Abstract

Background: Hematology analyzers (HA) are used for automated leukocyte counts, but are subject to errors if nucleated erythrocytes, fibrin precipitation, or prolonged sample sitting are present. Quality control is required to ensure accuracy of results. The clinical laboratory of the UPTD Health Laboratory of Sleman Regency, DIY has carried out quality control but has not involved Levey-Jennings control charts and six sigma analysis. This study aims to determine the internal quality assurance of the analytical stage of the HA device in the examination of leukocyte counts in that laboratory. Methods: This type of research is a descriptive study with the subject of the HA Sysmex XP-100 tool. The variables of this study are data from the quality control results of automatic leukocyte count examination in February-March 2024. Data analysis includes calculation of six sigma values, creation of Levey-Jennings control charts and evaluation using westgard rules. Results: The results showed that the examination of the number of leukocytes with the HA tool had an accuracy of 99.45% and a precision of 98.49%, while for the evaluation of the Levey-Jennings graph with the westgard rule at the normal level control, there was a rule of 12s on day 3 and 13s on day 6. Six sigma analysis shows that the performance of the tool for examining the number of leukocytes is at world class level with an average sigma value of 12.42. Conclusion: This Based on the results, it can be concluded that the examination of leukocyte counts with the HA Sysmex XP-100 at the Sleman Regency Health Laboratory UPTD, DIY has an accuracy of 99.45% and a precision of 98.49%. Evaluation of the Levey-Jennings control chart and Six Sigma analysis showed world-class performance, indicating that this tool provides excellent performance and meets international quality standards.

Abstract

The neutrophil lymphocyte ratio (NLR) is an indicator of the presence of a systemic inflammatory response. The neutrophil lymphocyte ratio parameter was calculated by comparing the number of neutrophils and the number of lymphocytes. Patients with severe COVID-19 who did not survive were found to have a higher neutrophil lymphocyte ratio compared to patients with a mild prognosis. Thisstudy uses a descriptive method, which aims to describe the value of the neutrophil lymphocyte ratio in COVID-19 patients based on the severity of symptoms. The average value of high neutrophil lymphocyte ratio (NLR) in COVID-19 patients occurred in the severe (6.12%) and critical (10.78%) categories. The average value of high neutrophils (Neutrophilia) in COVID-19 patients occurred in the severe (76%) and critical (82%) categories. The average value of low lymphocytes (lymphocytopenia) in COVID-19 patients occurred in the severe 17% and critical 12% category. The NLR value in patients with the severe category had an increase in the NLR value twice thenormal value, while in the critical category had an increase in the NLR value three times the normal value.

Abstract

Anemia is a condition in which the hemoglobin, a component in red blood cells, functions to bind oxygen and deliver it to all cells in the body so that they can carry out their functions properly. When you are anemic, your body feels weak and lethargic and gets tired easily, also known as 5L (Lethergic, Tired, Weak, Tired, Sluggish). Dizziness, cloudy eyes, and easy sleepiness accompany this. This community service aims to provide education and health checks to detect anemia early in children at the Al-Hikmah Cangkringan Orphanage, Sleman. The Methods of the study is four methods are used to implement community service activities: Target and Problem Surveys, Counseling on anemia, Health Checks (blood pressure, hemoglobin, and blood type checks), and Evaluation. The results of the study showed that examination of Hb levels in the children of Al-Hikmah Cangkringan Orphanage resulted in the category less than normal as much as 1 person (5%), normal as many as 17 people (90%) and the category above normal values as much as 1% (5%). Based on research results, most of the children at Al-Hikmah Orphanage are in the normal category and only 2 people need more supervision by the orphanage administrators.

Abstract

Transfusi darah adalah proses pemberian darah dari seseorang (donor) kepada orang lain (resipien) yang bertujuan untuk mengganti darah hilang akibat perdarahan, mengatasi shock, memelihara serta mempertahankan kesehatan resipien. Analisis pretransfusi khususnya pada uji serologi sangat diperlukan untuk menjamin darah yang ditransfusikan aman dan tidak menyebabkan reaksi transfusi. Proses pencocokan darah donor dan dengan darah pasien/resipien dilakukan melalui uji. uji silang serasi (crossmatch). Uji silang serasi dapat dilakukan melalui 2 metode yaitu yaitu menggunakan tabung dan gel card. Uji silang serasi metode tabung terdiri dari 3 fase yaitu fase 1, fase 2 dan fase 3. Pada Fase 3 merupakan tahapan yang paling berpengaruh, karena ada penambahan Anti Human Globulin (AHG). Reagen AHG berisi immunoglobulin yang berfungsi sebagai penghubung antibodi inkomplit (antibodi IgG yang menyelubungi eritrosit), sehingga akan terdeteksi melalui terbentuknya aglutinasi (gumpalan). Namun perlu diketahui bahwa ikatan antar antibodi tidak dapat bertahan lama. Ikatan antar antibodi (AHG dan antibodi inkomplit) dapat terlepas kembali (disosiasi), sehingga hasil pemeriksaan dapat menjadi false negatif. Tujuan penelitian ini adalah untuk mengetahui apakah waktu penambahan AHG mempengaruhi derajad aglutinasi pada saat uji silang serasi. Penelitian ini menggunakan metode uji silang serasi metode tabung dengan menggunakan golongan darah O rhesus positif sebagai sampel. Analisis data dilakukan melalui pengamatan derajat aglutinasi pada tiap fase uji silang serasi. Pada penelitian ini, adanya perbedaan waktu penambahan AHG mempengaruhi hasil pemeriksaan uji silang serasi. Kesimpulan dalam penelitian ini adalah efektifitas penambahan AHG pada uji silang serasi metode tabung yaitu pada menit ke 2-10 menit.
