Abstract

This study aims to analyze the relationship between the role of drug supervisors (PMOs) on improving the quality of life of tuberculosis patients. This research method is quantitative research with correlation research design with a sample of 40 tuberculosis patients. The sampling technique used purposive sampling according to the inclusion criteria. Data analysis used Chi-Square test. The results showed that 29 people (72.5%) had an active PMO role. The majority of quality of life based on aspects obtained in poor physical health is 33 people (82.5%) and the overall quality of life is good as many as 26 people (65%). Chi-Square statistical test results with an Asym.Sig value of 0.034 <0.05 means that there is a significant relationship between the role of PMO and quality of life. In conclusion, PMO has a very important role in the process of treating TB patients regularly so as to improve the health status and quality of life of patients. Abstract

Health problems in the elderly are increasingly becoming a major concern at global and national levels, along with increasing life expectancy. One of the most common health problems faced by the elderly is musculoskeletal disease, including rheumatism. This study aimed to determine the effect of rheumatic exercise on the ability of elderly individuals to perform daily activities. This study is a quantitative study using a quasi-experimental one-group pre-test and post-test design. The results showed that The results of the Wilcoxon rank test were p = 0.000, which was smaller than α = 0.05. This study shows the effect of rheumatic exercise on the ability of the elderly to perform daily activities. This shows that rheumatic exercise is an effective intervention to support the ability of the elderly to face physical and psychological challenges due to aging and chronic diseases, such as rheumatoid arthritis.

Abstract

Poor fluid management among hemodialysis patients can lead to detrimental outcomes such as high Interdialytic Weight Gain (IDWG), contributing to increased cardiovascular morbidity and mortality. While conventional education on fluid restriction exists in hospitals, non-compliance remains prevalent, highlighting the need for educational approaches grounded in self-efficacy theory to enhance patients' self-confidence in self-management. This study aimed to assess the impact of educational interventions based on self-efficacy theory on IDWG in hemodialysis patients at Malahayati Islamic Hospital. Using a quantitative quasi-experimental pretest-posttest control group design, the study included a total of 68 participants selected through purposive sampling. Statistical analysis revealed a significant difference (p < 0.05) in IDWG scores between the intervention and control groups post-intervention, indicating the effectiveness of self-efficacy-based education in improving compliance with fluid intake restrictions among chronic kidney failure patients undergoing hemodialysis.

Abstract

Diabetes mellitus is a chronic disorder that arises when the pancreas does not produce enough insulin or when the body cannot use the insulin it produces properly, diabetic ulcers also referred to as diabetic foot ulcers, are medical conditions characterized by chronic, non-healing, sunken ulcers with edema with clear boundaries. Wounds in diabetic patients occur due to partial or complete damage to the skin that extends to the tissue under the skin, tendons, muscles, bones, or joints that occur in a person suffering from diabetes mellitus. Wound care that is currently developing is modern wound care which has a better rate of development of diabetic wound repair compared to using conventional wound care. The use of dressings must be considered to maintain wound moisture balance, one of the dressings that can be used is zinc cream as a primary dressing. Purpose: To prevent infection and help accelerate granulation growth in diabetic foot wound patients using zinc cream. Methods: By means of data collection and observation using case studies and subjects in diabetic foot wound patients with nursing problems of skin / tissue integrity disorders. Results: After taking wound care measures using zinc cream primary dressing for 12 times in 6 weeks there is a BWAT value in the dorsum pedis area from 37 to 22 and in the planta pedis area from 39 to 24. Conclusion: Treatment using zinc cream as a primary dressing proved effective in accelerating the healing process of diabetic foot wounds.

Abstract

Lemahnya sistem rujukan berdampak pada penggunaan sumber daya manusia dan teknologi canggih tidak tepat guna berakibat pada penumpukan pasien yang terjadi di Rumah Sakit dan pada  akhirnya berdampak pada menurunnya kualitas pelayanan kesehatan. Penelitian dilakukan dengan menggunakan metode kuantitatif dengan pendekatan cross sectional yang bersifat survey analitik. Jumlah sampel yaitu 96 orang  yang ditarik dengan metode purposive sampling. Instrumen data menggunakan kuesioner dengan item corrected correlation 0,361 dan cronbach alpha = 0,6. Analisa data dilakukan dengan X2 test pada uji bivariat dan regresi logstik untuk uji multivariat pada taraf nyata α =0,05. Hasil analisa menemukan bahwa terdapat hubungan kepastian waktu(p=0,005), kepastian rujukan (p=0,007), waktu tunggu (p=0,034), akses layanan kesehatan(p=0,011), mutu layanan kesehatan(p=0,001), proses layanan kesehatan(p=0,004) sistem layanan kesehatan(p=0,003) dengan kepuasan pasien rawat jalan BPJS dan variabel yang paling dominan berhubungan dengan kepuasan pasien adalah kepastian rujukan (OR=8,886). Dari hasil penelitian disimpulkan bahwa terdapat hubungan kepastian waktu, kepastian rujukan, waktu tunggu, akses layanan kesehata, mutu layanan kesehatan, proses layanan kesehatan dan sistem layanan kesehatan dengan kepuasan pasien. Disarankan kepada dokter agar tepat waktu dalam pelayanan agar pelayanan lebih optimal.

Abstract

Premenstrual Syndrome (PMS) merupakan kumpulan gejala fisik dan emosional yang dialami perempuan menjelang menstruasi. Pengetahuan dan kemampuan remaja dalam mengelola PMS secara tepat sangat penting untuk menjaga kualitas hidup dan prestasi belajar. Namun, literasi kesehatan reproduksi di kalangan remaja masih tergolong rendah, khususnya di lingkungan sekolah menengah pertama. Penelitian ini bertujuan untuk mengetahui gambaran tingkat pemahaman dan strategi pengelolaan gejala PMS pada remaja putri di MTsN 1 Rantau Selatan. Penelitian ini menggunakan pendekatan kuantitatif deskriptif dengan desain potong lintang. Sampel penelitian sebanyak 60 siswi yang dipilih secara purposive. Data dikumpulkan melalui kuesioner tertutup yang mencakup aspek pengetahuan dan cara mengelola gejala PMS. Analisis data dilakukan secara univariat menggunakan distribusi frekuensi dan persentase. Hasil penelitian menunjukkan bahwa 56,7% responden memiliki tingkat pemahaman yang baik, 33,3% cukup, dan 10,0% kurang. Dalam pengelolaan PMS, 78,3% responden memilih istirahat cukup, 73,3% mengonsumsi makanan sehat, 63,3% menggunakan kompres hangat, dan hanya 41,7% yang melakukan olahraga ringan. Sementara itu, 20,0% responden menggunakan obat pereda nyeri. Mayoritas remaja memiliki pemahaman yang baik terhadap PMS dan memilih strategi pengelolaan non-farmakologis. Namun, partisipasi dalam pengelolaan aktif seperti olahraga masih rendah. Intervensi edukatif yang terstruktur diperlukan untuk meningkatkan literasi dan praktik pengelolaan PMS di kalangan remaja.

Abstract

Penyakit tidak menular (PTM) merupakan tantangan kesehatan utama yang memerlukan pendekatan pelayanan berbasis keluarga. Perawat memiliki peran sentral dalam memberikan asuhan keperawatan keluarga, terutama di tingkat pelayanan primer seperti Puskesmas. Namun, pengalaman perawat dalam konteks tersebut masih jarang dieksplorasi secara mendalam. Penelitian ini bertujuan untuk mengeksplorasi pengalaman perawat dalam memberikan asuhan keperawatan keluarga kepada pasien dengan PTM di Puskesmas Sunggal, Kota Medan. enelitian menggunakan pendekatan kualitatif desain fenomenologi dengan delapan informan, terdiri dari enam perawat, satu kepala Puskesmas, dan satu koordinator program PTM. Data dikumpulkan melalui wawancara mendalam semi-terstruktur, dianalisis menggunakan metode Colaizzi. Hasil penelitian ditemukan tiga tema utama, yaitu: (1) Komitmen profesional terhadap asuhan keluarga yang mencakup keterikatan emosional dan rasa tanggung jawab; (2) Hambatan struktural dan kultural, termasuk beban kerja tinggi, keterbatasan waktu, dan persepsi keluarga yang pasif; serta (3) Kebutuhan dukungan sistem dan peningkatan kapasitas perawat seperti tidak adanya SOP serta perlunya pelatihan edukasi dan komunikasi keluarga. Asuhan keperawatan keluarga bagi pasien PTM di Puskesmas Sunggal dipengaruhi oleh interaksi personal, keterbatasan struktural, dan belum optimalnya dukungan sistem. Diperlukan kebijakan operasional yang mendukung, pelatihan intensif bagi perawat, serta pendekatan kolaboratif yang melibatkan keluarga sebagai mitra aktif dalam perawatan.

Abstract

Diabetic foot ulcer is one of the most serious complications in individuals with diabetes mellitus, resulting from peripheral neuropathy, impaired vascularization, and chronic infection, which collectively delay the wound healing process. Proper wound management is essential to prevent severe outcomes such as amputation. Methods: This study employed a case study design to provide an in-depth clinical description of wound healing progression in patients with stage 4 diabetic foot ulcers. Purposive sampling was used to select three patients treated at Asri Wound Care Center Medan. Zinc cream was applied as an adjunct therapy for six weeks, with treatment administered twice weekly. Wound healing was evaluated using the Bates-Jensen Wound Assessment Tool (BWAT) and clinical observations. Results: All patients demonstrated observable improvements in BWAT scores, with reductions from 37 to 22, 40 to 26, and 39 to 24, respectively. Clinical observations indicated improved wound healing indicators, including increased granulation tissue, reduced exudate, and enhanced epithelialization. Conclusion: The application of zinc cream as an adjunct therapy may support the wound healing process in patients with stage 4 diabetic foot ulcers. However, due to the limited sample size and the absence of a control group, the findings should be interpreted cautiously, and further studies with larger samples and comparative designs are recommended.

Abstract

Hallucinations are a perceptual disorder in which patients perceive something that is not actually happening. The application of dhikr therapy in hallucinatory patients aims to control hallucinations, because this aspect is shown to maximize the benefits, treatment and feelings of peace for patients with hallucinations. Method: case study design using a descriptive method with a nursing process approach in 5 patients focused on one of the important problems in the case of auditory hallucination nursing care. Patients were taught dhikr therapy by reading tasbih ( Suhanallah ) 33 times, tahmid (alhamdulillah) 33 times and takbir ( Aallahu Akbar) 33 times, this therapy was carried out 2 times a day for 7 days with a duration of 15 minutes. The instrument used in the study was AHRS. Results: The first respondent to the implementation of dhikr therapy found an AHRS score of 25 to 13. In the second respondent to the implementation of dhikr therapy found an AHRS score of 26 to 14. In the third respondent to the implementation of dhikr therapy found an AHRS score of 24 to 13. the implementation of dhikr therapy found an AHRS score of 26 to 13. Conclusion: dhikr therapy is effective in reducing the frequency and intensity of auditory hallucinations and increasing the calmness of patients with auditory hallucinations.

Abstract

Gout arthritis is a metabolic disorder caused by the accumulation of uric acid crystals in the joints, causing acute pain. One non-pharmacological intervention to reduce uric acid levels is ergonomic exercise. This study aims to determine the effectiveness of ergonomic exercise in reducing uric acid levels in gout arthritis patients. Methods: A descriptive case study design was used in six respondents with gout arthritis and acute pain. The ergonomic exercise intervention was performed three times a week for 20 minutes for one week. Uric acid levels were measured before and after treatment. Results: A significant decrease in uric acid levels was observed in all respondents. Conclusion: Ergonomic exercise is effective in reducing uric acid levels in gout arthritis patients with acute pain.
