Abstract

Chronic Venous Leg Ulcers (CVLUs) are vascular diseases and require complex wound care from a patient, wound, health personnel and dressing factors. Cadexomer Iodine (CI) and Polyhexamethylene Biguanide (PHMB) are antimicrobial and broad-spectrum wound washing fluids. The uncontrolled infection will complicate wound healing due to bacteria or plankton from the biofilm stage to infection, which causes a long healing time. Purpose: to determine the effectiveness of the combination of Polyhexamethylene biguanide and Cadexomer Iodine on venous leg ulcer wound healing. Methods: This study is a case study using CVLUs as a sample, with pre-test and post-test designs, patients with CVLUs received topical antimicrobial CI and PHMB interventions on wounds for 7 weeks. The sample in this study used one sample with a pre-post treatment design for seven days.. The instrument uses the Bates Jensen Wound Assessment Tool. Results: The combination of CI and PHMB was shown to improve the healing of CVLUs by controlling bacterial infection in wounds. In the 2nd week, the wound was 26cm x 15cm, the slough was reduced, and the red base wound increased. In the 3rd week, the wound was stage 4, and the granulation increase was 75%. In the 7th treatment, the wound size scale increased 7cmx4cm, the wound became stage 2, and a significant increase in epithelialization was 75%. Conclusion: The combination of PHMB and CI has been shown to control infection at the biofilm stage to critical colonization, reduce odour and significantly control infection, and increase granulation and epithelialization in CVLUs

Abstract

Abstract

Background: Negative Pressure Wound Therapy (NPWT) is considered an effective treatment in facilitating the healing of chronic wounds. However, its effect remains inconsistent, which allows for further investigation. Objective: This study aimed to assess the effectiveness of the NPWT program in improving the management of chronic wound healing. Design: Systematic review and meta-analysis was used. Data Sources: The search strategy ranged from 2016 to 2021 in PubMed, CINAHL, ProQuest, and ScienceDirect. Review Methods: Risk of bias was done based on the Risk of Bias 2.0 guideline using RevMan 5.4.1, and meta-analysis was done using Jeffreys’s Amazing Statistics Program (JASP) software version 0.16.3. Critical appraisal of the included articles was done according to Joanna Briggs Institute’s (JBI) appraisal checklist. Results: A total of 15 articles were included, with 3,599 patients with chronic wounds. There was no publication bias in this study seen from the results of the Egger’s test value of 0.447 (p >0.05), symmetrical funnel plot, and fail-safe N of 137. However, heterogeneity among studies was present, with I2 value of 66.7%, Q = 41.663 (p <0.001); thus, Random Effect (RE) model was used. The RE model showed a significant positive effect of the NPWT on chronic wound healing, with z = 3.014, p = 0.003, 95% CI 0.085 to 0.400. The observed effects include decreased rate of surgical site infection, controlled inflammation, edema, and exudate, as well as increased tissue with varying forest plot size, as demonstrated by the small effect size (ES = 0.24, 95% CI -0.26 to 0.79, p <0.05). Conclusion: The analysis results show that the standard low pressure of 80-125 mmHg could improve microcirculation and accelerate the healing process of chronic wounds. Therefore, applying the NPWT program could be an alternative to nursing interventions. However, it should be carried out by competent wound nurses who carry out procedure steps, implement general patient care, and give tips on overcoming device problems and evaluation. Abstract

Background: The number of surgical procedures has increased significantly from year to year. One of the complications that often occurs after surgery is postoperative pain. Postoperative pain will begin to be felt after the effects of anesthesia wear off. The management of this pain is divided into two categories: pharmacological drug management and non-pharmacological relaxation management. The use of analgesics as primary therapy has limitations and risks of side effects, so it needs to be combined with non-pharmacological relaxation methods, such as the Benson relaxation technique. The Benson relaxation technique is a simple method that combines breathing techniques with spiritual aspects, aiming to reduce the activity of the sympathetic nervous system that triggers pain and anxiety. To address these issues, a community service activity on Benson relaxation education was conducted to prevent patients from becoming dependent on antibiotics. Objective. This Community Service (PkM) aims to increase knowledge about Benson relaxation and reduce the pain scale of post-operative patients using Benson relaxation techniques. Methods. This Community Service includes audio-visual media-based education and the implementation of Benson relaxation on 30 adult post-operative patients in the Ar-rahman RSI Purwokerto inpatient ward. Evaluation was conducted through pain scale measurements using the Numeric Rating Scale (NRS), as well as assessments of patients' knowledge before and after education using pre-test and post-test questionnaires. Results. This activity demonstrated a significant improvement in patients' understanding of Benson relaxation techniques, with 86.7% of participants initially having insufficient knowledge, but after education, 93.3% improved to the “good knowledge” category. Additionally, the pain scale decreased by an average of 1.9 points, with the majority of patients experiencing severe pain (43.3%) before the intervention, which improved to moderate pain (60%) after implementation. Thus, education and training in Benson relaxation techniques were proven effective as a non-pharmacological intervention in reducing post-operative pain.

Abstract

Background: Diabetic foot ulcers have risen with DM. The IDF reports 15-25% of diabetics get foot ulcers, rising to 25% over time. Due to biofilm and bacteria growth, diabetic ulcers need advanced wound therapy using antimicrobial dressings. Modern antibacterial treatments include sorbact hydrophobic and 0.9% cadexomer iodine. Purpose: Analyse the effectiveness of cutimed sorbact hydrophobic with cadexomer iodine 0.9% on diabetic ulcer infection. Methods: A single-blind, fold-over, randomised controlled study. Patients with diabetes who developed foot ulcers between September 1 and December 1, 2023. One hundred sixty-two participants were randomly assigned to receive cutimed sorbact hydrophobic or cadexomer iodine 0.9%.  After 120 days, both groups. Results: It was found that the two groups had similar body mass index (0.364), wound size (0.317), and baseline age (p=0.432). The mean difference in confidence value of -0.16 (OR -2.54 to 1.29; p=0.058) showed that diabetic foot ulcers in both groups had similar wound size on the thirtyth day. On day 60 of treatment, Sorbact Hydropobic showed a difference of 22.56±9.87 compared to Cadexomer Iodine 0.9%, with an MD-CI value of -6.75 (OR 5.19 to -0.34; p=0.039 At 90 days of treatment, cadexomer iodine showed a significant difference from sorbact hydropobic (9.73±2.14), with an MD-CI value of -12.29 (OR -9.19 to -4.26; p=0.016; R2=-0.417). Conclusion: Sorbact Hydropobic reduces wound size, infection, and bacterial resistance, speeding chronic wound healing. On chronic wounds, hydropobic sorbact can be used as a primary dressing.

Abstract

Background: Diabetes-related foot ulcers (DFUs) remain a major burden in low-resource services, with wide variation in healing and limb outcomes. Evidence using competing-risks or multistate methods to accurately estimate amputation-free survival (AFS) and time-to-healing, and to identify modifiable risks within routine care remains limited. Purposes: This study aimed to estimate AFS and time-to-healing and to test associations of peripheral arterial disease (PAD), kidney function, infection, glycemia, and off-loading with these endpoints among adults with DFUs. Methods: In a multicenter prospective cohort in Indonesia (October 1, 2022-September 30, 2023), we enrolled 620 adults with active DFUs meeting predefined criteria; exclusions included prior major amputation, non-diabetic ulcers, malignancy, incomplete dates, or day-0 loss to follow-up. Outcomes were AFS (first amputation or death) and time-to-healing (complete epithelialization), each under competing risks (amputation/death or healing). We estimated cumulative incidence with Aalen-Johansen and modeled transitions using cause-specific Cox and Fine-Gray sub-distribution hazards, adjusting for prespecified covariates; multiple imputation addressed missingness, proportional hazards diagnostics were performed, and subgroup/sensitivity analyses were prespecified. Results: Among 620 participants (PAD 41.9%, neuropathy 47.1%, infection 35.6%), the 12-month cumulative incidence was: healed 71%, minor amputation 14%, major amputation 4%, and death 8%. PAD, reduced eGFR (<60), infection, and higher HbA1c worsened AFS (sHR 1.78, 1.55, 1.42, and 1.09 per 1% respectively), while higher HbA1c and infection slowed healing (sHR 0.90 and 0.71); PAD reduced healing (0.79), and off-loading improved healing (1.22). Findings were consistent across sensitivity and subgroup analyses, indicating robustness. Conclusion: Most patients healed within 12 months, yet one in four experienced amputation or death. Results support routine vascular triage or revascularization, infection bundles, glycemic or renal optimization, and mandated off-loading within DFU pathways, and motivate further prospective evaluations of durability, safety, and cost-effectiveness.
