Periodontitis is multifactorial inflamation process and related to disproportion of cytokine. IL-10 is a dominant noninflammatory cytokines that related to gene polymorphism. Polymorphism G-1082A IL-10 genes has been reported to increase the risk of periodontitis occurs in Italian populations, apart from different result found in Brazilian. The purpose of this research was to determine the polymorphism G-1082A IL-10 in periodontitis patients in Indonesia, especially among Yogyakarta’s Javanese. This is a case-control research with subjects according to the inclusion criteria. DNA was taken by cotton swab from the epithelial cells of buccal mucosa, and was isolated using a PrestoTM (GeneAid) kit. Genotyping analysis by using the PCR RFLP technique and descriptive results were presented. The result showed that A allele frequency is 100% and no G allele was found. AA genotype in case group has lower frequency than in control group and vice versa. From this research, it was concluded that A allele was dominant in Yogyakarta’s Javanese, and AA genotype frequency is lower in individual with periodontitis.

The genotype variation of the interleukin 4 (IL-4) gene on spot -590 C/T is known to correlate with IL-4 protein expression in periodontitis, but the results found in the population are inconsistent. Sequencial genotype analysis of multiple genes often used stored DNA samples obtained from buccal mucosal epithelial cells. This study aims to determine the use of stored DNA samples obtained from buccal mucosal epithelial cells for genotype analysis specifically on -590 C/T IL-4 genes. This is a descriptive analytical study. The samples were 58 DNA derived from the buccal mucosal epithelial cells taken by sterile cotton swab isolated with PrestoTM kit (GeneAid). The DNA has been stored for 2 years at -20 °C. The genotype analysis was conducted using PCR RFLP technique with BSmFI enzyme and the data were presented descriptively. The results show that 51.72% of the stored DNA is still in good condition and 37.9% can be used for genotype analysis. It was concluded that DNA derived from buccal mucosal epithelial cells stored at -20 °C for 2 years can still be used for genotype analysis, but the quality of DNA is affected by storage time.

Background: Both amniotic and platelet-rich fibrin membranes are used to enhance open wound healing process clinically. The growth factors of these membranes are almost similar but the certain mechanism is still unclear especially in inducing fibroblast proliferation. Objective: to compare the effect of both membranes to fibroblast proliferation. Methods: This in vitro study was using fibroblast from periodontal ligament. The fibroblasts were cultured and then divided into 3 groups: fibroblasts with PRF releasate membrane (group I), fibroblasts only (group II), and fibroblasts with amniotic membranes (group III). The cells were observed in 24, 48 and 72 hours. Proliferation was tested by MTT assay and data was analyzed by two-way ANOVA followed by post-hoc test. Results: It showed that PRF releasate membrane induced fibroblast proliferation higher than amniotic in the first 24 hours meanwhile amniotic induced it two-fold in the next 24 hours. However, in the following time there were no significant differences between groups. Conclusion: It can be concluded that PRF membranes and amniotic membranes have the ability to accelerate proliferation of fibroblasts but have different effects at the time of induction.

Background: Repair of bone damage represents a fundamental issue in the treatment of periodontitis. The important indicator employed to monitor the bone damage repair process is the number of osteoblast cells. Achatina Fulica snail mucin (SM) contains glycosaminoglycans which have the potential to increase their number. However, the use of SM in dentistry remains limited. Purpose: To determine and prove the effect of SM gel in increasing the number of osteoblasts in rat models suffering from periodontitis. Methods: This study used 36 rat models divided into three groups, namely; a treatment group (T: 20% snail mucin gel, n = 12), a positive-control group (P: hyaluronic acid gel, n = 12) and a negative-control group (N: CMC-Na gel, n = 12). 0.2 ml of all material was applied to a pocket by means of a tuberculin syringe once a day for 14 days. Histologic observations using Haematoxylin-Eosin staining were carried out on days 3, 5, 7 and 14. Data was analyzed by two-way ANOVA followed by a post-hoc LSD. Results: A significant difference existed between the number of osteoblasts in the test groups. The highest number of osteoblasts observed was consistently that in the treatment group. Conclusion: The application of 20% snail mucin gel was effective in enhancing the number of osteoblasts in rats suffering from periodontitis.

Background: The exposed roots of teeth due to gingival recession can generate dentin hypersensitivity and esthetic problems because a patient feels that the teeth, especially the anterior teeth, look long. Recession in thin anterior gingiva often brings unsatisfactory treatment results, so mucogingival surgery, such as vestibular incision subperiosteal tunneling access (VISTA) with the addition of a connective tissue graft (CTG), can be chosen as an appropriate treatment technique. Purpose: This case report describes the creeping attachment phenomenon after treatment of anterior gingival recession with VISTA and CTG techniques. Case: A 28-year-old female patient came with Miller Class I gingival recession in thin anterior gingiva and malposition teeth, complaining about pain and esthetic problems. Case management: The patient was treated with a VISTA technique combined with CTG. The creeping attachment phenomenon seen at three months postoperatively obtained good root coverage so that the patient’s complaints were resolved even though periodic control was needed to evaluate oral hygiene. Conclusion: VISTA and CTG techniques are appropriate for treating anterior mandibular gingival recession with minimal trauma and provide significant results.

Introduction: Optimal bone regeneration is contingent upon the activities of osteoblasts, which are modulated by signalling molecules. Propolis, an abundantly beneficial natural substance, has the potential to induce the release of signalling molecules. Review: Four essential elements required for tissue regeneration via tissue engineering: scaffolds, signalling molecules, stem cells, and vascularization. Subsequently, osteoblast differentiation and activity are enhanced by signalling molecules induced by propolis. Propolis contains active ingredients that influence every stage of osteoblast differentiation. Propolis-induced increases in the release of signalling molecules (BMP, FGF, TGF) and the transcription factor RUNX2 stimulate the expression of diverse osteoblastogenic markers at each stage of differentiation: type I alpha 1 collagen during proliferation, alkaline phosphatase during maturation, and osteocalcin during mineralization. Conclusion: Propolis has the capacity to promote bone regeneration through its involvement in all phases of osteoblast differentiation (inducing the release of signalling molecules, increasing the transcription factor, and increasing the expression of osteoblastogenic markers). Additional research is required to determine how propolis influences the equilibrium between osteoblast and osteoclast activities in order to maintain optimal bone homeostasis and remodelling. The prolonged duration of bone regeneration necessitates the use of a carrier for propolis to remain on bone defects for an extended period of time.

Introduction: Managing multiple gingival recessions in the aesthetic zone presents a clinical challenge, particularly in cases involving tooth malposition, extrusion, and irregular bone loss resulting from trauma. These factors complicate treatment selection. The Laterally Closed Tunnel (LCT) and Modified Coronally Advanced Tunnel (MCAT), both modified tunneling techniques, offer several advantages when combined with a Subepithelial Connective Tissue Graft (SCTG), including enhanced blood supply, improved graft nutrition, and reduced postoperative morbidity due to minimal vertical incisions and limited flap extension. This case report aims to demonstrate the effectiveness of this combined technique in treating multiple gingival recessions. Case report: A 20-year-old male patient presented with hypersensitivity and aesthetic concerns following accidental trauma. Clinical examination revealed Miller Class III/RT2 Cairo gingival recession on the buccal aspect of left maxillary canine and Miller Class I/RT1 Cairo gingival recession on the left maxillary first premolar, associated with canine malposition and extrusion, a recession depth of up to 7 mm, and inadequate keratinized tissue. The patient was treated using a combination of LCT and MCAT with SCTG. At the 3-month follow-up, substantial root coverage, increased soft tissue thickness, and improved gingival aesthetics were achieved, along with resolution of dentine hypersensitivity. Conclusion: The combination of LCT, MCAT, and SCTG techniques appears to be an effective treatment option for managing complex trauma-induced multiple gingival recessions associated with tooth malposition, providing favourable root coverage and improved soft tissue aesthetics.
