Abstract

Introduction & Aim: Long-term care is provided for the elderly who experience limitations in carrying out independent physical activities. Caregivers as an important role in accompanying the elderly and families who experience limited independence due to chronic illnesses. Innovation is needed to equip caregivers to provide optimal health services. One of the long-term care has been shown by implementing SKT (Somporn Kantharadussadee Triamchaisri) for the elderly with chronic health conditions and physical limitations. The purpose of this activity is to disseminate SKT to the community through training for caregivers to achieve a better quality of life for the community in Pedungan Health Village. Method of Activity: The stages of this community project use the POAC concept approach, namely Planning by conducting assessments with partners regarding activities, Organizing through approaches with related parties, such as bringing in experts in the field of HCS from Thailand, conducting TOT at ITEKES Bali for the sustainability of HCS in Bali, readiness analysis caregivers in receiving SKT training. Actuating and Controlling conducted training for caregivers related to HCS, evaluation, and reflection which were carried out 4 times in June 2023. Results: These activity results showed that the results of the HCS training had an impact on changes in SpO2, blood pressure, and blood sugar over time. Discussion: This simple exercise pattern makes SKT a safe choice for those with chronic health conditions and physical limitations and for the elderly who need long-term care.

Abstract

Previous studies have shown that slow deep breathing techniques provide benefits in reducing anxiety in general, but there has been no specific review investigating its benefits in inpatient populations. Objective: To provide strong and up-to-date evidence on the effectiveness of slow deep breathing technique as a potential intervention in reducing anxiety in the inpatient population. Methods: This study is a literature review conducted during April-May 2023 using Google Scholar, PubMed, and ScienceDirect. The keywords used were "nafas dalam", "kecemasan", "rawat inap" in Indonesian, and "slow deep breathing," "anxiety," "inpatient care" in English. Results: The literature review identified eight articles that examined the anxiety of patients undergoing surgery and treatment in hospitals in Indonesia. These articles highlighted internal and external factors influencing anxiety. The dominant research approach employed experimental designs with pre-test and post-test measurements involving more than ten respondents. The study found that internal factors influencing anxiety included age, gender, education, health condition, and prior hospitalization experiences, while external factors included access to information, therapeutic communication, environment, and healthcare facilities. Conclusion: The implementation of slow deep breathing techniques is a major factor in reducing anxiety in inpatient populations. Further research is expected to emphasize the use of slow deep breathing techniques as a primary intervention in reducing anxiety levels in patients prior to undergoing surgery.

Abstract

Latar Belakang: Penyakit jantung koroner merupakan penyakit kronis yang menyebabkan tingkat rawat inap berulang yang cukup tinggi akibat kekambuhan penyakitnya. Berbagai alasan rawat inap berulang seperti masalah medis lainnya (komorbiditas) dan ketidakpatuhan terhadap rekomendasi kesehatan salah satunya kepatuhan terhadap terapi. Tujuan Penelitian: mengetahui korelasi antara kepatuhan terapi dan komorbiditas dengan kejadian rawat inap berulang pada pasien penyakit jantung koroner. Metode: Desain penelitian adalah deskriptif korelasi menggunakan pendekatan cross sectional dengan jumlah sampel sebanyak 61 pasien penyakit jantung koroner, menggunakan tehnik purposive sampling. Data di analisis dengan uji chi square menggunakan program SPSS. Hasil: rerata usia responden adalah 58.24 ± 5.43 tahun, dengan laki laki sebanyak 40 orang (65.6 %) dan tingkat pendidikan sebagian besar berpendidikan sekolah dasar yaitu sebanyak 27 orang (44.4 %). Terdapat hubungan yang signifikan antara kepatuhan program terapi (p value = 0.007) dan komorbiditas (p value = 0.024) dengan kejadian rawat inap berulang pada pasien penyakit jantung koroner. Kesimpulan: kepatuhan program terapi yang lebih baik memungkinkan menjaga kondisi pasien penyakit jantung coroner tetap terkontrol dan dengan adanya komorbiditas menyebabkan keterbatasan dalam perawatan diri sehingga diperlukan penanganan terhadap kondisi komorbiditasnya

Abstract

Hemodialysis (HD) patients with chronic kidney disease (CKD) lose muscle mass and strength due to protein energy wasting (PEW). The purpose of this study was to test the effectiveness of intradialytic resistance training (IRT) in increasing muscle mass and strength. It used a pre-experimental, one-group pretest-posttest design. Based on a sample size estimate using G*power software, 33 CKD patients undergoing HD at a private hospital in Bali were included. The patients were selected using purposive sampling. Muscle mass was measured using Bioelectrical Impedance Analysis (BIA), while muscle strength was measured using the Medical Research Council Muscle Scale. The Wilcoxon test evaluated IRT's efficacy on muscle strength, while the dependent samples t-test evaluated its impact on muscle mass. The results of this study found that IRT had no effect on muscle mass, with a mean difference of -0.330 (p = 0.274). On the contrary, IRT significantly improved muscle strength (p = 0.018). In conclusion, IRT proved beneficial for strengthening muscle but not for gaining muscle mass. Therefore, to improve muscle strength, patients with CKD undergoing HD are advised to actively perform IRT under supervision for a minimum of 30 minutes per day. Keywords: intradialytic resistance training, muscle mass, muscle strength, protein energy wasting   Abstrak Efektifitas Intradialytic Resistance Training Terhadap Massa dan Kekuatan Otot Pasien dengan Hemodialisis. Pasien hemodialisis (HD) dengan Penyakit Ginjal Kronis (PGK) kehilangan massa dan kekuatan otot yang disebabkan oleh Protein Energy Wasting (PEW). Tujuan penelitian ini adalah untuk menguji efektifitas Intradialytic Resistance Training (IRT) terhadap peningkatan massa dan kekuatan otot. Penelitian ini menggunakan pre-experimental one-group pretest, post-test design. Berdasarkan perhitungan besar sampel menggunakan G*Power Software, 33 pasien PGK yang menjalani HD di sebuah rumah sakit swasta di Bali terlibat dalam penelitian ini. Pasien dipilih menggunakan teknik purposive sampling. Massa otot diukur menggunakan Bioelectrical Impedance Analysis (BIA), sedangkan kekuatan otot menggunakan Medical Research Council Muscle Scale. Uji efektifitas kekuatan otot menggunakan Wilcoxon test, sedangkan t-test digunakan untuk menguji efektifitas massa otot. Hasil penelitian menunjukan IRT tidak efektif dalam meningkatkan massa otot, dengan mean -0,330 (p = 0,274). Namun, IRT terbukti efektif dalam mening-katkan kekuatan otot, (p = 0,018). Kesimpulannya, IRT bermanfaat untuk meningkatkan kekuatan otot, tetapi tidak untuk meningkatkan massa otot. Oleh karena itu, untuk meningkatkan kekuatan otot, pasien PGK yang menjalani HD disarankan aktif melakukan IRT dengan pengawasan minimal 30 menit dalam sehari.  Kata Kunci: intradialytic resistance training, kekuatan otot, massa otot, protein energy wasting
