. Over three months, patients consumed Golden Rice Cookies, and outcomes were evaluated based on fasting blood glucose (FBG), body mass index (BMI), and dietary compliance levels. The results demonstrated that Golden Rice Cookies were well-received, with high satisfaction scores for taste, aroma, and texture (≥4 on a 5-point scale). Regular consumption significantly reduced FBG by an average of 29.9 mg/dL, BMI by 1.3 kg/m², and increased dietary compliance by 20.5%. The flavonoids and polyphenols in P. crocatum contributed to oxidative stress suppression, inflammation reduction, and modulation of key biomarkers such as MMP-9 and NGAL. This program successfully empowered DM patients to manage their condition through a sustainable and locally based nutritional innovation. This approach holds promise for replication in other regions with high diabetes prevalence to support more effective and sustainable diabetes management.

Abstract

Type 2 Diabetes Mellitus (T2DM) is a non-communicable disease with increasing prevalence, requiring effective management strategies. One of the main challenges in T2DM management is the low adherence of patients to a healthy diet, physical activity, and regular blood glucose monitoring. This community service program aims to improve glycemic control and the quality of life of T2DM patients through the implementation of Virtual Health Coaching (VHC) using the DIACOACH application at Bajeng Primary Health Center, Gowa Regency. The program involved 60 participants from the Prolanis group who received six weeks of health coaching using the Motivational Interviewing approach and the FIRA model (Focus, Identify, Reflect, Act). The DIACOACH application was used as an educational and self-monitoring tool for patients in managing their diabetes. This program included training on the application’s use, virtual coaching sessions by healthcare professionals, and an evaluation of the intervention’s effectiveness by assessing patients' health parameters before and after the intervention. The evaluation results showed an improvement in patients’ understanding of diabetes management and increased adherence to regular blood glucose monitoring. Additionally, the DIACOACH application received positive feedback regarding ease of use and benefits for participants. This community service initiative demonstrates that digital health technology in community-based healthcare services can be an innovative solution to support more effective and sustainable diabetes management.

Abstract

Background: Diabetes management relies significantly on patient knowledge, which affects personal care practices and overall well-being. However, many patients face challenges in managing their condition due to insufficient support and motivation. Objective: This study aimed to evaluate the effectiveness of a motivational interviewing-based health coaching program using the  Focus, Identify, Reflect, Act (FIRA) model in enhancing diabetes knowledge and managing blood glucose, BMI, and blood pressure.Methods: A quasi-experimental study was conducted involving 121 adult participants with type 2 diabetes, allocated into intervention (n = 61) and control (n = 60) groups. The intervention group received six bi-weekly health coaching sessions over three months, based on the FIRA model integrated with motivational interviewing techniques. Pre and post-intervention assessments were conducted to evaluate diabetes knowledge, fasting blood glucose, BMI, and blood pressure. Statistical analysis included paired t-tests for within-group comparisons and independent t-tests or Mann-Whitney U tests for between-group analyses, based on data distribution.Results: The intervention group exhibited a significant improvement in diabetes knowledge (mean change = +3.80; p < 0.0001), while the control group showed a decline (mean change = -2.97; p < 0.0001). A significant reduction in fasting blood glucose was also observed in the intervention group (mean change = -17.00 mg/dL; p = 0.015), in contrast to a non-significant increase in the control group (mean change = +3.00 mg/dL; p = 0.365). No significant changes were found in BMI and systolic blood pressure. Diastolic blood pressure increased significantly in the control group (p = 0.001), while the decrease in the intervention group was not statistically significant (p = 0.432); the between-group difference was also not significant (p = 0.226).Conclusion: Motivational interviewing-based health coaching effectively improves diabetes knowledge and blood glucose management. Further studies are recommended to explore long-term impacts and personalised strategies to optimise patient outcomes.

Abstract

ST-Elevation Myocardial Infarction (STEMI) merupakan salah satu bentuk serangan jantung yang ditandai dengan adanya perubahan elektrokardiogram (EKG) pada segmen ST pada. Ventricular Septal Rupture (VSR) merupakan kondisi medis yang serius yang terjadi akibat komplikasi dari infark miokard (serangan jantung) akut. VSR terjadi ketika terdapat robekan atau lubang pada septum interventrikular. Secara umum, prevalensi VSR saat ini diperkirakan terjadi pada sekitar 0,2% hingga 0,5% dari seluruh pasien dengan infark miokard akut. Pemberian intervensi keperawatan berdasarkan evidence based praktis pada kasus VSR adalah mobilisasi progresif level 1. Untuk mengevaluasi aplikasi intervensi keperawatan mobilisasi progresif level 1 pada kasus recent stemi extensive anterior dengan ventrikuler septal ruptur. Studi ini menggunakan laporan kasus (case study) dengan praktik berbasis bukti. Subjek yang digunakan yaitu pasien dengan yang dirawat di ruang perawatan intensif jantung, analisa data menggunakan analisis deskriptif. Hasil penerapan yang dilakukan selama 2 jam dalam rentang waktu 5 hari, didapatkan hemodinamik yang stabil dan peningkatan aktifitas fisik yang toleransi dengan kondisi pasien. Mobilisasi progresif level 1 efektif diterapkan pada kasus VSR dengan masalah penurunan curah jantung dan intoleransi aktifitas. Mobilisasi progresif level 1 merupakan intervensi keperawatan yang efektif untuk pasien dengan VSR, yang dapat mempromosikan stabilitas hemodinamik dan meningkatkan fungsi fisik.
