Abstract

Patient Safety Incident Reporting for improvement of the service system and prevention of recurring patient safety incidents is a very important part and affects the quality of service, so it is necessary to evaluate the implementation of the ICP reporting system at the hospital. Objective: To ascertain the elements that impact hospital patient safety event reporting.  Method: This article was written utilizing a literature review methodology. Data was gathered from database sources, and 20 articles between 2018 and 2023 were examined for PICOT analysis using the keywords patient safety incident, reporting, and health service. Results: There are three elements that affect the reporting of patient safety incidents that take place in hospitals: government, organizational, and human factors. It can be observed from features of nurse skills and patient safety culture in addition to environmental influences (individuals, organizations, and government). Cultural barriers (avoidance of conflict and fear of being blamed and penalized) and practical barriers (lack of expertise, so officers do not know how to report occurrences, lack of feedback, and lack of socialization connected to reporting incidents). Discussion: As a result of the existing literature review, no one has discussed the existence of organizational/hospital and government targets in efforts to achieve the target percentage (%) of PSI reporting. In order to improve employee motivation and involvement in reporting patient safety incidents and, ultimately, raise the standard of care provided to hospital patients, more research is required regarding the implementation of patient safety incident reporting in hospitals. Conclusion: The reporting of patient safety incidents in hospitals is influenced by three factors: personal, organizational, and governmental. Lack of feedback has been recognized as one of the weaknesses in incident reporting.

Quantitative descriptive research design, with a total sampling of 73 pregnant women patient respondents at the East Bekasi Private Hospital in 2018-2019 who experienced pre-eclampsia. It was found that the results of the classification of respondents were mostly mild pre-eclampsia. The analysis used chi square with the conclusion that there is a relationship between the age of pregnant women and the classification of pre-eclampsia (P value 0.000), parity (P value 0.023), history of hypertension (P value 0.000). There is no relationship between education (P value 0.667) and work (P value 0.624). This research is expected to be a reference in order to improve service to patients, especially in conducting assessments on pregnant women. Where maternal age, parity and history of hypertension have a relationship with the classification of preeclmasia in pregnant women.

Abstract

Background: The transition by childhood to adulthood is called adolescence, typically beginning at the age of 12–24 years. Female adolescence is marked by several changes, particularly in the development of the hormone’s estrogen and progesterone. One of the characteristics of women entering adolescence is menstruation. in the process of menstruation, women may endure unpleasant symptoms, such as dysmenorrhea or menstrual pain caused by excessive uterine contractions that can interfere by daily operations. Purpose: This study aimed to explore how effective abdominal stretching exercises are in decreasing the degree of menstrual pain endured by adolescent girls in East Jakarta. Methods: This study conducted a pre-experimental method utilizing a one-cohort pretest–posttest design approach. Outcomes: The outcomes implied that before doing the abdominal stretching exercises, most participants endured moderate menstrual pain (scale 2–6), by 33 respondents (55.0%) reporting this stage. After the exercise, the pain degree decreased to a mild stage (scale 1–4) in 50 respondents (83.3%). A total of 60 adolescents took a part in the study. Utilizing the Wilcoxon Signed Ranks Test, the analysis yielded a p-value of 0.000 that is lower than the significance stage of 0.05. This implies that abdominal stretching exercises were effective in decreasing dysmenorrhea degree among adolescent girls in East Jakarta. Conclusion: Abdominal stretching can boost blood flow, ease muscle tension in the abdominal area, and promote relaxation. These effects help lessen the degree of menstrual pain endured by individuals by dysmenorrhea.
