Abstract

Wabah Virus Zika (ZIKV) merebak pada tahun 2015 hingga 2016 terutama di benua Amerika dan mendapat perhatian dunia. ZIKV merupakan penyakit yang ditularkan oleh nyamuk Aedes, umumnya self limiting dan jarang fatal pada dewasa. Perhatian lebih kepada ibu hamil karena potensi janin mikrosefalus. Centers for Disease Control and Prevention (CDC) telah mengeluarkan beberapa rekomendasi diagnosis dan manajemen ZIKV baik pada ibu hamil maupun pada bayi baru lahir. Pencegahan dan diagnosis dini ZIKV menjadi kunci utama penanganan.

Abstract

Perlemakan Hati Non Alkoholik (Non Alcoholic Fatty Liver Disease/NAFLD) merupakan salah satu penyebab utama dari penyakit hati kronis di negara berkembang. NAFLD dapat berkembang menjadi menjadi penyakit hati yang lebih berat seperti Non Alcoholic Steatohepatis (NASH), sirosis hepatis, dan karsinoma hati. Tatalaksana NAFLD terus berkembang, dan saat ini klinisi dihadapkan dengan berbagai pilihan alternatif baik dari segi nonfarmakoterapi maupun farmakoterapi. Tulisan ini akan membahas berbagai pilihan terapi NAFLD terkini tersebut berdasarkan bukti terbaru.

Abstract

Background: Sleep disturbance in pediatrics produce daytime sleepiness and decrease productivity. Obstructive sleep apnea (OSA) was the most common cause of daytime sleepiness in children. Adenoid and tonsil hypertrophy was a major determinant of OSAS in pediatrics, in addition to other disorders such as craniofacial anomalies and obesity. Understanding the correlation between adenoid, tonsil size and body mass index (BMI) can be useful in managing optimal treatment strategies. Objective: To examine the correlation between adenoid, tonsil size, and BMI with OSA severity in pediatric patient with OSAS. Methods: The medical records of the Department of Otorhinolaryngology Head and Neck Surgery were utilised for this retrospective cross-sectional study spanning from January 1, 2016 to December 31, 2018. This research comprised a cohort of nineteen patients in total. The medical record was queried for the patient's age, sex, adenoid, and tonsil dimensions. The results of a pre-operative polysomnography to assess the severity of OSA were obtained as part of a routine examination and documented in the patient's medical records. Results: The correlation between variables showed in crosstabulation and Scatter Plot. A p-value below 0.05 indicated that the correlation coefficient of -0474 between adenoid size and OSA severity was statistically significant. The correlation between tonsil size and the severity of obstructive sleep apnea (OSA) was not statistically significant (R2 < 0.3, p < 0.05). Similarly, the correlation between BMI and OSA severity showed no significancy with p-value > 0.05, respectively. Conclusion: Adenoid size measured with Cohen and Konak ratio show significant correlation with OSA severity. While tonsil size and BMI showed no significancy in OSA severity.

Abstract

Background: Chronic suppurative otitis media (CSOM) is a chronic inflammation involving the middle ear space and it became a burden especially in developing countries. Mastoidectomy remains the main treatment in CSOM with cholesteatoma. Recurrent cholesteatoma and suppurating process indicated the need of revision mastoidectomy. Purpose: Until now, study about the mechanism of recurrent cholesteatoma and its management still need further research. Case report:  This paper reports a case of chronic suppurative otitis media with recurrent cholesteatoma which underwent revision mastoidectomy. Clinical Question : What are the factors associated with the occurrence of recurrent cholesteatoma and how to managed it? Methods: Evidence base literature study about chronic suppurative otitis media with recurrent cholesteatoma and surgery treatment was performed through PubMed, ProQuest, and Google Scholar database. Results: Recurrent cholesteatoma that occurred after mastoidectomy can be caused by residue of the cholesteatoma itself or new forming of retraction pocket. Revision mastoidectomy is necessary to evacuated the cholesteatoma and provide good drainage to the mastoid. Conclusion: Routine follow up after mastoidectomy is an important thing to help prevent recurrence of CSOM especially with cholesteatoma.
