Abstract

Abstract: The introduction of autonomy as a fundamental important normative concept is probably the most important breakthrough in modern bioethics. However, when physicians and indeed, bioethicists, begin to assume that “autonomy is what we know is best for the patient”, it is doubtful whether in such a situation, autonomy as a principle is autonomy indeed and not some paternalistic conception of autonomy that is being imposed on patients. The main aim of this paper is to argue in favor of regarding autonomy as a choice. It argues that autonomy is a social concept, not an individual one. Therefore support for regarding it as a choice should naturally be found in autonomy’s social context, more specifically in its relationship with three major social determinants of contemporary medical practices, namely: freedom, culture and trust. Analysis of each of these relationships reveals the conclusion that the only “autonomous” way to foster autonomy is by regarding it as a choice. In other words, the first decision a patient should make is whether or not he wants to use his right for medical decision making. Kata Kunci: Otonomi, pilihan, kebebasan, kebudayaan, kepercayaan, tantangan modern

Abstract

Introduction: Cognitive function is essential for medical students to support learning at the university. One part of cognitive function that is very important in student learning is visual concentration. Visual concentration is the process of maintaining a concentration of thought on a visual object by putting aside other things that are not related. Concentration can be increased in several ways, one of which is to consume nutritious foods that contain flavonoids such as dark chocolate. The purpose of this study was to determine the effect of dark chocolate consumption on visual concentration of cognitive function in Atma Jaya’s medical students.Methods: This was an experimental study and the data were tested using t-paired statistical test. The research was conducted from March to April 2019 at Atma Jaya Catholic University of Indonesia School of Medicine and Health Sciences, Jakarta. The data ovtained from Stroop Test before and after intervention of dark chocolate. A total of 51 respondents were selected according to the research criteria.Results: As many as 48 people (94.1%) experinced an increase in the Stroop Test Score, a person (2%) did not experience a change in the Stroop Test Score, and two people (3.9%) experinced a decrease in the Stroop Test Score. The result of statistical test show significant results (p<0.01). The average score before the intervenion was obtained at 77.02±10.42 and the average score after the intervention was obtained at 77.02±11.81 (Δ= +12.26).Conclusion: There is an effect of dark chocolate consumption on visual concentration of cognitive function in Atma Jaya’s medical student

Abstract

The data were analyzed using Chi-Square.Results: Study on 110 subjects, consisted of 62 men(56.4%), aged divided by under of 58 years, and above of 58years were 52(47.3%) and 58 (52.7%). While subjects divided by stroke severity, i.e. minor, moderate, moderateto severe, and severe; 44 (40%), 58 (52.7%), 5 (4.5%), and 3 (2.7%) respectively. There were 77 participants(70%) had right side weakness and 33 participants (30%) were left side weakness. The changes of Barthel Indexof ADL score, were 73 (66.4%) improvement, 6 (5.5%) deterioration, and 31 (28.2%) remained. The study hasshown relationship between severity of stroke (p=0.008), weakness side (p=0.000), and physiotherapy onset(p=0.039) with the changes of Barthel Indexscore after treatment.Conclusion: There was a relationship between stroke severity, the weakness side of stroke, and physiotherapyonset with the better result of Barthel Index of ADL score after the physiotherapy.Keywords: Activity of Daily Living (ADL), Barthel Index, ischemic stroke, physiotherapy onset, stroke severity,Weakness side

Abstract

THE IMPACT OF GRANDPARENTING ACTIVITY ON COGNITIVE FUNCTION IN ELDERLY WOMANABSTRACT Introduction: Cognitive impairment is a neurodegenerative process which often experienced by the elderly. One of the factors associated with cognitive function is grandparenting which is common and has become a social norm in Indonesia.Aims: To determine the association between characteristics of the subjects and grandparenting activity with cognitive function in elderly woman.Methods: This study was conducted on July 2016–January 2017 in West Jakarta and as a part of Active Ageing Research in Atma Jaya University. The data were collected through grandparenting activity questionnaire and cognitive function assessment using Mini Mental State Examination (MMSE).Results: Based on 93 participants, the age mean was 67,51±5,058 and majority had education <9 years (54,8%). Data analysis showed that education, grandparenting, the quality of grandparenting, types of grandparenting activity specifically preparing food and/or feeding child and storytelling had a significant association with global cognitive function.Discussions: Physical, social activity, and cognitive stimulation while caring for grandchildren can prevent decline in cognitive function.Keywords: Elderly, cognitive function, grandparentingABSTRAK Pendahuluan: Penurunan fungsi kognitif merupakan suatu proses neurodegeneratif yang sering dialami oleh lansia. Salah satu faktor yang dianggap berpengaruh terhadap fungsi kognitif adalah aktivitas pengasuhan cucu yang masih umum dilakukan dan telah menjadi bagian dari norma masyarakat di Indonesia.Tujuan: Mengetahui hubungan antara karakteristik subjek dan aktivitas pengasuhan cucu terhadap fungsi kognitif pada perempuan lansia.Metode: Penelitian dilakukan pada bulan Juli 2016–Januari 2017 di Jakarta Barat dan merupakan bagian dari penelitian Active Ageing di Universitas Atma Jaya. Pengumpulan data dilakukan melalui pengisian kuesioner aktivitas pengasuhan cucu dan pengukuran fungsi kognitif menggunakan Mini Mental State Examination (MMSE).Hasil: Penelitian terhadap 93 subjek dengan rerata usia 67,51±5,058 dan sebagian besar berpendidikan <9 tahun (54,8%). Pendidikan, aktivitas pengasuhan cucu, kualitas pengasuhan cucu, serta jenis aktivitas pengasuhan cucu, yaitu menyiapkan dan/atau menyuapi makanan dan membacakan cerita memiliki hubungan yang bermakna dengan fungsi kognitif global.Diskusi: Aktivitas fisik, sosial dan stimulasi kognitif pada saat mengasuh cucu dapat mencegah terjadinya penurunanfungsi kognitif.Kata kunci: Aktivitas pengasuhan cucu, fungsi kognitif, lanjut usia

Abstract

   THE CORRELATION BETWEEN DISHAGIA AND ALTERED LEVEL OF CONSCIOUSNESS WITH ASPIRATION PNEUMONIA IN STROKE PATIENTS AT ATMA JAYA HOSPITALABSTRACTIntroduction: Approximately 40-96% of stroke patients will develop complications; the most frequent is pneumonia (33%). This is due to the underlying clinical manifestation of stroke such as dysphagia, immobilization, altered level of consciousness, and immune suppression which increases the risk of aspiration, therefore increases the risk for aspiration pneumonia. Early detection of aspiration pneumonia and its risk factors in stroke patients are essential, but the number of researches regarding this in Indonesia is still limited.Aims: To study the correlation between dysphagia and altered level of consciousness in stroke patients with aspiration pneumonia in Atma Jaya Hospital, Jakarta.Methods: This is a retrospective cohort research with a cross sectional approach towards 263 stroke patients in Atma Jaya Hospital, February 2016 until October 2017. Data was taken from the stroke registry and medical records, and analized with Chi-square test (α=0.05).Results: The incidence for aspiration pneumonia was 16% in the study population, most of which being male (57.8%), age <65 years old (79.5%), and length of education <9 years (71.5%). Age >65, dysphagia and altered level of consciousness were statistically significant with Odds ratio (OR) 2.26, 3.92, and 8.67, respectively.Discussion: Male, age <65 years old, and length of education <9 years were correlated with poor daily life habits which increases the risk of getting a stroke. Age >65 years old, dysphagia and altered level of consciousness can disturb the coordination and swallowing reflex which makes patients more vulnerable to aspiration and develop aspiration pneumonia.Keywords: Altered level of consciousness, aspiration pneumonia, dysphagia, strokeABSTRAKPendahuluan: Sebanyak 40-96% pasien stroke akan mengalami komplikasi, terutama pneumonia (33%). Hal ini dimungkinkan oleh adanya kecenderungan disfagia, imobilitas, penurunan kesadaran, dan supresi imunitas yang mempermudah terjadinya aspirasi hingga terjadi pneumonia aspirasi. Deteksi awal faktor risiko pneumonia aspirasi pada pasien stroke sangat penting, namun penelitiannya masih terbatas di Indonesia.Tujuan: Mengetahui hubungan antara disfagia dan penurunan kesadaran terhadap kejadian pneumonia aspirasi pada pasien stroke di RS Atma Jaya, Jakarta.Metode: Penelitian kohort retrospektif dengan pendekatan potong lintang terhadap 263 pasien stroke di RS Atma Jaya. Penelitian dilakukan mulai Februari 2016-Oktober 2017. Data diperoleh melalui data register stroke dan rekam medis. Analisis dilakukan menggunakan uji Chi-square dengan nilai α=0,05.Hasil: Insiden pneumonia aspirasi pada pasien stroke adalah 16%. Didapatkan mayoritas populasi laki-laki (57,8%) berusia <65 tahun (79,5%), dan berpendidikan <9 tahun (71,5%). Usia >65 tahun, disfagia, dan penurunan kesadaran memiliki hubungan yang bermakna secara statistik terhadap kejadian pneumonia aspirasi dengan rasio Odds (RO) 2,26, 3,92, dan 8,67 kali lipat.Diskusi: Laki-laki, usia <65 tahun, dan pendidikan <9 tahun berkaitan dengan pola kesehatan hidup buruk yang meningkatkan faktor risiko terjadinya stroke. Usia >65 tahun, disfagia, dan penurunan kesadaran dapat mengganggu koordinasi dan refleks menelan, sehingga pasien stroke lebih rentan terjadi aspirasi dan mengalami pneumonia aspirasi.Kata kunci:  Disfagia, penurunan kesadaran, pneumonia aspirasi, stroke

Abstract

      RELATIONSHIP OF MARITAL STATUS, APOE ε4, AND TYPES OF PHYSICAL ACTIVITIES ON COGNITIVE DECLINE IN ELDERLY WOMENABSTRACTIntroduction: The number of elderly population in Indonesia continues to increase every year, especially women. Elderly women are at higher risk of experiencing decreased cognitive function.Aims: To determine the relationship between the characteristics  of the subject, vascular risk factors, APOE ε4 genotype, and the type of physical activity with the decline of cognitive function in elderly women.Methods: A cognitive cohort study of 114 elderly women who had been observed for a mean of ±2.5 years, as part of the Active Aging study at Atma Jaya Catholic University. The independent variables of this study included were age, education, marital status, vascular risk factors (hypertension, diabetes, dyslipidemia, and BMI), APOE ε4, and types of physical activity. Cognitive function is measured using Mini Mental State Examination (MMSE).Results: There were 114 subjects with mean age was 71.22±7.297 and 33.3% experienced a decline in cognitive function. Age, marital status (single), and type of physical activity (cooking) have a significant relationship with Relative Risk (RR) of 4,45; 0,12; and 0,334.Discussions: Factors related to cognitive decline in elderly women were age. Lansia perempuan berisiko lebih tinggi mengalami penurunan fungsi kognitif.Tujuan: Mengetahui hubungan antara karakteristik subjek, faktor risiko vaskuler, genotip APOE ε4, dan jenis aktivitas fisik terhadap penurunan fungsi kognitif pada lansia perempuan.Metode: Penelitian kohort terhadap 114 lansia perempuan >60 tahun yang diikuti fungsi kognitifnya selama rerata ±2,5 tahun, sebagai bagian dari penelitian Active Aging di Universitas Katolik Atma Jaya. Variabel bebas penelitian ini meliputi usia, pendidikan, status perkawinan, faktor risiko vaskuler (hipertensi, diabetes, dislipidemia, dan IMT), APOE ε4, dan jenis aktivitas fisik. Fungsi kognitif diukur menggunakan Mini Mental State Examination (MMSE).Hasil: Didapatkan 114 subjek dengan rerata usia 71,2±7,3 dan 33,3% mengalami penurunan fungsi kognitif. Analisis multivariat menunjukkan usia, status perkawinan (single) , dan jenis aktivitas fisik (memasak) memiliki hubungan bermakna dengan risiko relatif (RR) masing-masing 4,46; 0,12; dan 0,334.Diskusi: Faktor risiko gangguan fungsi kognitif pada lansia perempuan adalah usia. Status perkawinan (single) dan aktivitas memasak merupakan faktor protektif.Kata kunci: Status perkawinan, APOE, aktivitas fisik, penurunan kognitif, lanjut usia

Abstract

Introduction: The prevalence of lower education elderly in Indonesia is still high, while low education is one of the significant risk factors in cognitive decline. Besides, many other factors are still controversial with cognitive decline. Aim: This study focuses on religious activities, vascular risk, apolipoprotein E (APOE) ε4, and their association with cognitive decline in the elderly who already have a vital risk factor: low education. Methods: This was a cohort study. There were 154 participants of lower education elderly, with the median follow up 2-3 years. The independent variables were religious activity, vascular risk, APOE ε4, and demographic characteristics. The dependent variable was cognitive decline, defined by lowered MMSE score ≥2 compared to the baseline data. Results: The mean age of the participants was 70.5±7.9 years old, with the majority being female (73.4%). Religious activity and cholesterol level had a significant association with cognitive decline in lower education elderly (p<0.05; RR=1.6; RR=1.5), other factors, such as APOE ε4, gender, age, marital status, hypertension, body mass index, diabetes mellitus, smoking, and depression showed no significant association (p>0,05). In multivariate analysis, cholesterol level and religious activity were associated with cognitive decline with a relative risk of 2.97 and 2.3, respectively. Discussion: High cholesterol level (≥200 mg/dL) and participation in religious activity were protective factors for cognitive decline in lower education elderly.
