Abstract

Healthcare professionals’ adherence to hand hygiene protocols is paramount to prevent Healthcare-Associated Infections (HAIs) and ensure patient safety. This study evaluates compliance with the World Health Organization's (WHO) "Five-Moment Hand Hygiene" protocol among nurses in two Indonesian hospitals: Regional General Hospital X in Makassar City and Provincial General Hospital Y in South Sulawesi Province. A total of 60 nurses participated, with demographic analysis revealing significant differences between the hospitals. Utilizing a comprehensive observation tool provided by the WHO, data analysis indicated variations in hand hygiene practices, particularly after touching patient surroundings. Despite these differences, the data followed a normal distribution, enhancing the reliability of statistical analyses. Significant disparities in hand hygiene practices were found after touching patient surroundings (p-value = 0.008; α < 0.05), underscoring the urgent need for targeted interventions among nurses to improve their compliance and enhance patient safety. This study recommends comprehensive training, resource provision, regular audits, fostering a supportive organizational culture, and integrating robust hand hygiene modules into nursing education. The study also suggests further research to identify barriers and improvement strategies. Addressing these recommendations collectively will bolster patient safety and reduce healthcare-associated infection burdens in Indonesian healthcare settings. The study's identification of specific deficiencies in hand hygiene practices provides actionable insights for healthcare administrators, infection control practitioners, and frontline healthcare workers, emphasizing the urgency of sustained efforts to promote proper hand hygiene practices.

Abstract

This review aims to examine patient engagement in care activities to improve patient safety. The research method uses an integrative literature review. A systematic search was carried out in the databases PubMed, ProQuest, Science Direct, Medline, Cochrane Library and Winley Online Library. The keywords used in the search are: (Patient participation OR Patient involvement OR Patient engagement) AND (Health care OR Care) AND (Patient safety). Articles must come from international databases, be published within the last 5 years and focus on patient engagement, care and patient safety with full text. Articles will be excluded if they do not focus on objectives, are more than 5 years old and if they have been reviewed. As a result of research, nine articles identified in this review that the patient's role in care can improve patient safety and the nurse's role in encouraging patients to participate in care can contribute to improving patient safety. In conclusion, patient involvement in patient safety activities can improve the safety of patients in hospitals. Keywords: Patient Safety, Patient Involvement, Care

Abstract

This study aims to assess nurse and patient satisfaction with how handoffs are handled. The methodology used is literature review. The Cochrane, PubMed, Ebsco, and Garuda Libraries are included in the Cochrane, PubMed, Ebsco, and Garuda Libraries. Each article discusses how surrender techniques affect nurse and patient satisfaction. Patients and nurses can benefit from different types of handover between shifts, rooms, and units, handover between nurses and nursing students, and SBAR (situation, background, assessment, and recommendations) communication techniques. Handover implementation can have a positive impact on patients and nurses, increase the knowledge of nurses and patients, and reduce malpractice cases.Keywords: Handover, Nurse Satisfaction, Patient Satisfaction

Abstract

Perawatan non-farmakologis dapat diberikan perawat  salah satunya melalui teknik guided imagery. Teknik guided imagery telah banyak digunakan dalam perawatan pasien dalam mengurangi rasa sakit pasien kanker, namun masih sangat sedikit yang membahas untuk penanganan nyeri pada pasien breast cancer. Tujuan penelitian ini untuk membuktikan intervensi penggunaan guided imagery dalam menurunkan nyeri pada breast cancer terhadap kadar endorfin. Desain penelitian quasi eksperimen dengan menggunakan metode non probability sampling (sampel non random)  dengan Convinience sampling, jumlah sampel 16 orang pasien breast cancer, yang kemudian dibagi menjadi 2 kelompok yaitu kelompok intervensi guided imagery dan kelompok kontrol dengan intervensi manajemen nyeri lainnya. Pengukuran nyeri menggunakan kuesioner nyeri McGill dan VAS. Penilaian nyeri pre dan post dengan menggunakan kuesioner McGill-PRI, McGill-PPI dan VAS pada kelompok intervensi signifikan menurun dengan hasil uji statistik p= <0,05 (p=0,00), dan hasil uji laboratorium kadar endorfin serum meningkat dengan hasil uji statistik signifikan p= 0,00. Dibandingkan dengan kelompok kontrol penilaian nyeri menggunakan kuesioner McGill-PRI, McGill-PPI dan VAS menurun pada nilai mean difference namun pada hasil uji statistik nilai p= >0,05, dan pada uji laboratorium kadar endorfin serum menurun dengan hasil uji statistik p= 0,38. Teknik guided imagery efektif menurunkan skor nyeri pada pasien breast cancer, dan terbukti secara empiris efektif meningkatkan kadar endorfin serum. Hasil penelitian ini dapat memperkuat fakta ilmiah bahwa intervensi manajemen nyeri nonfarmakologis guided imagery atau imaginasi terbimbing dapat membantu menurunkan nyeri pada pasien breast cancer.

Abstract

Introduction: Outside of nursing actions that are carried out in accordance with the professionalism of the nursing profession, there are actions that are carried out outside the realm of the nursing profession known as non-nursing task. Non-nursing measures have been shown to increase the risk of missed or delayed nursing care. The purpose of this study was to describe non-nursing task performed by nurses in the Baubau City General Hospital. Method: This research is a quantitative research using descriptive analytical survey analysis method which is used to describe the phenomena or events that are analyzed according to the data in one measurement without giving special treatment to the phenomenon to 85 respondents. The sampling method in this study used a purposive sampling technique. Result: Cleaning the patient's room and equipment is the most frequent non-nursing task by respondents with 33 respondents (38.8%) answering often and routine blood sampling for tests as many as 32 (37.6%) respondents stating often, while the act of providing care outside the scope of nursing, there were 5 respondents answered "always". Conclusion: It was found that there are several non-nursing tasks that are still often carried out by nurses at the Baubau city general hospital, and the act of providing care outside the scope of nursing is still always carried out by nurses.Therefore, it is hoped that the awareness of nurses to limit themselves to doing actions in accordance with the main tasks and duties of nurses.

Abstract

Introduction: One of the challenges in caring for neurology patients is the risk of increased intracranial pressure. Endotracheal suction is a routine nursing action but this procedure is reported can affect hemodynamic parameters. Artificial Airway Suction endotracheal recommends open and closed endotracheal suction safety and effective but several studies recommend the use of closed endotracheal suction so that study of the effect of endotracheal suction is needed. Objective: To determine the effect of endotracheal suction on hemodynamics of neurology patients. Method: The author was using a flow chart prism from 3 databases, namely Pubmed, Research Gate and Europe PMC with quantitative articles and Randomized Controlled Trial studies. Results: There were 11 articles found by the author, 8 articles compared the effect of endotracheal suction on hemodynamics, showing a significant effect between the use of open endotracheal suction and closed endotracheal suction on several hemodynamic assessment variables, which results were found more stable hemodynamic in the use of closed endotracheal suction, 3 articles looked at the effect of open endotracheal suction on oxygen saturation and all concluded that there was an effect of open endotracheal suction. 4 articles recommended the use of closed endotracheal suction in critical care areas because there were fewer physiological disorders. Conclusion: The use of closed endotracheal suction is recommended for neuro patients because of the more stable hemodynamic effects. The author suggests further studies on the effect of endotracheal suction of neurology patients spesifik for increased intracranial pressure.

Abstract

Spiritual care is care given to meet the spiritual needs of patients, but many patients state that they do not receive spiritual support from the medical team. This is because nurses' perceptions of spirituality and spiritual care are lacking, competence is not adequate and there are obstacles in meeting the spiritual needs of patients. So this review aims to identify the perceptions, competencies and barriers of nurses in fulfilling spiritual care. Method by Literature review research design. Search articles on PubMed, ClinicalKey for Nursing, and ProQues by keyword based on PICO. Found 1,839 articles in the period 2015-2021 (May), then screening was carried out and found 20 relevant articles to be discussed and analyzed. Results The majority of nurses have a moderate-good perception of spiritual care, nurses' competence is at a moderate-good level. Barriers found, lack of knowledge, inadequate skills, heavy workload, time constraints, lack of intimacy in providing spiritual care, cultural differences and lack of hospital management support. Conclusions nurses have positive perceptions, moderate-good competition and obstacles in fulfilling spiritual care. Spiritual care education or training is needed in order to increase self-awareness about spiritual nurses in fulfilling spiritual care for patients.

Abstract

 Clinical pathways involve the health team and part of the patient's records. Hence, team dynamics become a principal factor that needs to be appraised during implementation. Clinical pathways were considered as an additional workload for medical personnel. This research aims to review articles that discuss the perceptions and barriers among medical personnel in implementing clinical pathways. The articles were searched through the PubMed, Willey, ProQuest, and Google Scholar databases, which were published no less than ten years period from 2010 to 2020. The keywords used perception OR response AND barriers OR obstacles AND clinical pathway OR care. The pathway corresponds to the title and purpose of the review and is in full-text form. In total, eight articles meet the review criteria and objectives. The results showed that clinical pathways are perceived as a process that is directed at realizing benefits, creating habits, and requiring enthusiasm, support, and time. Obstacles for medical personnel that often arise in implementing clinical pathways include not having enough time, lack of direction and guidance, lack of staff awareness, lack of compliance in documentation, and lack of staff commitment. Conducting a direct study on evaluating the implementation of clinical pathways in hospitals is recommended.

Abstract

Public health insurance programs have become a global concern, especially in program planning such as resource use, knowing the effectiveness of pay-for-performance implementation in health insurance in Asian countries. Method Research design is a library study. Search for articles using the Pubmed, Ebsco, ScienceDirect, and Proquest databases. The research questions are structured using the PICO method, a database-based keyword search in MESH terms. Inclusion criteria are focused on the application of Pay-for-performance, on health insurance, applied in Asian countries, issued the last 10 years, in English. Found 748 articles, and only 9 articles were included according to research questions. Result: There are 9 articles included in this study. The results of a critical review of published articles, the application of pay-for-performance on health insurance in Asian countries improve the achievement of indicators, incentives and payment fines, and improve the quality of health services. Conclusion The application of Pay-For-Performance to health insurance in Asian countries is based on various indicators, payment systems, and improved quality of health services. Therefore, the application of Pay-For-Performance can be recommended for healthcare facilities in Asian countries.

Abstract

Globally, lung disease is one of the contributors to the increase in inpatients and the use of health facilities in hospitals. This literature review aims to identify the effectiveness of implementing integrated clinical pathways in lung disease. Methods: The design of this research is a literature review. Search articles using Pubmed, Ebsco, Cochrane Library, and Proquest databases. Structured research questions using the PICO method, and search keywords based on the database in MeSH Term. The inclusion criteria are focused on the application of integrated clinical pathways, applied to health care facilities, applied to lung disease, published in the last 10 years, and in English. There were 3582 articles found, and only 8 articles were included which matched the research question. Results: There were 8 articles included in this study. From the results of a critical assessment of the included articles, the application of ICP in lung disease can reduce patient length of stay, reduce patient care costs, reduce the number of patient readmissions, and reduce drug use in hospitals. Conclusion: The application of ICP in lung disease improves patient outcomes, therefore ICP can be recommended to health care agencies, especially health management.
