Abstract

Epulis often presents an aesthetic concern due to its location in the anterior region. The treatment involves eliminating any irritating factors, followed by surgical removal of the lesion. In young patients, a minimally invasive approach using a diode laser effectively reduces bleeding, accelerates healing, and alleviates discomfort. This report presents a 14-year-old male with the chief complaint of enlarged gums, which he had noticed three months prior. Intraoral examination showed the lesion to be localized in the interdental papilla between teeth 21 and 22, where a provisional diagnosis of epulis was established. The treatment was initiated with dental hygiene education, full mouth scaling, and polishing followed by epulis removal one week later using a diode laser, along with a diagnostic biopsy. Fourteen days postoperatively, an intraoral examination showed no residual epulis, a defined gingival margin, a pointed papilla, and a color blended with its surrounding tissue. The use of a diode laser for epulis removal in young patient proved to be an effective and minimally invasive treatment option. The results showed no evidence of residual epulis through normal probing depth and the absence of bleeding on probing. The patient expressed satisfaction with the treatment outcome.

Abstract

Introduction: Food intake is recognized as one of the key contributing factors to periodontal tissue disease. An open gingival embrasure (OGE) can result from various factors, including gingival biotype, poorly executed restorations, interproximal spacing, tooth loss, root angulation, the position of interproximal contacts, orthodontic treatment, and aging. Among minimally invasive approaches, the injection of concentrated platelet-rich fibrin (C-PRF) has shown potential for promoting periodontal tissue regeneration. Accordingly, this study aimed to analyze differences in fibroblast and blood vessel counts in the interdental papilla of Sprague–Dawley rats with experimentally induced OGE. Methods: This study employed a true experimental in vivo design involving 27 Sprague-Dawley rats, which were randomly allocated into three treatment groups. Each group received an injection of one of three test materials into the interdental papilla region: fixed-angle C-PRF, horizontally centrifuged C-PRF, or phosphate-buffered saline (PBS) as the control. Histological evaluations were then performed at three observation time points (days 1, 3, and 5), and the data were analyzed using two-way analysis of variance (ANOVA), followed by the least significant difference (LSD) post hoc test. Results: The horizontally centrifuged C-PRF group showed the greatest increase in fibroblast proliferation and blood vessel formation compared with the fixed-angle C-PRF and PBS groups. On day 5, the mean fibroblast counts were 239.06 ± 0.41, 200.60 ± 0.40, and 174.93 ± 0.30 in the horizontally centrifuged C-PRF, fixed-angle C-PRF, and PBS groups, respectively. The corresponding mean blood vessel counts were 43.33 ± 1.53, 37.67 ± 1.15, and 28.67 ± 0.58. Two-way ANOVA revealed significant differences among treatment groups and observation periods, as well as a significant interaction between treatment and observation period (p < 0.05). Conclusion: In a Sprague-Dawley rat model of experimentally induced OGE, the group receiving horizontally centrifuged C-PRF injections showed higher fibroblast and blood vessel counts than the fixed-angle C-PRF and PBS groups at all observation time points. These findings suggest that C-PRF prepared using the horizontal centrifugation method may have greater potential to promote periodontal tissue regeneration by enhancing fibroblast proliferation and angiogenesis within this experimental model.
