Abstract

The COVID-19 pandemic has affected various aspects of people's lives. To prevent transmission, one of the policies made is to Work From Home (WFH) which allows workers to work more flexibly and still meet the requirements for physical distancing. However, WFH can also cause a decrease in physical activity in workers and have a negative impact on workers' health. Low back pain (LBP) and headache are complaints that are often felt by workers due to WFH. The purpose of this Community Service activity is to increase public knowledge about low back pain and headaches due to WFH. This activities is carried out online using a zoom meeting in the form of webinar with the lecture method using PPT, questions and answers, and discussions as well as sharing experiences about LBP and headaches. The target participants are 30 people. The participants seemed serious and enthusiastic in participating in the webinar as evidenced by the many questions asked. Although the participants' education was quite good, knowledge about LBP and headaches was still lacking.

Abstract

Abstract

Return of spontaneous circulation after cardiopulmonary resuscitation is often followed by hypoxic-ischemic brain injury, which may affect neurological recovery. Electroencephalography (EEG) is commonly used to evaluate brain function and prognosis in these patients. EEG patterns can be classified into benign, malignant, and highly malignant categories, which reflect the severity of brain injury. Objective of this study was to evaluate the relationship between EEG patterns and clinical outcomes in patients after return of spontaneous circulation. This case series included six patients who underwent EEG examination more than 24 hours after cardiac arrest. EEG findings were classified according to the Westhall classification and compared with clinical outcomes using the Cerebral Performance Category (CPC) scaler The Results were Four patients showed highly malignant EEG patterns, including electrocerebral inactivity or continuous suppression, and all had poor outcomes (CPC 4–5). Two patients with benign EEG patterns had relatively better outcomes (CPC 3–4). No patient achieved good neurological recovery (CPC 1–2). Low EEG reactivity was observed in all cases, and amplitudes below 10 µV were mostly found in patients with CPC 5. Highly malignant EEG patterns were strongly associated with poor neurological outcomes after return of spontaneous circulation. EEG may be useful as part of a multimodal approach for prognostic evaluation in post-cardiac arrest patients.

Abstract

Introduction: Nephrotic syndrome is classically associated with a hypercoagulable state driven by the compensatory hepatic overproduction of procoagulant proteins and platelet hyperreactivity. However, massive proteinuria can rarely disrupt this paradigm, inducing a paradoxical bleeding diathesis that presents a significant clinical challenge. Discussion: A 22-year-old man presented to the emergency department with a two-week history of generalized edema and a recent onset of melena. Laboratory investigations revealed profound hypoalbuminemia (1.1 g/dL) and extraordinary, massive nephrotic-range proteinuria (63.42 g/24 h), accompanied by dyslipidemia and anemia. Coagulation profiling demonstrated a significantly prolonged activated partial thromboplastin time (aPTT) of 51.4 seconds, confirming an active upper gastrointestinal hemorrhage in the setting of ultra-severe nephrotic. He was treated with methylprednisolone, furosemide, candesartan, and simvastatin, together with albumin transfusion for severe hypoalbuminemia. Conclusion: This case highlights a rare hemorrhagic paradox in nephrotic syndrome and underscores the importance of prompt coagulation workup, including a mixing study, when bleeding occurs in a condition typically considered prothrombotic
