Abstract

Maternal-fetal attachment (MFA) makes an essential contribution to maternal and fetal health and influences the adoption of a healthy lifestyle during pregnancy. Mothers who have a strong MFA tend to have healthy behaviors and give birth to healthy babies. Mothers who feel emotionally connected to their fetus tend to be more motivated to take care of their own health. They realize that their health directly affects the health of the developing baby. The purpose of this study is to provide an overview of maternal-fetal attachment in primigravida in the third trimester. The quantitative descriptive research method was used with 85 primigravida mothers who have met the inclusion criteria (third-trimester primigravida; no complications during pregnancy, self-examination at the Sleman regional public health centre) and have participated in this study. The Indonesian version of the Prenatal Attachment Inventory (PAI) questionnaire is used to review maternal-fetal attachments in a face-to-face interview. The results of the study provide an overview of the fact that most primigravida mothers in the third trimester have high maternal-fetal attachment scores. In addition, effective information communication and education for pregnant women were identified as factors that contributed to the level of maternal-fetal attachment. Effective maternal-fetal attachment-related programs in the education of new-reproductive couples are needed to be developed and implemented by health workers, including by midwives.

Abstract

The high rate of early marriages in Yogyakarta is partly driven by unplanned pregnancies (Kehamilan Tidak Diinginkan/KTD). KTD is influenced by several factors, including a lack of adolescent knowledge about reproductive health and environmental influences that shape attitudes, mindsets, and behaviors regarding adolescent sexuality. This Community Service Programme (PKM) aligns with government initiatives aimed at reducing and preventing early marriages, starting within educational settings. The programme's goal is to establish the Youth Reproductive Health Information Centre (PIKR-R) at SMP Muhammadiyah 7 Yogyakarta as a preventive measure against early marriages, while also enhancing students' knowledge of adolescent reproductive health. The focus of this PKM is the implementation of Health Technology Assessment through the establishment of the Youth Reproductive Health Information Centre, with the aim of preventing early marriages at SMP Muhammadiyah 7 Yogyakarta. Students from various professional backgrounds, including Midwifery, Psychology, and Communication Science, will be involved in the programme. This initiative supports three out of the eight Key Performance Indicators (KPIs) for Higher Education Institutions, including enabling lecturers to engage in off-campus activities, providing students with off-campus experiences, and applying research outcomes for community benefit, particularly at SMP Muhammadiyah 7 Yogyakarta.

Abstract

Background Stunting remains a major public health concern in many low- and middle-income countries, with long-term consequences for cognitive development, educational achievement, and economic productivity. Midwives, particularly in decentralized health systems like posyandu in Indonesia, play a vital role in early prevention efforts. However, limited research has explored their contextualized contributions within community-based health services. Objective This study aimed to explore the strategic roles of midwives in preventing stunting, focusing on their functions as healthcare providers, educators, and community facilitators. Methods A qualitative phenomenological study was conducted involving six community midwives selected via purposive sampling. Data were collected through semi-structured in-depth interviews and analyzed thematically. Triangulation was performed using additional informants including coordinating midwives, nutritionists, and community health workers. Results Four primary themes emerged: (1) midwifery care in stunting prevention; (2) community engagement and educational activities; (3) communication and health promotion strategies; and (4) identification of local health issues. Midwives were actively involved in growth monitoring, nutrition counseling, home visits, and cross-sectoral collaboration. Challenges included low community participation, persistent stigma, and limited awareness about stunting. Conclusion Community midwives play a strategic role in stunting prevention through education, early detection, and collaborative community engagement. Strengthening their communication skills, cultural competence, and integration with local stakeholders is essential for optimizing the impact of stunting prevention programs and ensuring sustainable public health outcomes.

Abstract

Dysmenorrhea, or menstrual pain, is one of the most common gynecological complaints among women of reproductive age, particularly among female students. Its impact extends beyond physical discomfort, affecting academic performance, psychological well-being, and overall quality of life. Despite its high prevalence, few studies have examined the determinants of menstrual pain severity among female medical students in Indonesia. This study aimed to identify the determinants of menstrual pain severity among female medical students at the Faculty of Medicine, Universitas Negeri Surabaya. A cross-sectional, descriptive quantitative study was conducted in November 2024, involving 112 participants selected through simple random sampling. Data were collected using a structured online questionnaire and analyzed with Spearman’s rho correlation and ordinal regression to assess associations between menstrual pain severity and independent variables, including body mass index (BMI), age at menarche, menstrual cycle characteristics, and exercise habits. Bivariate analysis indicated significant associations between menstrual pain severity and BMI (p = 0.004), age at menarche (p = 0.032), and exercise frequency (p = 0.038). However, in the adjusted multivariate regression model, only menstrual cycle length greater than 35 days remained significantly associated with increased pain severity (B = 1.143; p = 0.017). Other hypothesized predictors, including BMI, age at menarche, and exercise, did not retain independent effects after adjustment. In conclusion, irregular menstrual cycles—particularly very long cycles—were identified as the only independent determinant of increased menstrual pain severity among female students. These findings underscore the importance of considering menstrual cycle patterns when exploring risk factors for dysmenorrhea, while suggesting that commonly presumed predictors may not directly influence pain severity in this population.
