Abstract

BACKGROUND:Breast cancer is the most common cancer in Indonesia and is a leading cause of cancer-related deaths in the country. Metastases are responsible for most deaths among breast cancer patients. Breast cancer is typically classified into four subtypes based on immunohistochemistry: luminal A, luminal B, HER2+, and TNBC. OBJECTIVE:To determine the association between the metastatic pattern of breast cancer and their molecular subtypes at RSUD Dr. H. Abdul Moeloek Lampung. METHOD:This study was an observational analytic study with a cross-sectional design. The sample comprised 81 individuals who had suffered from breast cancer and were recorded in the medical records section of RSUD Dr. H. Abdul Moeloek Lampung between 2013 – 2021 and met the inclusion and exclusion criteria. The association between breast cancer subtypes and the incidence and metastase sites were tested using the Chi-Square test. RESULT:This study found a significant association between breast cancer subtypes and the incidence of metastases (p <0.05). The TNBC subtype had a more frequent rate of metastases (34.5%) than the other subtypes (31% luminal B, 20.7% luminal A, and HER2+ 13.8%). The study also revealed a relationship between ER status and the incidence of metastases (p < 0.05). The luminal A subtype tends to metastasize to the liver, luminal B tends to metastasize to the bone, HER2+ tends to metastasize to the lung and liver, and TNBC tends to metastasize to the lung. However, no significant association was found between breast cancer subtypes and metastatic sites. CONCLUSION:There was a significant association between breast cancer subtypes and the incidence of metastases, but not with the metastatic sites.

Abstract

This research describes the implementation of the school literacy movement at SDN 88 Pekanbaru. The type of research chosen is a qualitative approach, the instruments used are interview guidelines and observation guidelines. The results showed that the implementation of the school literacy movement (GLS) at SDN 88 Pekanbaru had not been carried out optimally. Literacy is a language skill possessed by a person in communicating reading, spe aking, listening and writing, one of the most common supports for literacy is the library, schools can implement a library visit program. The library visit program is an organized activity that is applied to students regularly every week with different classes to carry out literacy activities. Qualitative research is research that is descriptive in nature and tends to use analysis. Process and meaning (subject perspective) are emphasized in qualitative research. The theoretical basis is used as a guide so that the research focus is in accordance with the facts in the field. In addition, this theoretical basis is also useful for providing an overview of the research setting and as material for discussing research results.

Abstract

Congenital Heart Disease (CHD) is a congenital heart disease where this disease has been present since a person was born. In pregnancy there are significant physiological changes in the cardiovascular system. These hemodynamic changes pose problems for mothers with congenital heart disease. This study is a case report. Data was obtained from a 31 year old woman who came with complaints of preterm pregnancy with complaints of severe shortness of breath since 2 weeks before entering the hospital with CHD. On physical examination, it was found that the general condition was seriously ill, compost mentis, blood pressure 107/84 mmHg, pulse 130x/minute, respiration 39x/minute, temperature 36.6°C, SpO2 70% NRM 15 lpm. On chest examination, a continuous murmur and wet rhonchi were found. Hematological examination showed an increase in ESR, urea, creatinine, SGOT and SGPT. Chest X-ray showed suspicion of pulmonary edema, bronchopneumonia and cardiomegaly. The results of the echocardiography examination showed PDA balanced shunt, PH, Severe TR, Moderate PR, Mild MR, RV mass 0.5 x 0.9 cm susp Infective Endocarditis. The patient was diagnosed with G1P0A0 26 weeks pregnant, not yet in labor with Dyspenu ec CHF with Congenital Heart Disease (PDA), Intrauterine JTH. Management of this patient is observation of general condition and vital signs, IVFD RL 500 cc 20 tpm, injection of furosemide 2 ampoules, ISDN 3 x 5 mg. Then transferred to the intensive care unit (ICU) and then terminated the pregnancy abdominally. The patient was declared dead on the 2nd day of treatment due to cardiogenic shock.
