Abstract

Introduction: Euthanasia is the most controversial ethical topic in the world of health. Nurses have an important role in providing holistic care for people with life-limiting conditions. However, they experience internal moral conflict and helplessness when patients ask them to assist in the dying process. This literature review aims to determine what is known about nurses' perceptions and attitudes toward euthanasia. Methods: The literature search was carried out using three databases, namely Google Scholar, PubMed, and Scient Direct. The inclusion criteria for the literature search were articles with publication years 2014-2022 in Indonesian or English and complete articles that could be open access. The keywords used in searching for articles were adjusted to the Medical Subject Heading (MeSH), namely knowledge, nurses' attitudes, euthanasia, and nursing euthanasia. Results: The literature review identified fifteen studies for inclusion in the final analysis. One article was semi-experimental, eight articles used a descriptive design with a cross-sectional approach, two articles used an epidemiological descriptive design, and two articles used descriptive-analytical methods. Nurses exhibit a range of attitudes towards euthanasia, including both positive and negative perspectives. Conclusion: The issue of euthanasia has presented a significant challenge for nurses in achieving their goal of providing holistic care. Their personal, professional, and transpersonal perspectives influence nurses' perceptions and attitudes. Potential implications for nurses relate to education, practice, and research. Nurses must be informed about existing laws and provided in-depth education and professional guidance to help guide action. Further research is needed to explore the impact on carers' emotional well-being, clarify their role, and determine the support they may require when engaging in euthanasia.

Abstract

Type 2 DM remains a challenge both globally and nationally, due to the high incidence of diabetic foot ulcers and patients with uncontrolled glycemia. This is due to poor self-care for foot care and glycemic control. Not treating them quickly will increase lifelong disability, depression, quality of life, and risk of death. Diabetic foot spa based on Orem Theory was tested for glycemic management and peripheral neuropathy in type 2 DM patients. The purpose of this study was to determine the effectiveness of diabetic foot spa based on Orem's Theory on glycemic control and reduction of peripheral neuropathy symptoms in patients with type 2 DM in the work area of East Denpasar Health Center I. This study used a pre-post quasi-experimental design with a control group of 64 respondents with a diagnosis of type 2 DM consisting of 32 control groups given foot exercises and 32 intervention groups given diabetic foot spa. Translated with DeepL.com (free version) Glucose meter, neuropathy symptom score (NSS) questionnaire, and 10 mg monofilament were used. This study consisted of univariate and bivariate analysis. Bivariate analysis used Dependent and Independent t-test to see the difference before and after diabetic foot spa intervention. The results showed that the effectiveness of diabetic foot spa in the intervention and control groups using independent t-test obtained a p-value <0.05 which states that diabetic foot spa is effective in controlling glycemia and reducing peripheral neuropathy in patients with type 2 DM. The conclusion of this study is that diabetic foot spa based on Orem's Theory is effective in controlling glycemic and reducing symptoms of peripheral neuropathy in patients with type 2 DM.

Abstract

Background: Hypertension is a common condition in primary health care, affecting approximately 25% of the adult population and more than 50% of those aged over 65 years. As the "silent killer of hypertension", it can cause stroke, which is a major cause of morbidity and mortality worldwide. This study aims to detect early signs of stroke using the BE-FAST method in the elderly in Karangasem, Bali, and to spread the effectiveness of this method in reducing the risk of stroke. Method: This community-based education program uses the POAC (Planning, Organizing, Actuating and Controlling) approach. A total of 79 elderly participants were recruited and underwent health examinations, including blood pressure measurements and the BE-FAST screening test. The BE-FAST method assesses balance, eyes, face, arms, speech, and timing to identify early signs of stroke. Results: The results showed that 55.7% of participants had hypertension, and 67.1% had difficulty moving their legs and arms on one side of their body. Additionally, 36.7% had difficulty opening their eyes or experienced vision problems, and 32.9% experienced facial weakness or numbness. The BE-FAST method is effective in detecting early signs of stroke, with a sensitivity of 92%. Conclusion: This study highlights the importance of early detection of stroke using the BE-FAST method in the elderly population. The research results show that this method is effective in identifying early signs of stroke and can be easily taught and applied in the community. Application of this method can reduce the risk of stroke and associated morbidity and mortality.
