Abstract

Pasien terkonfirmasi COVID-19 yang menjalani isolasi akan menimbulkan rasa kesepian, kecemasan, kebosanan, kesedihan, stres dan trauma sehingga dapat memicu respon fisiologi yang dapat menyebabkan penurunan imunitas serta memperburuk kondisi kesehatan pasien terkonfirmasi COVID-19. Jenis penelitian adalah kualitatif dengan pendekatan fenomenologi deskriptif. Partisipan dalam penelitian ini 13 orang yang dipilih dengan teknik purposive sampling. Metode pengumpulan data dengan wawancara mendalam secara langung dan wawancara tidak langsung dengan durasi 50-60 menit. Alat pengumpulan data berupa kuesioner data demografi, panduan wawancara, catatan lapangan dan alat perekam suara. Metode analisa data mengunakan pendekatan Colaizzi. Hasil penelitian ditemukan lima tema yaitu respon fisiologi, respon psikologi, mekanisme koping, kebutuhan memperoleh dukungan selama menjalani perawatan COVID-19, dan dinyatakan negatif COVID-19. Pengalaman pasien terkonfirmasi COVID-19 mengalami berbagai tantangan. Mayoritas partisipan menyatakan memiliki respon negatif saat menjalani perawatan diruang isolasi COVID-19 maka dibutuhkan mekanisme koping yang stabil serta dukungan keluarga, teman, dan masyarakat. Peranan tim medis dalam pemberian intervensi klinis dan kualitas pelayanan sangat berpengaruh terhadap proses pemulihan pasien terkonfirmasi COVID-19. Pemberian perawatan secara holistik dari tim medis dapat meningkatkan pendekatan manajemen diri yang membantu pasien terkonfirmasi COVID-19 semangat melawan penyakit yang dialami.

Abstract

Stroke is a term used to describe neurological changes caused by a disruption in the blood supply to part of the brain. Stroke can cause a decrease or even loss of function controlled by these tissues, one of the symptoms that arises is muscle weakness in the affected body parts such as the fingers. Stroke is a clinical sign that develops rapidly due to focal or global brain function disorders, it can cause death for no other cause than vascular. One way to increase muscle strength and help recovery of extremities is by holding a serrated rubber ball therapy. This article uses a case study design and the subjects used in this case are patients with non-hemorrhagic stroke disorders who are treated at RSI Malahayati Medan with nursing problems of impaired physical mobility due to muscle weakness in the extremities. The results of implementing the intervention showed that after the serrated rubber ball grasping therapy was carried out for 4 days, it was carried out 2 times in 1 day with a duration of 7-10 minutes. After 4 days of intervention, muscle strength of 5-4 was obtained. Conclusion: The application of the serrated rubber ball grasping therapy technique in non-hemorrhagic stroke patients can be used to help increase muscle strength. Suggestion: It is hoped that patients can carry out independent therapeutic actions by holding a serrated rubber ball when they experience weakness in the muscles of the extremities.

Abstract

One of the cardiovascular diseases that can cause death is Unstable Angina Pectoris (UAP) Benson relaxation is a passive relaxation technique without using muscle tension so it is very appropriate to reduce pain. Benson relaxation is a development of the relaxation response method by involving the patient's belief factor, which can create a calm internal environment that can help patients achieve higher health and well-being. Method:Descriptive method in the form of case studies. Nursing care approach consisting of from assessment, nursing diagnosis, planning, implementation, evaluation and documentation nursing. The subject of this study was one person. Results:Benson relaxation is a passive relaxation technique without using muscle tension so it is very appropriate to reduce pain. Conclusion: Benson Relaxation therapy in patients with Unstable Angina Pectoris (UAP) was carried out for 3 days. It was found that there was a decrease in the pain scale. This is evidenced by the results of the evaluation on the first day, March 1, 2024, the pain scale was 7 to 7. The second day, March 2, 2024, the pain scale was 7 to 5. The third day, March 3, 2024, the pain scale was 5 to 3.

Abstract

Diabetes mellitus is a chronic disorder that arises when the pancreas does not produce enough insulin or when the body cannot use the insulin it produces properly, diabetic ulcers also referred to as diabetic foot ulcers, are medical conditions characterized by chronic, non-healing, sunken ulcers with edema with clear boundaries. Wounds in diabetic patients occur due to partial or complete damage to the skin that extends to the tissue under the skin, tendons, muscles, bones, or joints that occur in a person suffering from diabetes mellitus. Wound care that is currently developing is modern wound care which has a better rate of development of diabetic wound repair compared to using conventional wound care. The use of dressings must be considered to maintain wound moisture balance, one of the dressings that can be used is zinc cream as a primary dressing. Purpose: To prevent infection and help accelerate granulation growth in diabetic foot wound patients using zinc cream. Methods: By means of data collection and observation using case studies and subjects in diabetic foot wound patients with nursing problems of skin / tissue integrity disorders. Results: After taking wound care measures using zinc cream primary dressing for 12 times in 6 weeks there is a BWAT value in the dorsum pedis area from 37 to 22 and in the planta pedis area from 39 to 24. Conclusion: Treatment using zinc cream as a primary dressing proved effective in accelerating the healing process of diabetic foot wounds.

Abstract

Auditory hallucinations are a common sensory perception disorder in patients with mental disorders. Dhikr therapy as a non-pharmacological intervention is proven to have a calming effect and help patients focus more on reality. A case study was conducted on one patient at Prof. Dr. M. Ildrem Mental Hospital in North Sumatra for seven days using the nursing process approach. Evaluation results with the AHRS scale showed a decrease in scores from 25 to 13. Patients experienced a decrease in the intensity of hallucinations and increased self-control. The conclusion of this case study is that dhikr therapy is effectively applied in mental nursing care as a nonpharmacological intervention to help patients control auditory hallucinations.

Abstract

Isolation is an unpleasant experience. Changes in the environment, physical isolation from loved ones and boredom cause negative effects that can deteriorate the health of confirmed COVID-19 patients. Explorasi of the experiences of COVID-19 confirmed patient from several research articles aims to help identify the quality of life of patient with confirmed COVID-19. Methods: PRISMA is used for evaluation of research results. SPIDER to formulate research question with the database used by PubMed, CINAHL, Proquest, and Google Scholar. JBI is used to extract analyze data. Results: 6 qualitative studies included in the review. The experiences of 108 COVID-19 confirmed patients with 32 findings resulted in 7 categories based on similarities in meaning and three themes that were synthesized from categories, namely the physiological response of patients with confirmed COVID-19, the psychological response of patients with confirmed COVID-19, and treatment in health fasilities. Conclusion: patients with a confirmed COVID-19 experience various responses while undergoing treatment in the COVID-19 isolation room. In response to this response, patients with confirmed COVID-19 apply different coping mechanisms. In this condition, social support is an important factor in supporting the recovery of patients with confirmed COVID-19. Providing services in the COVID-19 isolation ward needs to rely on and understand the natural condicions of patients with confirmed COVID-19 by developing appropriate intervention strategies.

Abstract

Halusinasi pendengaran adalah gangguan persepsi sensori yang umum pada pasien gangguan jiwa. Halusinasi pendengaran menimbulkan distres, risiko perilaku berbahaya, dan beban perawatan; intervensi non-farmakologis terstruktur seperti terapi dzikir berpotensi menjadi adjuvan untuk menurunkan intensitas gejala dan meningkatkan kontrol diri. Terapi dzikir sebagai intervensi nonfarmakologis terbukti memberikan efek menenangkan dan membantu pasien lebih fokus pada realita. Tujuan studi kasus ini untuk Mendeskripsikan perubahan skor Audiotory Hallucination Rating Scale (AHRS) setelah penerapan terapi dzikir terstruktur yang diintegrasikan dengan strategi SP1–SP4 pada seorang pasien dengan halusinasi pendengaran. Studi kasus dilakukan pada satu pasien di RSJ Prof. Dr. M. Ildrem Sumatera Utara selama tujuh hari menggunakan pendekatan proses keperawatan. Hasil evaluasi dengan skala AHRS menunjukkan penurunan skor dari 25 menjadi 13. Pasien mengalami penurunan intensitas halusinasi dan peningkatan kontrol diri. Kesimpulan dari studi kasus ini adalah terapi dzikir efektif diterapkan dalam asuhan keperawatan jiwa sebagai intervensi nonfarmakologis untuk membantu pasien mengontrol halusinasi pendengaran. Temuan ini mendukung integrasi terapi dzikir terstruktur sebagai adjuvan dalam SOP perawatan standar dengan pemantauan AHRS pra–pasca, disertai pelatihan singkat bagi perawat dan pelibatan keluarga untuk teknik menghardik, interaksi suportif, dan penjadwalan aktivitas; ke depan, uji terkontrol bersampel lebih besar dengan penilaian tersamar serta pencatatan waktu sesi (pagi/siang) diperlukan untuk menguatkan evidensi dan mengoptimalkan implementasi klinik.

Abstract

Diabetic foot ulcer is a common and serious complication of diabetes mellitus that requires effective wound management to accelerate healing and prevent further complications. Objective: This study aimed to evaluate the effectiveness of moist wound healing using hydrogel in improving wound healing outcomes among patients with diabetic foot ulcers at Asri Wound Care Center Medan. Methods: This study employed a case study design with purposive sampling. A total of five patients with diabetic foot ulcers stage 4 were included, consisting of three patients in the treatment group and two patients in the control group. Wound healing outcomes were assessed using the Bates-Jensen Wound Assessment Tool (BWAT) over seven wound care sessions. Results: The treatment group demonstrated a more significant improvement in wound healing compared to the control group. BWAT scores in the treatment group decreased from 43 to 24 (R1), 40 to 20 (R2), and 39 to 20 (R3), indicating marked reductions in necrotic tissue, slough, biofilm, and exudate, along with improved wound base and granulation tissue formation. In contrast, the control group showed smaller reductions in BWAT scores, from 40 to 34 (R4) and 41 to 33 (R5), reflecting a slower healing process with residual necrotic tissue and exudate. Conclusion: Moist wound healing using hydrogel was more effective than conventional wound care in accelerating the healing of diabetic foot ulcers, as evidenced by greater BWAT score reductions and improved wound characteristics. These findings support the use of hydrogel as an effective dressing in diabetic wound management.

Abstract

Diabetic foot ulcer is one of the most serious complications in individuals with diabetes mellitus, resulting from peripheral neuropathy, impaired vascularization, and chronic infection, which collectively delay the wound healing process. Proper wound management is essential to prevent severe outcomes such as amputation. Methods: This study employed a case study design to provide an in-depth clinical description of wound healing progression in patients with stage 4 diabetic foot ulcers. Purposive sampling was used to select three patients treated at Asri Wound Care Center Medan. Zinc cream was applied as an adjunct therapy for six weeks, with treatment administered twice weekly. Wound healing was evaluated using the Bates-Jensen Wound Assessment Tool (BWAT) and clinical observations. Results: All patients demonstrated observable improvements in BWAT scores, with reductions from 37 to 22, 40 to 26, and 39 to 24, respectively. Clinical observations indicated improved wound healing indicators, including increased granulation tissue, reduced exudate, and enhanced epithelialization. Conclusion: The application of zinc cream as an adjunct therapy may support the wound healing process in patients with stage 4 diabetic foot ulcers. However, due to the limited sample size and the absence of a control group, the findings should be interpreted cautiously, and further studies with larger samples and comparative designs are recommended.
