Abstract

Patients with active lung tuberculosis (TB) have low body mass index (BMI) because of a wasting. Changes in body weight during the initial period of TB therapy correlated with the death risk, illness severity and relapse as well as decrease of patients physical function. This study aimed to measure the effectiveness of soybean supplementation among TB patients with standard therapy in improving physical function and nutritional status. Fifty nine patients were randomly divided into two groups.Intervention group received one hundred fifty gram nutritional supplementation and control group did not receive anything. Both groups were on the same regular antituberculosis treatment. Assessment of outcomes were done twice, before supplementation and then after 1 month of the treatment. There were significant differences in the changes in proxy physical function parameter in the control and treatment group (3.16 ± 0.83 vs. 4.50 ± 1.47, P < 0.05) and changes in nutritional status variable (0.33 ± 0.15 vs. 0.47 ± 0.16, p < 0.05). However there was no significant difference in the change in mid upper arm circumference (0.83 ± 0.31 vs. 0.93 ± 0.31, p > 0.05). Further research is needed to confirm this result.

Abstract

Statins are the most widely prescribed drug for hyperlipidemic therapy and for the prevention of Cardiovascular disease. But although statins are very effective for the prevention of atherosclerosis, there are still patients with CVD. In this case, it is thought to be the influence of genetic factors. There is a static gene link to statin dose required for therapy, but there are still no relevant pharmacogenomic tests for statin therapy guidelines. This article describes basic pharmacogenomic terminology, genes (CETP, HMGCR, SLCO1B1, ABCB1, and CYP3A4 / 5), and progression of statin pharmacogenomic biomarkers for predicted therapeutic outcomes.

Abstract

Background: Monkey pox is an infectious disease caused by viruses of the family Poxviridae, genus Orthopoxvirus, which can infect humans, vertebrates and arthropods. Symptoms of monkey pox include increased body temperature, headache, muscle aches, weakness, swollen lymph nodes in the neck, armpits and groin, and skin rashes. Giving antivirals is very effective in treating monkey pox infections caused by the monkey pox virus. Cidofovir is an antiviral that is given intravenously, for the treatment of monkey pox patients. The effectiveness of cidofovir in cases of monkeypox in humans is still uncertain. However, this drug has been shown to fight poxviruses in in vitro and animal studies. Based on this background, researchers are interested in conducting research on "Effectiveness of the Antiviral Cidofovir in Monkeypox Patients with Severe Symptoms". Method: This research is descriptive qualitative research with library research. Search for articles in this literature review using the 2010-2022 database with keywords according to the topic. After getting the reference sources, they were filtered until they got 25 journals consisting of 15 international journals, 10 national journals, and 2 textbooks. Result: The results of a literature study in 11 research journals were found to show the effectiveness of the antiviral cidofovir in monkey pox patients with severe symptoms, with recovery between 14-21 days at the fastest and between 35-52 days at the longest. Conclusion: The antiviral cidofovir is effective for treating monkeypox with severe symptoms. Apart from that, it was also found that the cure rate for monkey pox patients treated with cidofovir reached 57% -85% depending on the severity of the disease.

Abstract

Latar Belakang: Hipertensi merupakan kondisi medis yang ditandai oleh peningkatan tekanan darah secara persisten melebihi batas normal, yaitu ≥140/90 mmHg. Meskipun dapat disembuhkan secara permanen, hipertensi tetap bisa dikendalikan melalui pengobatan yang tepat. Dalam hal ini, kepatuhan pasien terhadap terapi memegang peranan penting guna mencegah komplikasi jangka panjang. Tujuan: Penelitian ini bertujuan untuk menggambarkan tingkat kepatuhan serta tingkat kepuasan pasien terhadap terapi hipertensi di Puskesmas Dukuh Kupang, Surabaya, selama periode Oktober hingga Desember 2023. Metode: Penelitian ini menggunakan desain deskriptif dengan pendekatan kuantitatif. Data dikumpulkan melalui penyebaran kuesioner kepada 50 pasien hipertensi yang menjalani pengobatan di Puskesmas Dukuh Kupang. Kepatuhan terapi diukur menggunakan instrumen Morisky Medication Adherence Scale (MMAS-8), sedangkan tingkat kepuasan pasien terhadap pengobatan dievaluasi melalui Treatment Satisfaction Questionnaire for Medication (TSQM). Hasil: Dari total 50 responden, sebanyak 28% pasien menunjukkan tingkat kepatuhan tinggi, 36% dengan tingkat kepatuhan sedang, dan 36% tergolong rendah. Dari segi kepuasan terhadap layanan terapi obat hipertensi, mayoritas pasien berada dalam kategori cukup puas (74%), sedangkan sisanya (26%) mengaku sangat puas. Kesimpulan: Secara umum, tingkat kepatuhan dan kepuasan terhadap terapi hipertensi di Puskesmas Dukuh Kupang tergolong cukup baik. Namun, masih dibutuhkan upaya tambahan, khususnya untuk pasien dengan kepatuhan rendah dan kepuasan yang belum optimal, melalui peningkatan edukasi serta perbaikan mutu layanan kesehatan.

Abstract

The Covid-19 pandemic has caused significant global mortality and morbidity. Covid -19 infection is caused by the SARS-CoV-2 virus, which requires a receptor on the host cell to enter, primarily Angiotensin-Converting Enzyme 2, encoded by the ACE2 gene, and Transmembrane Serine Protease 2, encoded by the TMPRSS2 gene. Several factors can influence Covid-19 mortality and morbidity, including gender, comorbidities such as diabetes mellitus (DM), obesity, cardiovascular diseases, and genetic factors. ACE2 and TMPRSS2 polymorphisms are believed to worsen the condition of Covid-19 patients. This study aimed to identify polymorphisms in the ACE2 (rs2285666 G>C) and TMPRSS2 (rs2070788 A>G) in  Covid-19 survivors with hypertension and diabetes mellitus. The total sample was 30 hypertension and diabetes mellitus   patients treated at Islamic Hospital Surabaya. A total of 3 ml of blood was drawn from the median cubital vein and placed into an EDTA vacutainer. DNA extraction was performed using the High Pure PCR Template Preparation Kit (Roche). Identification of ACE2 and TMPRSS2 polymorphisms was performed using the rhAmp SNP genotyping method with the LightCycler® system (Roche 480II). Genotyping results for ACE2 showed that 55.2% of patients had the GG (wild-type) with a G allele frequency of 68.9%. Genotyping for the TMPRSS2 showed that 60% of patients had the GG (mutant-type) with a G allele frequency of 78.3%. During Covid-19 infection, neither GG genotype (ACE2) and the GG genotype (TMPRSS2) exhibited severe symptoms.
