Abstract

Latar Belakang: Tekanan darah tinggi merupakan suatu keadaan dimana terjadi peningkatan tekanan darah di atas ambang batas normal. Manifestasi rongga mulut pada pasien hipertensi dapat berupa gingivitis, periodontitis, reaksi lichenoid, hiposalivasi serta kelumpuhan saraf wajah. Tujuan dari penulisan studi kasus untuk memberikan informasi penatalaksanaan emergensi pada kasus perdarahan yang keluar dari rongga mulut pasien disertai hipertensi urgensi. Laporan Kasus: Pasien laki-laki berusia 68 tahun datang ke Instalasi Gawat Darurat RSUD Sultan Fatah Demak dengan keluhan air liur keluar darah sejak jam 7 pagi. Pasien datang ke Instalasi Gawat Darurat RSUD Sultan Fatah Demak sekitar jam 9 malam. Kondisi sistemik pasien dengan kondisi muntah darah (-) batuk darah (-) demam (-) mimisan (-) trauma (-). Pasien memiliki riwayat hipertensi yang tidak terkontrol. Pasien mengatakan sudah pernah mengkonsumsi obat penurun tekanan darah namun tidak teratur. Hasil pemeriksaan klinis terdapat oral bleeding et causa hipertensi urgensi, periodontitis generalisata, dan nekrosis pulpa gigi 46. Gambaran klinis yang dapat ditemukan pada pasien hipertensi adalah perdarahan gingiva karena diperkuat dengan adanya penyakit periodontal seperti gingivitis maupun periodontitis. Kesimpulan: Kasus emergensi yang berkaitan dengan adanya riwayat hipertensi yang memiliki resiko perdarahan dapat ditangani dengan pemberian obat penghenti perdarahan kemudian sebelum dilakukan prosedur perawatan dental. Terdapat hubungan yang kuat antara riwayat hipertensi dengan kasus penyakit jaringan periodontal, sehingga perlu dilakukan rujukan kepada spesialis penyakit dalam dan spesialis bedah mulut untuk mengetahui penyebab perdarahan yang terjadi. Prognosa pasien akan baik jika dilakukan perawatan medis kolaboratif antar spesialis tersebut

Abstract

Introduction: Odontectomy is the surgical removal of teeth. The choice of anesthesia is also one of the considerations in the odontectomy surgical technique. Information regarding gender, age and comorbidities in patients can also be considered in the selection of anesthetics for odontectomy procedures. Purpose: to provide information regarding impacted teeth and to determine the frequency distribution of odontectomy patient profiles at the Oral Surgery Polyclinic of Sultan Fatah General Hospital, Demak Regency. Methods: descriptive study by collecting secondary data using a retrospective approach from medical records from visits to the Oral Surgery Polyclinic, Sultan Fatah Demak Regional General Hospital from 8-12 May 2023. Results and Discussions:  a total of 36 patients were obtained on 8-12 May 2023, with 32 patients who met the inclusion criteria. The majority of patients were male with a percentage of 55.6%. Based on the age of the most patients in the age range of 21-30 years (15 patients), while the least age group is the age group <20 years (4 patients). Based on the type of teeth, the most odontectomy performed was tooth 48 with a total of 14 teeth. Conclusion: The distribution of the frequency of odontectomy patients at Sultan Fatah General Hospital is most commonly found in the male sex, age group 21-30 years, the type of tooth that was odontectomy was tooth 48.

Abstract

Benign Fibrous Histiocytoma (BFH) is a benign soft tissue tumor consisting of fibroblastic and histiocytic cells. The most often affected locations by BFH lesions are the buccal mucosa, mandibular bones, gingiva, lower and upper lips, soft palate, and floor of the mouth. The etiology of BFH is unknown, but the etiology is thought to be reactive changes from previous infections such as insect bites or thorn pricks. Signs and symptoms of BFH include round-shaped swelling, appearing reddish at first, and turning brown. Case: A 40 year old male patient came to the Sultan Fatah Regional Hospital Demak with complaints of swelling of the left cheek below the eye, dizziness and fever for the last 7 days. Patients eat and drink with bread mixed in water or tea. The patient came in a compos mentis state, but the patient was less cooperative. Case management: The patient underwent incision and drainage. Tissue was taken for biopsy examination measuring 1 x 0.4 x 0.3 cm. The patient follow-up was carried out 7 days after the procedure. Results: Follow-up at 7 days after the procedure showed the patient's mouth opening was 1 cm. Conclusion: In this case, from subjective, objective, and supporting examinations, cause of infection has not been found. The treatment carried out is incision and drainage of the infection.

Abstract

Background: Submandibular abscess is an inflammation accompanied by pus formation in the submandibular region. Submandibular abscess ranks the highest of all deep neck abscesses. 70-85% of cases caused by dental infections are the most common cases, the rest are caused by sialadenitis, lymphadenitis, oral wall lacerations or mandibular fractures. The aim of this study was to determine the case profile of submandibular abscess caused by undetected mandibular fracture and its treatment.Method: The research method used was descriptive observational method. Data was taken from direct observation during the procedure and secondary data came from the patient's medical record in the oral surgery department of Sultan Fatah Demak Hospital.Outcome: Based on the case report, it was found that the treatment given to the patient had high success with minimal intervention.Conclusion: Submandibular abscess on the right lower jaw has been treated with incision and drainage so that the inflammatory products of anaerobic bacteria in the submandibular space can be removed properly.

Abstract

Tumor is a mass or abnormal growth of tissue in the body. Tumors are divided into 2 namely benign tumors and malignant tumors. Manifested by a lump, although some cases show ulceration, pain and bleeding. The size of the lesions ranged from 0.8 – 5.0 cm (mean 2.6 cm) and the lesions were usually asymptomatic. Primary Health Research (Riskesdas), the prevalence of tumors in Indonesia shows an increase from 1.4 per 1000 population in 2013 to 1.79 per 1000 population in 2018. A 62 year old male patient came to the Oral Surgery Clinic at Sultan Fatah Hospital, Demak Regency on April 28, 2023 with complaints of lumps on the edges of the tongue and gums the size of corn kernels, the color is similar to the surrounding area since the last 1-2 years. Examination of the tongue feels springy when touched, there is a wound at the edge of the mass, and it does not rupture easily but bleeds easily. Panoramic radiographic examination revealed a round opacity parallel to tooth 38. The provisional diagnosis obtained was benign neoplasm of the vestibule and multiple necrosis of the pulp of teeth 24, 27, 38, 43. The planned treatment was excision of the tumor as well as odontectomy of teeth 24, 27, 38 and 43 under general anesthesia.

Abstract

Submandibula abscess is a mixture of pus that collects in one area in a network, namely the submandibula area caused by bacterial infection, parasites, or other foreign bodies. The cause of submandibula infection generally originates from the mandibular first and second molar teeth from the spread of infection from the sublingual and submental spaces. Symptoms of a submandibula abscess are characterized by pain and swelling of the cheeks to the inner neck where infection occurs. Infection that occurs in the submandibula space originates from infection of the floor of the mouth, teeth, pharynx, and submandibula lymph nodes. A 49-year-old female patient came to the Sultan Fatah Hospital in Demak with clinical symptoms leading to a submandibula abscess, where there were complaints of swelling on the left cheek up to the neck, the remaining root teeth were often sore and loose, and shortness of breath. Radiological examination performed on the patient showed the impression of a left submandibula abscess. Laboratory tests obtained positive HbsAg indicating that the patient is infected with Hepatitis B. Hepatitis B is a disease that can worsen the condition of the immune system. Proper infection control and sterilization protocols must be followed to reduce the risk of infection.

Abstract

Background: Tooth extraction is a procedure that involves removing the tooth from the tooth socket. The wound healing process after tooth extraction ranges from 3 to 14 days. Wound healing can be treated with povidone iodine, but excessive use has side effects. Another alternative is red shoot leaves Methods: Experimental research with posttest only control group design. The sample was 30 male wistar rats, divided into 5 groups, namely positive control given povidone iodine, negative control base, 2% of Syzygium myrtifolium walp leaves extract, 4% of Syzygium myrtifolium walp leaves extract, and 6% of Syzygium myrtifolium walp leaves extract.. Results: Obtained a p value of 0.048 (p <0.05), indicating that there is a significant difference in the amount of epithelial thickness of all groups and the administration of Syzygium myrtifolium walp extract gel with a concentration of 6% on day 14 is thicker. Conclusion: Giving Syzygium myrtifolium walp leaves extract effect on wound healing after tooth extraction in male wistar rats.

Abstract

Belum adanya indikator penilaian efektivitas pengadaan obat dan bahan medis habis pakai (BMHP) di RSU Fastabiq Sehat PKU Muhammadiyah diduga menyebabkan kekosongan beberapa obat penting, seperti obat jantung, akibat keterlambatan pembayaran dan kekosongan stok distributor. Kondisi ini berdampak pada terganggunya pelayanan pasien. Diperlukan strategi pengadaan yang lebih efektif untuk meningkatkan mutu pelayanan kefarmasian dan menjamin ketersediaan obat. Penelitian ini bertujuan menilai efektivitas pengelolaan obat pada tahap pengadaan serta menyusun strategi pengadaan yang tepat di Instalasi Farmasi rumah sakit tersebut. Metode yang digunakan adalah campuran (mixed method) dengan desain Sequential Explanatory, diawali dengan analisis laporan pengadaan tahun 2024, lalu dilanjutkan wawancara mendalam terhadap tujuh informan. Hasil menunjukkan dana tersedia telah 100%, namun alokasi untuk obat hanya 15%, dengan tingkat kesesuaian pengadaan 89%, frekuensi pengadaan 42 kali, tanpa kesalahan faktur, serta keterlambatan pembayaran 21 kali. Disimpulkan bahwa efektivitas pengelolaan pengadaan masih perlu ditingkatkan melalui penetapan jadwal pembayaran, penentuan prioritas, penguatan basis data, pengaturan frekuensi pengadaan, dan digitalisasi sistem administrasi.

Abstract

Background: Benign tumors are collections of cells, tissues or organs that grow slowly and usually enlarge with peripheral expansion, pushing on surrounding structures. Squamous odontogenic tumor (SOT) is a very rare benign epithelial intraosseous tumor, which can cause arising from the neoplastic transformation of Malassez epithelial cells on the lateral surface of the periodontal ligament. Objective : To discuss the management of odontogenic squamous tumors using conservative surgical excision. The goal of treatment for oral myxoma was surgical excision. Case management : A 51 year old female patient came to the SULTAN FATAH Hospital Polyclinic, Demak Regency on May 26 2023 with complaints of a lump on the upper left gum of the jaw. The patient felt the lump about 1 year ago. The patient does not feel pain in the lump. Clinical examination showed a lump in the upper jaw on the front of the patient's left side.Conclusion : Management of the patient undergoing surgical excision of the tumor with general anesthesia and infiltration in the case of odontogenic squamous tumor, based on subjective examination the patient complained of a lump with a tumor mass measuring 10 x 8 cm, redder than the surrounding tissue, round, not moving, attached to the gums of the upper jaw.

Abstract

 Introduction: Maxillofacial trauma is an injury related to the face or jaw caused by physical force, foreign objects, or burns, including injury to any of the bone structures, skin, and soft tissue of the face. Every part of the face may be affected, the eyes and their muscles, nerves and blood vessels may be injured which can cause visual disturbances, diplopia, dislocation of the eyeball and the orbital bones can be fractured due to being hit by something strong. Case report: Initial examination carried out pain screening using the Numeric Rating Scale (NRS) method showing a score of 4. Assessment of the level of consciousness using the Glasgow Coma Scale (GCS) method showed a GCS result of 15 (E4 M6 V5). Vital signs examination showed blood pressure 134/78 mmHg, pulse 112x/minute, height 160 cm, weight 90 kg, respiration 22x/minute, temperature 36.4 C, oxygen saturation 98%. Physical examination of the patient found that the head was within normal limits, the lip mucosa was wet (+). In the oral and nasal cavities, sources of bleeding and loose teeth are visible. On eye examination, the skin was normal. Thorax examination was normal. The management carried out was administration of intravenous fluids (inf RL 20 tpm), then injection of 1 ampoule of ranitidine, 1 ampoule of ketorolac injection, 500 mg tranexamic acid injection. The patient was also referred to a dentist who specializes in oral surgery for further examination and treatment. Conclusion: The classification of the dentoalveolar fracture in this patient is based on the Ellis and Davey classification, as a class 7 fracture, namely a fracture of the tooth that causes a change in position or displacement of the tooth. This fracture causes loose teeth and injury to the gingival soft tissue. The ideal dentoalveolar fracture treatment in this patient uses interdental wiring so that the teeth can be maintained properly. The patient was also referred to a dentist who specializes in oral surgery for further examination and treatment.
