Abstract

If hypertension sufferers do not regularly check their blood pressure, they will not realize that they have high blood pressure because generally hypertension sufferers do not experience any signs and symptoms. The purpose of this study is to provide family nursing care to Mrs. N by administering bay leaf decoction therapy to reduce pain levels due to hypertension in Kualu Nenas Village, the working area of ​​the Tambang Community Health Center (UPT). This study uses a descriptive research method in the form of a case study with a nursing care approach. This study was conducted on June 22-28, 2025 with 1 respondent in one family conducted at the client's home. From the case analysis on the client, it was found that there was a decrease in pain levels and blood pressure after being given bay leaf decoction therapy, on the first day the pain level was 6 then decreased to 2 on the seventh day while the results of the blood pressure examination on the first day from 211/129 mmHg decreased to 144/93 mmHg on the seventh day. It is hoped that clients and families will always pay attention to their health, check their blood pressure regularly and carry out non-pharmacological therapy, one of which is bay leaf decoction therapy.

Abstract

Skin integrity disorders are conditionsin which individuals are at risk of damage to the epidermis and dermislayers of the skin. These skin integrity disordersinclude rough, dry, itchy skin with a blackish color andsmall crackson the skin. If left untreated, this will cause the skin tobecome inflamed or even infected.An. N's complaints include dry, rough, itchy, scaly skin with a darkened appearance sincehe began undergoing regular blood transfusions due to his thalassemia condition.This study used direct observation methods to assess,analyze data, and diagnose patients with dry skin with one respondent, An.. It was conducted in the Thalassemia room of Arifin Achmad Regional General Hospital in Riau Province. The purpose of this scientific workis to obtain an overview ofskin integrity with the application of virgin coconut oil. The results of this nursing care showed a decrease in skin integrity disorders from a scale of 3to a scale of 0 after 3 days of treatment. This indicates that the application of virgincoconut oil can reduce skin integrity disorders in children. It is hoped that patients and their families willapply Virgin Coconut Oil to dry skin to obtain better results so that the skin returns to normal.

Abstract

Demam menjadi salah satu tanda dan gejala yang sering muncul pada anak-anak, dikatakan demam apabila terjadi peningkatan suhu tubuh diatas normal yaitu >37,5 ℃. Jika anak mengalami demam maka anak akan mudah rewel bahkan sampai menangis. Tujuan karya ilmiah ners ini adalah untuk mendapatkan gambaran asuhan keperawatan dengan pemberian terapi water tepid sponge dan kompres aloe vera. Penelitian ini menggunakan metode desain studi kasus yang dilakukan secara deskriptif dengan pendekatan asuhan keperawatan. Penelitian ini dilakukan pada tanggal 08-10 Mei 2025 dengan sampel yaitu  satu responden yang dilakukan di Ruangan Anggrek RSUD Arifin Achmad Provinsi Riau. Pada An. A keluhan utama pasien mengatakan demam, badan terasa panas, keluarga pasien mengatakan anaknya demam sejak 5 hari, demam turun naik, pasien tampak lemas, T : 38,8 ℃, dari keluhan yang dirasakan oleh pasien maka dirumuskan diagnosa hipertermia b.d proses penyakit (infeksi) d.d suhu tubuh diatas nilai normal. Salah satu intervensi yang tepat untuk menurunkan suhu pada An. A adalah terapi water tepid sponge dan kompres aloe vera. Implementasi dilakukan selama 3 hari berturut-turut yang dilakukan pada pagi hari selama ±20 menit. Hasil asuhan keperawatan ini didapatkan adanya penurunan suhu menjadi 37,10C yang diukur menggunakan termometer. Hal ini menunjukkan bahwa terapi water tepid sponge dan kompres aloe vera dapat menurunkan suhu tubuh pada anak. Diharapkan pasien dan keluarga mengaplikasikan terapi water tepid sponge dan kompres aloe vera secara rutin dengan harapan suhu tubuh menjadi normal.

Abstract

ABSTRACT Problems that can occur if someone has gouty arthritis include permanent joint damage, the appearance of lumps (tophi) under the skin, kidney stones, and even kidney failure if this condition is not treated properly. In addition, this disease can also increase the risk of other chronic diseases such as high blood pressure/hypertension. High uric acid levels are indirectly associated with an increased risk of cardiovascular diseases, such as coronary heart disease and stroke, as they can damage blood vessel linings. Chronic pain, limited mobility, and other health problems can significantly reduce quality of life. The purpose of this study was to determine the difference in uric acid levels before and after administering a decoction of binahong leaves. This study was conducted in Kualu Nenas Village, Tambang Community Health Center Working Area. This study was a qualitative descriptive study. This study was conducted from June 22-24, 2025. The main complaints experienced by the client were joint pain, especially at night, swelling of the right foot, difficulty walking, and redness. The pain felt like being pricked by needles. The client also complained of difficulty falling asleep, stating that sleep was restless and she often woke up at night. During the day, the client also had difficulty sleeping. The uric acid level test result was 7.2 mg/dl. Vital signs were as follows: BP: 135/95 mmHg, HR: 92 beats per minute, RR: 20 breaths per minute, temperature: 36.7°C. The intervention provided by the researcher was the administration of boiled binahong leaf water to reduce uric acid levels in patients with gouty arthritis. The results showed a decrease in uric acid levels after administering boiled binahong leaf water twice a day. It is hoped that clients and their families will continue to consume boiled binahong leaf water if uric acid levels increase.
