Abstract

Human Immunodeficiency Virus (HIV) merupakan satu dari dua human T-cell lymphotropic retrovirus yang utama. Virus tersebut akan menginfeksi dan menghancurkan limfosit T-helper (Cluster Differentiation 4-CD4), sehingga menyebabkan inang kehilangan imunitas seluler. Hal ini menyebabkan pasien mudah terkena infeksi oportunistik. Kondisi imunodefisiensi pada orang dengan HIV-AIDS (ODHA) menyebabkan prevalensi terinfeksi herpes zoster 15-25 kali lebih besar dibandingkan dengan populasi umum. Semakin rendah kadar CD4 pada tubuh, maka keparahan manifestasi kulit akan semakin meningkat. Tujuan laporan kasus ini adalah untuk membahas suatu kasus pria yang datang ke IGD RSUD. dr. Saiful Anwar dengan HIV stadium 4 disertai manifestasi pada mata dan kulit. Seorang pria berusia 40 tahun, datang dengan keluhan muncul luka bernanah di area wajah sejak sekitar 2 minggu lalu. Keluhan dirasakan nyeri seperti terbakar dan perih. Awalnya luka berbentuk bintik berisi air berukuran kecil pada area pelipis, kemudian makin membesar dan menyebar di seluruh wajah. Pasien juga mengeluh kedua matanya tampak merah. Satu tahun yang lalu pasien didiagnosis HIV dan hanya mengkonsumsi ARV sekitar 3 minggu. Hasil tes laboratorium menunjukkan hasil positif HIV menggunakan 3 metode dan nilai anti-HSV-1 IgG 135,4 U/ml. Pemeriksaan Gram menunjukkan adanya polymorphonuclear (PMN) multiple. Pemeriksaan Tzank menunjukkaan multinucleated giant cell dan sel akantalosis. Tes fluorescence juga menunjukkan hasil positif (+), menandakan adanya cacat pada epitel kornea. Pasien didiagnosa herpes zoster ophtalmicus dan diberi terapi Acyclovir 5x800 mg selama 7 hari. Pasien menunjukkan perbaikan selama diberikan pengobatan. Herpes zoster ophtalmicus dapat terjadi pada pasien HIV. Diagnosis dapat ditegakkan melalui penemuan klinis dan hasil tes fluorescence dan Tzank. Melalui diagnose yang memadai, pengobatan yang tepat dapat dilakukan sehingga dapat dicapai angka kesembuhan yang tinggi.

Abstract

BACKGROUND: There is currently insufficient study focusing on comprehensive studies that thoroughly assess the prevalence of Streptococcus pneumoniae specifically among elderly patients with pneumonia in Asia. OBJECTIVE: To analyze the prevalence of Streptococcus pneumoniae as a causative factor of pneumonia among elderly populations in Asian countries. METHODS: This meta-analysis was designed to systematically collect data from PubMed, Scopus, and Embase within the timeframe of January to February 2024. The study focused on gathering prevalence data of Streptococcus pneumoniae among elderly patients diagnosed with pneumonia in Asian countries. Statistical analysis employed a single-arm meta-analysis methodology to consolidate and evaluate the cumulative prevalence of Streptococcus pneumoniae in this specific population group. RESULTS: The total sample included in our study comprised 15 articles. Through our analysis, we found that the cumulative prevalence of Streptococcus pneumoniae among elderly patients diagnosed with pneumonia in Asia was 9% (Event rate: 0.09; 95% CI: 0.06, 0.12; Egger's test: p = 0.7047; p Heterogeneity < 0.0001; p < 0.0001). CONCLUSION: Streptococcus pneumoniae remains a plausible causative agent of pneumonia in elderly Asian populations.

Abstract

BACKGROUND: Patients with liver cirrhosis are highly vulnerable to psychological disorders; however, the implications of these mental health issues on the severity of esophageal varices remain unclear. OBJECTIVES: This study aimed to investigate the correlation between the grading of esophageal varices and the occurrence of anxiety and depression in cirrhotic patients experiencing upper gastrointestinal bleeding. METHODS: This study was conducted on inpatients at Dr. Saiful Anwar General Hospital, Malang, who were diagnosed with liver cirrhosis and experienced upper gastrointestinal bleeding over a six-month period using a consecutive sampling method. The grading of esophageal varices from endoscopic data was categorized according to the First Baveno International criteria. Anxiety and depression levels were assessed using the Hospital Anxiety and Depression Scale (HADS). Statistical analysis was performed using the Chi-Square test with a significance level of p-value < 0.05. RESULTS: Out of 53 respondents, 46 had large-grade esophageal varices and 7 had small-grade varices. Based on HADS scores, 38 respondents exhibited symptoms of anxiety, 10 were within the normal range, and 5 exhibited symptoms of depression. A significant moderate correlation was found between the grading of esophageal varices and the presence of anxiety and depression in cirrhotic patients with upper gastrointestinal bleeding (p = 0.001; r = 0.445). CONCLUSION: There is a significant correlation between large-grade esophageal varices and the occurrence of anxiety in cirrhotic patients experiencing upper gastrointestinal bleeding.
