Abstract

Tuberkulosis (TBC) masih menjadi beban kesehatan masyarakat terutama di negara dengan insidensi tinggi seperti Indonesia. Strategi Directly Observed Treatment Short-course (DOTS) yang direkomendasikan WHO berperan penting dalam meningkatkan keberhasilan pengobatan, namun kepatuhan selama fase intensif masih menjadi tantangan. Penelitian bertujuan menganalisis hubungan implementasi strategi DOTS dengan kepatuhan minum obat anti-TB pada fase intensif pada pasien TBC paru di Kabupaten Bone Bolango. Penelitian ini menggunakan desain kuantitatif analitik dengan pendekatan cross-sectional. Sampel sebanyak 75 responden diperoleh melalui total sampling. Data dikumpulkan menggunakan kuesioner terstruktur dan dianalisis menggunakan uji Chi-square dan regresi logistik. Hasil penelitian menunjukkan terdapat hubungan signifikan antara seluruh komponen DOTS dengan kepatuhan: komitmen pemerintah (p=0.006), pemeriksaan mikroskopis (p=0.003), pengawasan minum obat (p=0.000), ketersediaan obat (p=0.002), serta sistem pencatatan dan pelaporan (p=0.004). Faktor yang paling dominan adalah pengawasan minum obat, dengan p-value 0,001 dan OR = 0,133 (CI: 0.039 – 0.459). Implementasi DOTS sangat berhubungan dengan kepatuhan minum obat fase intensif. Penguatan peran pengawas minum obat, ketersediaan obat, dan sistem pelaporan diperlukan untuk meningkatkan keberhasilan pengobatan.

Abstract

The prevalence of obesity has continued to increase every year. The WHO reports that in 2022, more than 160 million children and adolescents aged 5-19 years will be obese. Obesity has become a growing public health concern in low- and middle-income countries such as Indonesia due to changes in diet, sedentary lifestyles, and weak food control policies. This study aims to analyze the factors that contribute to obesity in adolescents, including genetic factors, gender, dietary Advanced Glycation End products intake (dAGE intake), eating frequency, water consumption, physical activity, and sleep duration. This study employed a quantitative approach with a cross-sectional design. The sample consisted of 50 students selected using a total population sampling technique. The research instruments included a 24-hour food recall, the International Physical Activity Questionnaire (IPAQ), and the Pittsburgh Sleep Quality Index (PSQI). All instruments demonstrated acceptable psychometric properties, with significance values below 0.05 and Cronbach’s alpha coefficients exceeding 0.70. Statistical analysis showed that physical activity (p = 0.027) and sleep duration (p = 0.006) were significantly associated with obesity. Other factors, including genetics, gender, dAGE intake, eating frequency, and water consumption, were not significantly associated with obesity. Sleep duration was identified as the most influential factor (Exp(B) = 0.103). Factors such as physical activity and sleep duration were associated with obesity. Adolescents need to focus on increasing physical activity, adopting healthy eating habits, and regulating optimal sleep duration as early obesity prevention measures.

Abstract

Dental caries is a chronic disease characterized by the destruction of hard dental tissues, including enamel, dentin, and cementum, resulting from bacterial activity within dental plaque that produces acids from food residues, particularly sugars and carbohydrates, adhering to tooth surfaces. These acids cause enamel demineralization, which, if it progresses, leads to the formation of cavities. This study aimed to analyze the risk factors associated with dental caries occurrence based on the def-t index among children aged 6–7 years in Tolinggula District, North Gorontalo Regency. This cross-sectional study involved 74 children aged 6–7 years selected using purposive sampling. Data were collected through structured questionnaires and clinical examinations, including def-t index assessment and salivary pH measurement. The questionnaire had undergone expert content validation and demonstrated acceptable reliability (Cronbach’s alpha > 0.70). Data were analyzed using the chi-square test and multiple linear regression. The findings revealed significant associations between dental caries occurrence based on the def-t index and the consumption behavior of cariogenic foods and beverages (p = 0.000), toothbrushing frequency (p = 0.000), and salivary pH (p = 0.000). Among these variables, salivary pH was identified as the most dominant factor associated with dental caries occurrence (p = 0.000). Several risk factors were found to be significantly associated with dental caries occurrence based on the def-t index among children aged 6–7 years in Tolinggula District, North Gorontalo Regency, with salivary pH being the strongest contributing factor.

Abstract

Tuberculosis (TB) remains a major public health burden, particularly in high-incidence countries such as Indonesia. The World Health Organization–recommended Directly Observed Treatment Short-course (DOTS) strategy plays a crucial role in improving treatment success; however, patient adherence during the intensive phase of treatment remains a significant challenge. This study aimed to analyze the relationship between the implementation of the DOTS strategy and adherence to anti-tuberculosis medication during the intensive phase among pulmonary TB patients in Bone Bolango Regency. This study employed a quantitative analytical design with a cross-sectional approach. A total of 75 respondents were recruited using total sampling. Data were collected through a structured questionnaire and analyzed using Chi-square tests and logistic regression analysis. The findings demonstrated significant associations between all DOTS components and treatment adherence, including government commitment (p = 0.006), microscopic examination (p = 0.003), treatment supervision (p = 0.000), drug availability (p = 0.002), and recording and reporting systems (p = 0.004). Treatment supervision was identified as the most dominant factor influencing adherence, with a p-value of 0.001 and an odds ratio (OR) of 0.133 (95% CI: 0.039–0.459). The implementation of the DOTS strategy is strongly associated with adherence to anti-tuberculosis medication during the intensive phase of treatment. Strengthening the role of treatment supervisors, ensuring consistent drug availability, and improving recording and reporting systems are essential to enhance treatment success.
